I’m a new LPN and they did not teach the Snellen chart in my course but very briefly went over what it was used for. This was very informative and to the point of learning it well. Enjoyed this video and would recommend to someone needing to learn how to use it! 👍🏼

Hi, that is a great question. In grade 4, I personally memorized the chart so I could avoid glasses, but I was busted when they swapped out for a different chart. What I will do if I suspect it is jump around and point to letters here and there. Finally, if I really suspect it, I might ask them to be checked by an optometrist, who will have a variety of charts, including projected ones, that can't be memorized. However, usually, I'm dealing with someone who has some sort of eye problem. They want to see and have any problems taken care of. So, I'm not usually caring for someone who is trying to avoid glasses or otherwise minimize any problems with vision. DK
This is a great question. If you suspect it, swap it out for a different chart. If you don't have one, then maybe the best option is to refer to an optometrist who will have all sorts of cheat-proof ways to check visual acuity. The great thing with having a physical chart is you as the clinician DO memorize it, which means you can look at the patient while they are reading it, and make sure they are doing it correctly (such as making sure they are fully covering the eye, etc.). Great questions! You can do 10 feet and then ensure you adjust (so if someone read the 20/10 line perfectly at 10 feet, it would become 20/20). Eye clinics will have fancy mirrors set up to give a longer distance too. The numbers on the right are so you can say, "Can you read line 8?" I find because they are on the right side, it is not intuitive for anyone.

when you ask a patient to read the letters, does this mean they should be able to see the letters crisp and clear? or just that they can see the letters a bit blurry but sufficient to read it? My understanding is that you want the vision to be able to read it without any blurriness?
Ideally no blurriness, but you score them based on what they answer, blurry or not. If someone needs glasses, an optometrist would try to get them to the right level of correction without over-shooting.
I did that in 5th grade and avoided glasses I have been lucky cuz I’ve been told I needed them for a month now and I just continue to sit in the front of the room and the nurses bad memory
Clever question! I usually start with the injured eye as they are less likely to see well with it. Starting with the "good" eye increases chance of someone remembering letters for when testing the injured eye.
Great video. The only thing different I was taught (when I was training in St. Louis) was if they only got two letters correct on a line then, as you said, call it the line above: 20/50 but give them credit for the two they got right. So it would be 20/50 +2. Maybe that's just a U.S. thing...

me tooooo I rmb they did an eye checkup at my school and I lied to the ppl and said I couldn’t see any of the letters and they sent a little home to my mother telling them I need to go to an eye doctor to get a checkup and my mother told them I can see and stuff bc she knew I was lying but they told her she needed to go so I went and I ended up getting glasses and now I wear glasses and I’m blind 🥲

Hi, Is this test an optimal test to check for double vision problems with one eye or is the black background and white letters more appropriate to check for double vision problems? I have gone to two eye doctors and they put me in front of a digital white screen with black letters; then , they precede to take measurements with the phoropter. When my new glasses were ready I noticed that I was having "ghosting of the image" aberration. I only notice this aberration when I look at a black background and white letters, but not in the case of white background and black letters.
I have refractive amblyopia in my right eye. (Doc said patching won’t work as I’m 24 now). Left is really good. I’m 20/20 on it but would fail my right eye at any distance. What would be my score?
Super smart question. In order for the test to be valid, it has to be the correct chart, on the correct type of non-glare paper, the correct size, and the person must stand the correct distance away. So, you can't really just print this out, stick it up and check one's vision reliably. Thanks for the question, DK

It's based on what someone can see, even if blurry. BUT an optometrist can fine-tune things usually on a glasses prescription so there no or minimal blurriness.

can the limit of strikes in total be more than 3 if for example there were 2 on on line and 2 on another is that to much or is it okay because 3 aren't on the same line. Does the total number of streaks matter?

Same thing just with the metric system. Instead of 20/20 for 20 feet, it's 6/6 for 6 meters. I believe 3/6 would mean you could see at 3 meters what the average person can see at 6 meters but it could be the opposite.
This video explained what I had often wondered about. What does 20/20 mean. So now I know that it's for each eye. One eye could be 20/20 and the other 20/30 ,etc. So, I guess someone with good vision in each eye is 20/20 20/20.
Hi, generally, I report the score for each eye, then report the score from when the patient does the chart with both eyes open. So it would be something like: R: 20/20 L: 20/30 Bilateral (both): 20/20-1
I have seen other Snellen charts as well where the sequence of letters are different from this one I think there are two three different Snellen charts to trick the patient , I wanna know is the sequence of letters is always the same for different Snellen charts or the doctor can change the letters the way he wants to??
Yes, we get sneaky. Plus I will often randomly point to different letter out of sequence. Some people fake eye exams, but that's a bad thing to do. We want our patients to have good vision, so we're a little sneaky to make sure our assessment is a reliable indicator of a patient's vision.
Hello, this is a good video. Last week I had my eyes checked and my final number written out by the eye doctor was -2 and -1.75. However I noticed that I wasn't able to read the last (tiniest) line at all despite the corrective glasses the doctor was slipping in. He finally said I didn't really need to be able to read the last line and then jotted down my number. Is this right or should I find another doc? and after that, the spectacles shop guy made a call to his factory person and said he could go ahead and make me specs with both lenses being -1.75 as they didn't have -2.
There is an odd tradition of not giving the patient the "full" prescription. Benevolently, it is to not give someone too strong a prescription that eyes would struggle against, but there are ways to prevent that. My great optometrist gives me a full prescription and I love having that sweet 20/13 vision (until my eyes change a bit).
@@dkcalgary I see. I thought that the doctor was just being lazy. I do remember now that over the years whenever I'd get spectacles made, some felt just meh while others felt crystal clear and sharp - s much so that I felt some new lease of life. I guess the "meh" glasses was where the guys decided to reduce/increase the number a bit because they didn't have my specific number. Thank you for your reply.
So OS 20/30 -1, can they move on to the next line since they only got one letter wrong and if the pt gets two letters wrong below that, Which would be the final acuity? The line below 20/30 or 20/30 -1?
No error, if they correct themselves. Most likely it's because they blinked, and the fresh tear film helped clear the vision from staring so hard at the previous letters. Our tear film accounts for 60% of what gives us clear vision. Good question, though!

My right eye is like... perfect vision but then my left eye is blurry with things farish away with things that my right eye can see.

I was expecting to see 20/20 or 20/200 or numbers the doctor said this is the official way to describe if you are unable to see even the worst case numbers. But I can’t find any reference to “hand motion” in any eyesight medical documentation.

sir, what is 6/6 & 3/6 eye sight? Because, I have seen such criteria in military & police recruitment.
Same thing just with the metric system. Instead of 20/20 for 20 feet, it's 6/6 for 6 meters. I believe 3/6 would mean you could see at 3 meters what the average person can see at 6 meters but it could be the opposite.
This video explained what I had often wondered about. What does 20/20 mean. So now I know that it's for each eye. One eye could be 20/20 and the other 20/30 ,etc. So, I guess someone with good vision in each eye is 20/20 20/20.
Hi, generally, I report the score for each eye, then report the score from when the patient does the chart with both eyes open. So it would be something like:
R: 20/20
L: 20/30
Bilateral (both): 20/20-1

I have seen other Snellen charts as well where the sequence of letters are different from this one I think there are two three different Snellen charts to trick the patient , I wanna know is the sequence of letters is always the same for different Snellen charts or the doctor can change the letters the way he wants to??
Yes, we get sneaky. Plus I will often randomly point to different letter out of sequence. Some people fake eye exams, but that's a bad thing to do. We want our patients to have good vision, so we're a little sneaky to make sure our assessment is a reliable indicator of a patient's vision.

Hello, this is a good video. Last week I had my eyes checked and my final number written out by the eye doctor was -2 and -1.75. However I noticed that I wasn't able to read the last (tiniest) line at all despite the corrective glasses the doctor was slipping in. He finally said I didn't really need to be able to read the last line and then jotted down my number. Is this right or should I find another doc? and after that, the spectacles shop guy made a call to his factory person and said he could go ahead and make me specs with both lenses being -1.75 as they didn't have -2.
There is an odd tradition of not giving the patient the "full" prescription. Benevolently, it is to not give someone too strong a prescription that eyes would struggle against, but there are ways to prevent that. My great optometrist gives me a full prescription and I love having that sweet 20/13 vision (until my eyes change a bit).
@@dkcalgary I see. I thought that the doctor was just being lazy. I do remember now that over the years whenever I'd get spectacles made, some felt just meh while others felt crystal clear and sharp - s much so that I felt some new lease of life. I guess the "meh" glasses was where the guys decided to reduce/increase the number a bit because they didn't have my specific number. Thank you for your reply.

So OS 20/30 -1, can they move on to the next line since they only got one letter wrong and if the pt gets two letters wrong below that, Which would be the final acuity? The line below 20/30 or 20/30 -1?

What if someone corrects the letter read out loud and gets it right after correcting their answer. Should that be counted as an error?
No error, if they correct themselves. Most likely it's because they blinked, and the fresh tear film helped clear the vision from staring so hard at the previous letters. Our tear film accounts for 60% of what gives us clear vision. Good question, though!

The "zed" comes from the Greek word "zeta" which is a more accurate name for the letter. It's only called "zee" in the US because the guy who wrote the alphabet song wanted it to rhyme lol
