BT sends a morse code signal thru Jack's helmet post credits-he isn't dead.

If you feel bummed about bt dying, just remember he transferred some of his ai into your helmet in effect and cause, hence the morse code message at the end of the credits.

When could he have? There wasn't any time to. The body doesn't function without the eye in there. He couldn't get into the cannon without the eye socketed, and he didn't have time to throw the sere kit with Jack during his last throw.
