As an expert the clamp shouldn’t be on the copper wires. The clamp has to be on the plastic sheath because when pulling it’ll expose more copper that will likely cause OSI Layer 1 problems. The only part of the modular plug that should be having contact with the copper wiring is the splices which sends electrical signals to the network.

If the jacket is cut too far back, the connector won't crimp onto it, and the only thing holding the connector on will be the wire connections. While it might work for a bit, these wires are easy to break, sometimes in a non-obvious way because only a single wire fails.
No. Normal patch cables use the following order with pin side up: OW, O, GW, Bl, BlW, G, BrW, Br. Basically the green white and blue white wires are swapped.
