Very sorry to hear about your problem. Keep in mind that cricopharyngeus spasm (CPS) does not affect voice directly. The sensation of the “lump” can become worse with voice use, and the sense of ease and comfort in using the voice diminish, but the actual quality and capabilities of the voice should not change. Lonaj Gamer Actually I only sleep 1 and a half to 2 hours at a time. I have a lot of health issues, so as tired as I am I have to get my sleep in segments. It’s worse when I first wake, or if I talk/sing. If I start to cry (for whatever reason) my throat gets really tight and I have trouble breathing. Apparently GERD (acid reflux) causes this. I’m going to see a specialist soon.

Hey I just was freaking myself out over this and I had the condition for a month and a half. I did a lot of research on my own and came to the conclusion that this was what I had. Then I finally just decided to go to a doctor and found out that it’s just globus, aka anxiety/stress. Working out and drinking a lot of liquids helped out quite a lot and it eventually does go away once you find a good outlet for releasing your stress (id really recommend excercise). Point being is that I diagnosed myself when I shouldn’t have (even though everything in this video is on point) and I finally just went to a doctor to have a medical professional tell me that it’s just anxiety. My point is to not worry about this being anything serious, just work to reduce stress and see a doctor if you’re that concerned!

I had been experiencing these symptoms for about 2 weeks. Repeated searching online turned up very little aside from cancer. While this made me very anxious about the condition, I kept feeling like it was something else. It felt more like a muscular tension than what I would expect from a tumour. After repeated searches, my terms changed a little bit and I ended up at this video. While watching, my symptoms immediately began to become less severe. Today, about 12 hours later, my symptoms are almost gone. I was really starting to stress myself out about it which was making it worse but since watching this video I feel much less anxiety about it. Now, to get some time off from work to take a vacation and hopefully clear it up all together. Thank you for this video... it helped a lot if not completely cured me :)

I experience anaphylaxis with certain foods and it causes dangerous swelling in the throat so when I get those symptoms I’m not sure if it’s allergies or this strange condition 😭anyways no worried you’re not alone! CPS might last that long; even so, symptoms fluctuate. Often the best treatment is a clear diagnosis: for many, this relieves anxiety and seems to start the process of resolution. Occasionally we suggest relaxation techniques, with help from a qualified SLP or PT. Muscle relaxation medication is another option, though I myself don't recommend this as continuous or ongoing treatment.

Often, once people become completely sure of the diagnosis (with the help of a personal physician), then they can begin to take the choking sensation and “throw it over their shoulder,” as with the feeling of a contact lens or a pair of glasses. It seems that, for many, breaking the cycle of worry is a powerful beginning to relief. (“Drat-there’s that crazy tight muscle again..." *Shrug*)

Thank you very much! My anxiety is already subsiding now after getting this explanation. The anxiety was clearly making it worse. At first I thought I was having an allergic reaction. But after it coming and going I finally looked up "lump in my throat". It feels like when you need to cry and you hold it back. I of course, at these times had no reason for this feeling. Then I thought my tongue was swelling but now I can tell that it is having a has time relaxing because I'm reacting to this lump feeling.

Thanks for your followup. It's impossible to sort this all out in this format. I would only say that the word "sensation" could be key, as there are disorders that entail a notable sensation of lump (e.g., CPS) without actual physical explanation or ominous significance. You veered in a different direction, though, with terms like gastroparesis and tachycardia, which are physical conditions. (Consider this informational, not medical advice. The video was very informative, but the advice at the end - go see your doctor about the problem - is useless. Most doctors - even ENTs - have never heard of this problem, and if you tell them that's what you think you have, they either won't listen or won't have the faintest idea how to treat it.

Most doctors look at us self-diagnosers as self-endangering fools, and rightly so (I could totally see "yeah, this heart attack I'm having is certainly irritable bowel syndrome!" happening every once in a while).

Cricopharyngeus spasm doesn't cause pain, but a feeling of pressure or a lump in the throat. And that feeling diminishes while eating. A symptom of true pain as you describe demands careful examination by an ENT doctor--especially if any history of sustained smoking or alcohol use. If this examination turns up nothing, we consider other less common diagnoses like hyoidynia, neuralgia, etc. (Consider this informational, not medical advice. I've had this for a couple of months. I too felt like I had a pill stuck in my throat. Mine is not that bad. I had neck surgery for the second time nine months ago and I thought it had something to do with that. I talked to my surgeon and he said throat mussels sometime does that but most of the time it goes away. I saw an ENT doctor in Montgomery Alabama. He told me he thought I had a cricopharyngeal spasm. Basically my throat mussel tightened up. He said there are were four things to try. I'm not going in depth with is explanations but here they are: (1) Leave it alone and see it if corrects itself. (2) Get a Botox injection to relax the mussel but sometimes you'll have to keep getting shots as if it does not fix all the time. (3) Stritch it with an endoscope type procedure. (4) Perform throat surgery where they where they make an incision in the throat mussel to make it relax/stitch the mussel. I just saw him last week and I haven't done anything as of yet. I am getting a routine colon scope next month and I told my gastroenterologist about my neck problem. He is from University of Alabama Birmingham and comes down to the Montgomery Clinic at Baptist South. He acted like he knew what was going on and he said he can perform a procedure to stritch my neck mussel. Of course not using the same endoscope. LOL I hope this information helps someone.

Thank you, this is very helpful. Ive been to the hospital twice for this condition, but nothing really came of it. None of the nurses/doctors seem to really know. Ive been given an appointment to get an endoscopy just to make sure its not something more serious (cancer) but im extremely doubtful of that. I believe its stress related.
All of the occasions I can remember this happening (maybe 5 times in my life) have been during times of extreme stress and anxiety. As well, I have tourettes syndrome, which causes nervous tics and gets worse with stress. However considering I have had tourettes my entire life and dont normally have this issue, I believe its incidental. I think stress is to blame. This video was extremely helpful, because it at least gave a more concrete medical explanation for whats happening. Usually I can only struggle to explain it to people, but this video has helpful terminology. It made me feel more relaxed, too, having a more clear answer.

I want to ask every body who watches this video something for research purposes. Have you ever had nasal problems? Do you have problem breathing through the nose? Most prominently do you have a deviated septum?
I know it sounds weird relating this to the nose but I have made a few observations since having Gerd and this feeling in particular. Some people believe that this problem is caused by trapped air in the neck. Well I got to thinking about how this air becomes trapped. Firstly I believe many of us are breathing primarily through our mouth instead of our nose (this definitely applies for me). Breathing issues, panic and anxiety can do this to you.
When we are breathing through our mouth (which is not natural) the mouth and throat become dry and I believe it creates this feeling.
Get back to me, especially if you have nasal issues or a deviated septum. It may show as a crooked nose on one side. It's just my theory but if I can see that others have these issues too then I believe it may hold credence.
    Thank you All,
May god bless and protect you.

I like the humor in your question… The importance of eating to the discussion of CPS is for its diagnostic input. I don’t really suggest eating as a therapy (though, of course, a person could divide normal caloric intake into multiple smaller meals). The main issue for most people is to get a diagnosis; once that happens, the problem often begins to resolve because worry drops away. (Consider this informational, not medical advice. I've been dealing with this since 2015/2016 and I was told because I have chronic sinusitis and the drainage was affecting my tonsils and the only time I noticed it happen was when I had lots of drainage from my nose to my throat. So I was given antibiotics and then it would go awayand repeat itself every so often. I finally went to an ENT and was advised to have my tonsils removed and I did and for a while I didn't feel it but today it's back. And I've searched high and low for answers but nothing really other than this and cancer (And I don't think that's the case). So I'll go back to my ENT this week and explain to him again and see what's going on. I'll keep you all in my prayers and hopefully find something to help this cause it's kind of irritating but I rather be alive and irritated.

They are in the “musculoskeletal” compartment of the neck, while CPS is a disorder of the “visceral” compartment of the neck-involved with airway, swallowing, and voice. I’m writing this quite late at night. I dare not to go to bed, even though I have to get to work early. The sensation gets much more suffocating if I go to bed when I`m not dead tired to sleep. So for the past to months, I`ve been trying everything from love making, smoking pot, breathing exercises, to gargling salt water, reading books while I`m havind a few glas of wine, and yet nothing has helped. And the books I`ve been reading could normaly knock anyone out like you where reading phonebooks. This is affecting the quality of my life. And I don`t know what is triggering this. I am a happy person.
But tonight, after watching this video, I feel that there might be hope. I feel that the doctor is exactly describing my symptoms. And that is the most important thing before a treatment. I`m so greatfull to the doctor. I’ve been so desperate lately as to seek help with quacks and people I normally resent.
I`m going to show this video to my doctor and see what he will suggest.

Cricopharyngeus spasm (CPS) is not life-threatening. As to your "choking" symptom, that may or may not fit CPS; CPS does NOT cause breathing difficulty or lodgment of food, but just a sensation of constriction. For many, the most helpful treatment is to just get a clear diagnosis and be relieved that it is nothing serious or life-threatening. Hope you get a clear diagnosis soon! Your description is still fairly classic for CPS. If in fact your personal physician confirms the diagnosis, and assuming the voice’s capability/ limitation profile is normal, he or she would not require cessation of singing as a treatment. He or she would tell you that it might feel uncomfortable to sing, but there would be no damage to the voice if you sang anyway.

I think I may have something similar to this. It started about 4 months ago. It doesn't feel like a 'lump' more like someone is pressing on my larynx, that feeling you get just as you are about to cry. It really tightens up and feels very heavy. Trouble is I am a singer and I have had to put everything on hold. It comes and goes, better in the morning and worse throughout the day. Fine when eating. Doctor checked and thought I was fine but sending me for blood tests anyway. It's horrible.

Standard treatment begins with detailed teaching to understand the nature of the problem. Next would be seeing a well-versed speech pathologist for anterior neck relaxation exercises. Then, if still a problem, consider Botox injection (rarely needed in my experience). I might next try a “neuralgia” medication like amitriptyline or gabapentin. Hi! Love this video. Very informative. I've have been searching for such information for 5 years now. Does this problem also affects speech or singing? It seems to be triggered immediately in my throat when I eat anything oily, specially affecting my voice. I have lot of trouble speaking/ singing including all the troubled feeling mention in this video. Could Circophayrngeal Spasm precisely be a reason to this sensation?

unfortunately I got some mild tinnitus on my left ear a week ago, now, the sound isn't bothersome and i forget it completely during the day, but apparantly my throat began to feel tight a couple of days later after i got tinnitus, is there a link between the fullness i feel in my ear and the contraction (i feel a pressure difference between the sorrounding air and my inner ear)

Many people use the term “pain” initially but can agree after careful review that the sensation is more in the realm of pressure/constriction/golf ball in throat. About half of my patients report having unusual stress, and I think the mechanism here is not unlike getting tight posterior neck and shoulder muscles. For a large group, however, CPS seems to just happen out of the blue.

Thank You!!! I got the flu, and been coughing like crazy, think I may have pulled something, but I also have a history of reflux, which I read can affect the cartilage/muscle in this area. Maybe the flu and coughing cause the extra strain. Went to emerg, and doc just checked for swollen tonsils and glands, but nothing. Once he concluded I had the flu he just wanted me out of there as fast as he could. Yet I knew this was no longer just a simple viral infection. THANK YOU!!!

I've had symptoms for about 3 weeks now. Yesterday I felt pretty much perfectly fine, but today when I tried to go hiking it hit me hard. It feels hard to breathe almost with it and is still there when I am completely relaxed. I live at almost sea level but have been in Colorado for the past few weeks (which is when this started). How do I know whether this is altitude sickness or a crycopharyngal spasm? Also, how long will it last, and what should I do. Water helps a bit, but hasn't completely gotten rid of it. I want nothing more but to feel fine again

@Combatarms420 This is an informational (not specific medical advice) response to several prior posts. CPS does not cause pain, nor does it interfere with ability to swallow. Eating reduces or abolishes the sensation, if transiently. This is an issue with increased muscle tone causing SENSATION of lump. Eating food works fine, and seems like "therapy," There is another dx of CPDysfunction (CPD, not CPM--they are different). Refer to CPD video too. And above all, see your doctor!

Symptoms of choking or lodgment, constriction, pressure, etc. are in my experience related to muscle tension such as caused by hyper-tonicity of the upper esophageal sphincter (cricopharyngeus muscle). LPR is more related to morning sore throat, deep morning voice, increased morning mucus, etc. (Consider this informational, not medical advice. Don’t act without input from your personal physician.)

  • @Laryngopedia
    @Laryngopedia  11 ปีที่แล้ว

Hiatal hernia is very common, and I have not noticed that such individuals have throat symptoms associated with it. Instead, I would think about the possibility of cricopharyngeus spasm. If the sensation is variable--it feels like a blockage but eating makes it feel better--then it is likely just that. Of course, you would want your personal physician to verify.

LERD or LPR (silent reflux) can also cause globus sensation if you have a lot of throat mucous or post nasal drip. Force a yawn to relax the muscle. Do not hack up the phlem or try to clear your throat or the globus gets worse. Instead open your mouth and exhale air hard out the mouth to relieve the mucous. I used a nasal saline wash with distilled water daily for a week plus saline spray twice a day and it helps clear out the mucous. Research LERD online for more tips on clearing your LERD symptoms..

What you have described sounds awfully similar to what i am experiencing. But i am curious, what causes this issue? Can it be cause by trauma to the neck, infections, or just happens to some people for other reasons? Thank you for this video it has been much help. I'v been in constant pain for about a year now and I can hardly take it anymore. I was tired of getting "nothing is wrong" for an answer and decided to start doing research myself.

  • @Laryngopedia

Sorry for your distress. I have seen CPS occur in three primary contexts: In some, it occurs out of the blue with no discernible explanation. In others (a fairly large group) it seems to correlate with life stress, though CPS often starts after some days or weeks of “coping” with that life stress. In a third very small group, it seems to have started after a painful swallowing episode--meat stuck transiently in the throat, for example. (continued in next post...)

    Here's my personal experience.
Last night I was working out like around 11 : 00 pm. And then something close to my heart started to feel really bad like if I was about to have a heart attack but then a minute later I felt better. Now here's the part where things changed. As I was about to go to sleep, I felt this uncomfortable thing inside my throat and I have trouble talking, breathing, drinking sometimes, moving my neck around, and it feels like I about to choke. I hope all of you and myself feel better.

I've had this for about 3 years now where I all of the sudden felt a lump in my throat while walking to school one morning I was 18 at the time. It caused me to gag, at uncontrollable times before I learned that if I suck in air and make myself burp I can be a little more in control of when and where it happens. (I had to learn how to burp I never could before) however the burp sounds like I'm throwing up very loud and disgusting. This shoots out a large amount of air and relieves the symptom for a short while until I eat again. It is rather embarrassing if someone hears me.
Anyhow I have been for a barium swallow, chest x-ray, scoped down throat and still haven't found a conclusion. It is usually flared up more in the morning. But happens after I eat. Definitely flares up when I am nervous.
I am now on anti depressants having found myself in a real melancholic state of mind from this occurence very withdrawn from how I had used to be. But the anti depressants really changed me back to feeling like my old self. I take anti anxiety medication when I go out to socialize which is rare because I have become anti-social and don't get out much because of fear of embarrassment.
Nexium helped with my stomach feeling terrible each and every day in the first year, and I don't have to wake up feeling like complete shit everyday. I don't take nexium anymore did not help with lump feeling. I was diagnosed with GERD but I never really have heartburn just the lump feeling.
I sometimes take teva-domeperidone 10 mg before meals it seems to help sometimes.
I developed IBS which was brutal I won't go in to detail. Now I use Align and that really proved effective.
I think the lump in my throat is the root of all my problems, it is the reason I worry and barely get out anymore. I gave up trying to find an answer for a while sick of doctors looking at me sideways when I tell my story. I can't help but feel that my quality of life is not as it should be.

So the more anxious i get anout trying to be normal the worse i feel so i can barely go out anymore until i solve this problem. Its been over a year now everyday i have this crap in my throat it makes me clear my throat constantly but there's no relief ever.
