I believe this has been addressed in the Wakanda Files; Shuri states the reason for Red Skull's deformity is due to him taking the Super Soldier Serum without also using the Vita Rays. Nothing to do with bacteria or the absence of Penicillin.

And more importantly, it amplifies the defining characteristics of anyone who takes it. "Good becomes great. Bad...becomes worse". Essentially, the unperfected Serum made Schmidt, who was already a High Functioning Sociopath, as physically ugly as he was mentally twisted.
I always figured that it was because Red Skull took an early version of the Serum. Remember, Eckstein himself said that "The Serum was not ready, but more important, the man." Which, again, does imply that the serum that Red Skull used was in fact an earlier version that didn't have all the kinks worked out.
So this theory is essentially the same as why Deadpool has his face, that his healing factor is constantly fighting a more aggressive issue, in Deadpool's case Cancer but with Red Skull its Bacteria? A bit too convenient if you ask me. I would understand if the explanation was that the earlier form of the serum stimulates the cells into a hyper form of keotsis causing inflammation of the cells like that of a person overheating.
bucky's serum is derived from the red skull's DNA but it looks like zola modified/improved it because bucky didn't turn into a monster (he turned out to be a physical pair of Steve's) I think the only imperfection of the zola serum is that it takes time to take effect and it doesn't seem to improve mental processing only physical 🤔

I was thinking yesterday since red skull no longer needs to protect the soul stone on vormir could he somehow make his way back to earth and begin to cause trouble once again? I would like to see it
My question is how come he didn’t just vanish with thanos while thanos had the space stone and just gotten ahold of the soul stone red skull just decided to stay on vormir? Also him making his way back to earth to fight sam and bucky while trying to rebuild hydra in the present would make for a good storyline
It's a good theory at least for the MCU. Of course if this is true then why did steve age the way he did in End game? Cause the theory stands on the basis that the super soldier serum enhances the body on a cellular level. Now in the 616 comics they did a stint with old man Captain America where Steve lost his serum effects and he "instantly" aged to close his actual physical age. However in the Marvel Multiverse things don't always work the same way so unless they spill the beans it's very much an open ended question.

I mean....It didn't just deform Red Skull. Emil Blonsky was also experiencing deformities due to the Serum injected on him, which is why he was looking sickly, and his spine protruding. Combine that with excessive Gamma radiation like the Hulk, and you get an Abomination. Anyways, I agree it comes down to the version of the serum utilized. Where the one used by Red Skull and Blonsky may have had negative effects due to the serums make up and even their Psychological profiles. Perhaps Hydras version was highly altered after recovered by Winter Soldier to avoid the negative characteristics of the soliders altering them. We'll see how much Falcon and The Winter Soldier explore this. Can't wait.

I think that theory is completely wrong. Red Skull's deformity comes from the fact that he didn't have Erskine's serum and injected himself with an unfinished copy of the real super soldier serum.
The thing is, his 'head' resembled a leather mask so much I'm half-convinced it was an infection-proof, sealed mask covering his entire head. No way could any chemical concoction have turned his entire head that red.

Bucky was already a trained soldier with a good heart thats why they had to brainwash bucky while the serum also gave bucky super strength
