New Initiatives for DIAGNOSIS of Tuberculosis under RNTCP .

In this video I shall talk about new initiatives for diagnosis of TB under RNTCP based on new guidelines. CBNAAT or Genexpert has been introduced for diagnosis of TB particularly drug resistant TB. The turnover time for CBNAAT is 2 hours. Indications for CBNAAT are PLHIV, Paediatric case, extrapulmonary TB sites, non responders of TB treatment, previously treated TB cases, Drug resistant TB contacts, TB HIV coinfection.

A very nice presentation indeed Dr. , however I would like to point out one thing, regarding the Turn Around Time of CBNAAT i.e. the Genexpert MTB/RIF being 2 hrs!!
Yes, the GeneXpert system can provide the results in 2 hrs after the sample-reagent mixture is loaded on the cartridge and the Xpert machine. However, when mentioning the TAT, we must keep in mind about the other factors, such as:
-the testing capacity of the lab per day- depends on the no. of working modules in the lab,
-no of the sample presented in the site,
- pre-analytical time for sample processing time,
- Note: sputum with presence of excess amount of blood isn't acceptable, as the CBNAAT will generally give errors on such samples.
we need to take into account of all the things that goes on in the process.
So It needs to be clear that after the sample-reagent mixture placed in the cartridge is inserted in the GX machine, then it will take 2 hours to generate the report, and we need to account the above mentioned variables, hence it is best to mention the TAT of GeneXpert to be minimum of 3-4 hours to possibly within the next day.

I am mentioning this because of the fact that few health professionals themselves mention that the reports will be ready in 2 hrs which is n't true and it creates confusion among the patients as they expect the results within 2 hours.
So future Medical Dr.'s kindly note that at early the report of GX(if given ar 1st hr can come at early within the end of the day or the day tomorrow, depending on the facilities of the testing site.

Ma'am your mode of explaining the components and their sub class is worth appreciable. However, I have a concern that what are the other tests which can be considered in case of extra pulmonary, specifically in case of pediatric?

Mam why do we bother with culture? Anyway we have to know whether the bacilli is monoresistant or mdr or xdr before starting the treatment for which anyway we have to go for CBNAAT and LPA, then why culture is included in the algorithm ?

Rifampicin sensitive means TB+ and sensitive to first line drugs , Rifampicin resistant means MDR TB, Rifampicin Indeterminate means repeat CBNAAT again to confirm
