      I agree with the pilot. Tower told him "taxi H, hold short of taxiway Q." Without prepositions, Tower really means taxi ON H and hold short BEFORE Q, pilot hears it as taxi on H and hold short AT Q thinking he's suppose to turn at Q. If you look at the chart there are lines for both positions. Saying "hold short of taxiway Q" doesn't sound like hold at the line on H but rather hold at the line on Q which he did.
      Really is bad airport design. Why would anyone hold on H in the middle of another taxiway? Logically the line should be placed before Q to avoid any confusion. But everyone thinks the pilot is wrong and agrees with ATC.

      A word of advice to you operators on the common frequency. Controllers do not need you to back them up. They normally make their own judgement based on the total nature of the incident. As the operator of another aircraft in the area you have limited knowledge about incidents occurring with other aircraft, you also do not pay as close attention to instructions, as if they were issued to your aircraft. Stay off the frequency with the tidbits of agreement or descension. I defer to controllers even if they make errors or did not clearly issue instructions because although we are both busy bees. having to convert each intention into words is a little more labor intensive than raising a flap handle or turning on the landing lights.

Basically it means "we have to bury this guy on the public dime because neither he nor anyone else in his life has the wherewithal to bury him." Most indigent people - like, upwards of 90% - are not homeless; they're mainly the disabled, unmarried or widowed elderly, minimum wage workers, escapees from abuse, etc.

      Good CRM with a two pilot crew will have the captain read back the ATC instructions in the flight deck and maybe even jot them down on their scratch paper or mark their efb ground chart. I have been asked to copy "a number" and ask for a supervisor to pull the tapes. It can save embarrassment and a little additional training to maintain safety is a good thing.

    • @rnzoli
      @rnzoli 2 ปีที่แล้ว

      @@AzimuthAviation I agree. I think the PM (pilot monitoring) read back corectly, but the PF (pilot flying) didn't confirm the same and acted differently due to wrong habits or distractions. Hard to say this without listening to the QAR/CVR after the completion of the flight. This is a little bit of a disadvantage for the flight crews you know :) In the ATC facility that I visited, there was a piece of equipment, that stored the sound samples for up to the 20 latest instructions on the frequency. So with a quick push of these buttons, the ATCO's could immediately re-play and verify their own understanding of what was being said on the frequency recently. Obviously this ATCO didn't need this, but the pilots of AA58 would have been better off having such functionality before starting an agitated discussion....

    The pilot is thinking "hold short [of 25R] AT Quebec" rather than "hold short of Quebec". Holding short of a crossing taxiway when you are dealing with a nearby runway, and you're trying to GET to a runway can mess with your brain. The pilot is saying one thing, but is visualizing another. The controller should have said, "the instruction was to hold short OF Quebec, which you have read back now, twice, but you are ON quebec short of 25R." That would've woken up the pilot who couldn't get past his own expectation bias. This is perfect case study in human factors, imo.

"Hold short of taxiway Quebec, this frequency"- Not his fault the American was only listening with half an ear, and then doubled down instead of saying, "my bad" My point here is: The ATC got agitated, yes, but I feel with a little justification. The pilot made a mistake, and through arrogance, made it worse! That attitude was a part of this whole problem. Had the pilot simply said "I think I heard it differently tower. Where do you need me now?" this would never had made video.

      Disagree, ATC gave very clear instructions at the very beginning. "Taxi H, HOLD SHORT of Q, this frequency". Pilot got all three major pieces of information , where he is and what he needs to do, plus frequency. Instead they almost got onto an active runway, which definitely would have been a phone number to copy later. These guys have thousands of taxis done over the years and they know the drill, perhaps routine got them as they were used to what they actually committed to taxi from a few repeated times before. But if you are unsure of something, or get something new and don't understand it, request to say again, consult taxi chart, ask your copilot and if you mess up, don't argue, correct and debrief later. 10,000 hours or 1000 hours, talking and actively listening to ATC is still the same.

      But no runway number was given (nor read back), so it still wouldn't make sense. They can't tell you to hold short of a runway without using the words "runway" plus the runway number.

    • @Joe-ef7sk
      @Joe-ef7sk 2 ปีที่แล้ว +1

      That may be what he heard in his brain, but it still shouldn't have made sense.. Holding short "at" Quebec would put them holding short of a runway. Runway hold short instructions (or cross instructions) *always* contain the runway identifier, and readback is *always* supposed to contain the runway identifier. The pilots just simply weren't paying attention, and confirmation bias took over -- they had their expected taxi route, and that's what the executed, regardless of what the controller said.

    I’m not sure if this is an asset or a liability, but my default reaction to a challenge like this from ATC is (internally, in my mind) “Oh, Geez….where did I f$&@ up? Oh, man, I hope this isn’t serious.” For some pilots, their initial reaction is “No way I could have screwed this up. It HAS to be ATC’s fault….not mine.”
    Self confidence and self assuredness is good….until it becomes hubris.

    I am amazed at how many pilots can't get this instruction correct. Clearly they are under the impression that "hold short of Q" means "hold short of the runway ON Q", when they are two different clearances. This type of confusion happens surprisingly often.

    • @coma13794
      @coma13794 2 ปีที่แล้ว

      I'm thinking it's because the pilot was expecting to be given a rwy crossing instruction, or at worst, he'd hold short of the rwy at Quebec. So, when he was issued "hold short of quebec," he compared it to the "hold short at quebec" that was already fallback plan, and his mind made it work. I know that's a lot of guessing on my part, but it's the only logical explanation that would give an airline pilot this much confidence while still clearly screwing up.

    Just speculating here, but maybe the pilot interprets “hold short” instructions as meaning runway vs “stop short” instructions as meaning taxiway. Confirmation bias could have caused him to interpret “hold short at Quebec” as “hold short runway 25R at Quebec”. It might explain the right turn towards the runway hold short line. Not condoning what happened but watch it again with that in mind.

    • @_OpdeeMist
      @_OpdeeMist 2 ปีที่แล้ว

      I do appreciate your speculation as it indicates that you like to see the good in people. I would however have to disagree with your optimistic reasoning. That is the reason standard terminology is used so there is no confusion. The instruction/clearance was On Hotel hold short of Quebec remain on this frequency. The Quebec hold short line was missed. He and in particular the PF should have asked to confirm if in any doubt of the instruction. Obviously the PF was also not doing his/her job showing a multitude of errors. Poor SA, I would assume not following SOPs, and not listening or requesting a confirmation. Aviation tries to reduce the concept of speculation especially with communication. That’s how incidents/accidents happen.
      You’re 100% with regard to confirmation bias…. That’s another reason for two independent thinking crew members actively listening. We’ve all made mistakes thru either fatigue, or other Human factors but a real man and a good crew member will admit a mistake and try and learn from it. Not double down and show the world what a mediocre pilot and person he is….
