First Question :
Provisional diagnosis: pharyngeal pituitary gland
Cause: maybe the patient had a congenital anomaly causing failure of ascend of rahtks pouch leading to pharyngeal pituitary gland so during the operation, may be the doctor had removed the anterior lobe of pituitary gland by mistake instead of removing the adenoid which lie also the in the roof of the nasopharynx. As a result, the patient had hormonal disturbance due to loss of secretion of hormones from anterior lobe.

Second Question:
Procedure should be taken: MRI scan of the brain
Provisional diagnosis: pituitary gland tumor
Cause:
Due to the abnormal high level of prolactin released , the pituitary gland may have a tumor or swelling that affects its hormonal function so in case of pituitary tumor, the optic nerves (optic chiasma) which lie above the gland may be compressed causing visual field disturbances such as bitemporal hemianopia. so MRI scaning of brain will assist in this case.

The answer of question 1
This is due to the presence of congenital anomaly which is called pharyngeal pituitary gland due to failure of ascend of rathkes pouch so the gland is formed in nasopharynx and it is removed by mistake during the operation which leads to this hormonal disturbance as pituitary gland is the master gland which secretes many important hormones for the regulation of activity of other glands
