First Question : Provisional diagnosis: pharyngeal pituitary gland Cause: maybe the patient had a congenital anomaly causing failure of ascend of rahtks pouch leading to pharyngeal pituitary gland so during the operation, may be the doctor had removed the anterior lobe of pituitary gland by mistake instead of removing the adenoid which lie also the in the roof of the nasopharynx.
