I wish i can give you a printable template but i dont know how to make a printable. For fitting, it is during taking measurement and scoring might be the problem. Just make sure you measure exactly and when you score make sure you are scoring as per the line.

It's an ordinary plastic cover that can be bougth in educational store. Itis usually used for covering books or notebooks, etc..
