At the very last section we're asking if you ever applied for or been issued a passport. I haven't since I was 7 years old should I include that Info ?
If one of the parents of the applicant is no longer alive, do we still need to include their name in the Parental information section?

Question - and I'm sure somebody has already asked it in here - after I list my spouse's name and information, it asked if I was ever divorced / widowed. Is that basically asking me if I am still currently married to her? Or do I just simply put in the most recent divorce date of my previous marriage?

Also, those are not backslashes...they are just slashes. Backslashes lean the other way.

You can attach an additional sheet of paper that indicates you’re unaware of the specifics
You can apply for their death or birth certificates and there you will find the information.
If a parent passed away at any point should we still leave one of the sections blank even if we know their information?

Do we have to write everything in capital letters? Or just the beginning where our name is, or, can we write that part without capital letters?
Also, by the permanent address, it says complete if residence is different from mailing address, my mailing address is the same as my permanent address, so can I leave this blank? Or would it not hurt to fill it out anyway?

Hey Kurt, I prefer capital letters for clarity and to avoid mistakes. Legibility is the most important factor, however.
As long as your mailing address is also your physical address and not a PO Box or something like that, then it's okay to leave that section blank.

As long as your Primary Contact Number (item #7) is listed, an additional number isn't necessary

I have a question, I'm applying for my baby, who is 1. In item 21, "Have you ever been issued..." would it be my passport info or would it be "no" because my child has never had a passport before?

The occupation is about the child not parent. The form is a little confusing.
I was divorced around 50 years ago, it was one thing I never thought I would need this info! How do I find out?
You’d have to check your divorce decree, or maybe call the courthouse you were divorced. At worst, you can attach a sheet of paper to your application and write there that you’re unsure of the date, etc….
Hello. How would I fill out my full name if I am a Jr.? Would I add the Jr next to my last name? Or next to my middle name?
For Field #10, if I don’t have an additional phone number do I use the same number used in Field #7 or do I leave it blank?
Section 12 applies to the applicant and not the parent?

If you've never gotten a passport, you should use DS-11 and provide the documentation showing you are am American citizen. I also have a video about how to prove citizenship, if you need help with that you should watch that video 🙏

If the application is for a child the marital status is for the applicant or the parent filling it out ? I feel like a idiot asking.
When asking about the applicant, the marital status is referring to the child, not the parent. 'single' would be an appropriate response for most

I was married 23 with an older woman who took advantage of me and tricked me! Long story… I don’t no any information on her other than her name😢. Any suggestions what I can do to get my passport and married again. Appreciate any input

In 'Parental Information' do we just list our only living parent if one is deceased?
For field #10, if naturalized parent's birth names are non-Roman alphabet in their foreign birth certs, do you use foreign characters or do you anglicize them?
@@dogwink personally, I would anglicize them and then attach an additional sheet of paper to the application that includes the names in their native language and mention that you’ve tried your best to anglicize them in the application

Hello, I’m filling out this form for my abroad child to complete the CRBA form and she’s just a year and half years old. So should I leave the Social security section, the marriage section, and permanent address section Blank?
Fill in all the info you have. If it's irrelevant, like marriage date, leave it blank.
If I may, it says in the instruction section of DS11 for filing for the child's first passport, you need to put zeros in the child's SSN section..do not leave it blank.
I am filing for my 6 months old child the CRBA and first passport..but, I am yet to find a clear and certain answer on what to put in the height and weight of the baby!!!!.
I believe I will just leave it blank as he is just an infant, or maybe see if the official ask me to fill it out differently.
Do you know what will happen if I left one question blank? (Question #14) In my case, when I filled out the form online, question 14 (place of employment) was not required. However, when I arrived at the facility center, I had to clarify one of the questions and ended up manually filling out the form. I simply copied the information (except the one I needed to correct) from the printed form, and question 14 was still not required in this case. Now, I'm concerned if this will be an issue or not.

Please make a video that if a child of a US citizen is born in another country, then they should stay in the other country and apply for a passport for the child at the US embassy.

Where it says a parents place of birth, is it written out like: Milwaukee, WI or Milwaukee, Wisconsin?
Filling out for my husband who is currently married to me but previously divorced once. In # 11, (I put my information down as his most recent spouse), but when it ask the question right after about ever having been divorced, its asking in general correct, if he has ever been divorced (not specifically asking if divorced from me correct)? So I would still put his divorce date from previous marriage on there even if the other spouse information is mine?

One more question ..I have been seeing conflicting info on what type of pen ink is required or recommended for completing and signing the form ..from no ballpoint pen to rollerball, gel ink, felt tip marker or some type of signature ink. I noticed you were using I think was a gel pen in your video. Gel pens are what I use all the time, but I just want your thoughts and recommendations on what is the best type of pen ink to use for such an important document. Thanks.
Hey my pleasure to help! They require black ink and for it to be in big block letters, I don’t know why the type of pen would matter much as long as it writes well and legibly

No you shouldn’t falsify anything, but you can attach another sheet of paper and explain any unique situations there
