CataractCoach 1335: behind the scenes: inside my OR - cataract surgery

  • Surgeons from around the world frequently email me asking to come visit my operating room to watch surgery. In the OR, an observer would only be able to see a limited amount -- just the view of the room and the video monitor showing the microscope view. You can see the room view in this video and the microscope view is what we normally share here on CataractCoach.
    Our standards for cataract surgery are very high -- this is the highest level of care and it is exactly how I would want surgery done on my own eyes. The staff members are excellent and they work at a relaxed but efficient pace. The total time to clean the room, set up everything, do the time-out, prep & drape the patient, complete the surgery, and then prepare for the next case is about 15 minutes. This is about the same pace as previous posts about cycle time that we featured here in the past. In the morning shown here, we completed 10 cataract cases by 9:30am with a start time of 6:30am, and that included two complex cases which required more time. Again, remember that it is not about speed -- but rather being in the state of flow where efficiency is natural. Tomorrow I will post the microscope view of the the actual surgery shown here.

    I thought I never heard this, I rather do 6 beautiful and perfect surgeries than 8 or 10 in a day. Normally surgeons wants to go go. They haven’t even finished one already thinking on the next one. Thanks

    Hey... Dr dr Devgan, i find your videos very informative and helpful. I wanted to ask a question about your scrub tech. As i see her in this video swinging her arms, i think she wants to dry them after washing up with surgical pyodine scrub solution. Water drops near elbow might travel back towards hands as she swing her arms and compromise the purpose to get arms and hands germ free. We don't allow this movement here and we are taught not to do that. What u say about it

      she is using an alcohol based cleaner on her hands which evaporates very quickly (like Sterillium). The first scrub of the day is with water and povido-iodine scrub, then Sterillium between every case. In this video, notice how she does not need to use a towel to dry her hands -- the alcohol solution has already evaporated. There are no water droplets (or any other fluid droplets) to become airborne.

    Do you work out of just one room or bounce between two?

      either is fine with me. on this day, just the one room. With a cycle time of 15 to 20 min per case, that's an easy 3 cases per hour in the one room. On this date, this was the 10th case completed by 9:30am (start time of 6:30am) and we even had two more complicated cases which took a bit longer (10 min cut time instead of 5 min cut time)

    Thanks dr Devgan for sharing so many gems with us. @ questions for you; I am not sure If I am mistaken but I saw 8 staff members inside the operating room. I assume that the surgery cost increases a lot AND maybe the risk of infection as well...., no?

      No issues with infection. Zero endophthalmitis in 25 years

    You are very efficient using one room. Does your cataract surgery suite have only one room?

      we have 7 operating rooms at our surgery center. This video is to show how you can be efficient in one room. Alternating between two or more operating rooms can be even more time efficient for the surgeon.
