  • With CataractCoach, I am sharing everything that I have learned or figured out during the past 25 years in ophthalmology. And this is particularly true in the operating room where I have a wealth of experience and surgical pearls that will make you a better surgeon. This video covers a few critical things that we do in our surgery center in order to provide the best outcomes and the safest patient experience. The most common IOL related error in the operating room is putting the IOL for patient A into patient B. How has this never happened to me in tens of thousands of surgeries over 20+ years? I explain my method here. Also, when the patient comes for the second eye cataract surgery, how can we quickly and easily check the refractive outcome from the first eye? I explain that too. And finally, how do I set up my phaco machine settings for efficiency, but yet still have the ability to use special modes if needed? Again, the magic will be explained in the video.

    I noticed that you use several options for monofocal IOLs (Alcon, J&J, B&L). How do you determine which monofocal choices are more adequate for each patient? I was curious because I have been exposed to a single company as a current resident. Thank you so much.

      The more tools in your toolbox, the better. Goal is to tailor the IOL to the patients and their needs. I recommend getting familiar with all IOL options and then go from there.

    In 4.12 you just mentioned the patient underwent 1st eye a week ago,
    So does it make any sense in checking after a week with those lens if the patient mite go into a myopic or hyperopic shift.?

      You can certainly tell if the patient is healing a bit myopic or so after a week.

    Sorry, I couldn't help but notice that your watch crown is on the proximal end, is it probably upside down?😜

      It’s the Apple Watch. You can have any orientation. I like the crown away from the wrist because it is out of the way when I wear gloves and wrist wraps for bench press day! 225 is ready for you (225 lbs = 100 kg). Come join me for a workout

    Hello Dr. Devgan, I've got couple questions if you dont mind. So if there's an astigmatism at ax 180, that's when you do main incision temporarily and position yourself at the side of the patient and not behind patients head? Also, you mentioned that the patient had undergone phaco on the other eye 1 week earlier. Don't you wait 1 month or is it totally okay to do after 1 week? As a bonus,maybe you could record a video about how do you get the best results for IOL calculation, because here in my country we get a lot of misscalculations when it comes to it, mostly + refraction (which is super sad)

      You’re quite petite. I’m quite large. With a wider PD there can be better stereo acuity and depth perception
