    I have two black tattoos which I am removing via Q-switch. I've had around 8 sessions so far and it looks like I might need 5-6 more. I was thinking of switching to PicoSure for faster removal even though it's much more expensive (triple the price) but after watching this I've concluded it won't be worth it as it won't be removing my tattoos much faster. Am I correct to assume that? And thank you for all the help, this was very informative.

    Thank you, your video makes a lot of sense
    I was initially worried on getting subpar treatment at my clinic that my aesthetician set me with appointments with a nanosecond machine (Lutronic Spectra XT)
    Though the difference between pico and nanosecond machines are mainly the pulse duration which is shorter in picosecond machines with higher peak power which gives a theoretically better safety margin of heat transferring to tissue and lower required fluence levels.
    But the amount of energy that the skin can take before unwanted side effects occur and the amount of pigment clearance that the immune system within intervals are still a factor outside of shattering the pigment.
    Best to get help from an experienced clinic and not to judge if they have the latest laser.

    Had multiple of both, with self lasers from China too. And tattoo removal cream and gel. Still a working project.

    I've been using cutera enlighten and I'm at 9 sessions and still need about 5-6 more based on how my tattoo looks

    Hey doc, hope you're doing well.
    I've recently been researching on possible treatments for the different pigmentation concerns I've got and that has lead me to this video.
    You seem to be so very knowledgeable and all your points make complete sense to me.
    My question to you is that, if I wanted to get my dark lips lightened, would you recommend the Q switch or the Pico laser? Do either have an advantage when it comes to targeting melanin, particularly the lips?
    Also what type of laser and what wavelength do you think would be safe and effective at treating this?
    Would love your insight on this. Thank you.

    My daughter was burned really bad by Q switched lasers and had blisters so bad and after 2 sessions she didn't proceed anymore. I am scheduled for 3 sessions with a Pico way laser next Tuesday. From what I was told it is not as intense and is less likely to scar or blister. I will update this comment once I have my first 3 sessions. I was told it would still take 10 plus sessions, and that I would need to wait 2 months in between the sessions.

      It’s a selling point is that the sales reps use and trying to sell pico second lasers that it is less likely to burn and scar you in reality because they firing so fast you have to be even more careful with them because you were giving a lot of energy in a very short period of time. Anytime I see burning and scarring it’s usually due to the laser operator either using the wrong laser entirely for a tattoo such as a hair removal laser that fires in milliseconds for the correct Laser, which would be a Q swish or a pico second Laser except at to high of a
      Fluenc or energy for the tissue to tolerate, you can indeed get blistering from the Picosecond Lasers as well.

    I was wondering if you would recommend me the Pico laser or the Q-laser for my tattoo removal. I have a black fine line tattoo, with no shading if that matters, that goes from the middle of my upper hand to the arm fold. It was done 2,5 years ago.

    hello.. what is the best laser for skin of color? fitzpatrick IV. i have problems with skin darkening /hyperpigmentation.. i have used different creams and peels but nothing seems to work but just got darker.. i noticed that my skin tends to darken after insect bites and healed pimples and takes longer time to lighten compared to others. 😔. whick laser is best for me? q switch or pico? and what wavelength? 755, 1064 or 1320? your reply is much appreciated 😊

    Hi there, just a couple of questions the first is, is there a difference between picosecond and picoway ? the second question is I have seen a laser that covers 1 to 2 cm with a round nozzle leaving multiple dots within the circumference and then what looks like a Q switch is used after this process and looks really good before and after. I assume the first laser breaks the ink into small pieces and the Q switch does the rest. Thank you for your video and time.

      Picoway a brand laser that fires in picoseconds ( a trillionth of a second versus a Q which is a billionth of a second. Many laser companies make pico today so you see enlighen, picosure, picoway. I think the dot you are describing is a mark made by the laser beams thenselves. Sometimes when a smaller beam is used you see a dot several mm in diameter

      I’m very partial to the traditional Q swished YAG laser. It will do a good job on an old black tattoo provided. It’s a quality laser that’s properly maintained.

    Which will be more effective to remove Cafe Au Lait birthmark faster? Pico or Q switch?
    Is Revlite/Revlite SI a Q switch?

      Yes it is. I imagine both would be effective in treatythe cafe au lait however skin test small areas first at different settings to make sure you don’t darken the spot. I think they are equally effective

      No because the rate limiting step is our immune system. The pico makes a finer dust of ink we thought it would make it easier for the immune system but since our immune system is already running at capacity it didn’t really change things.

    Hello Doctor, I watched the video and I’m still a bit confused. What are you thoughts on the Picoplus from Lutronic? I have the choice between two clinics, one with the Picoplus by Lutronic and the other one with Revlite by cynosure. For additional information, I’d say I’m Fitzpatrick 5 (however very light on my chest/neck where the tattoo is) it’s about 7cm long, 2mm large. Black tattoo. Thank you!

      Probably either. You need the YAG laser which both have.

    Hi doctor, I've been removing a cover up tattoo totally black with a lot of ink... I've had 10 sessions, the first 3 with qs and the following with Lutronic picoplus. The tattoo is fading but there are still area where the ink seems very stubborn.Do you think coming back to qs nanosecond could help? Thanks in advance

    A couple of corrections to be made, lasers were first discovered in the early sixties and not built for tattoo removal. Secondly Nd:YAG lasers with the right output head or SHG crystal (second harmonic generation) can produce 532nm, 755nm and 1320nm from a 1064nm Nd:YAG laser, KTP (potassium Titanyl Phosphate) produces 532nm (green) for example so you can remove reds, greens, blues etc but you're right about yellow being a problematic one and white is almost impossible to remove without skin damage.
    I do hate it when there's information spouted on YT which is plain incorrect due to the person not actually understanding what they're talking about. You're almost there though on your laser info, it just needs a little polishing.
    Thanks for the video though as all information can be used to determine which is best for your scenario...

      The first tattoo to be lasered was in 1962 using a Q Ruby laser. Lasers have been around for decades however tattoo removal is really only popular in the last 20 years. Using heads to create wavelenghts can be done however making them fire in nanoseconds or picoseconds is another thing and sometimes disastrous. I once had a Q Alex 755 the rod would shatter quite frequently which is why these lasers are not that popular

      I’m partial to the Q YAG but a Picosecond YAG should work just as well. Hyperpigmentation may be difficult and require 5 or more sessions if it’s resistant

    Do you think Astanza Trinity Laser is better than PicoSure or Enlighten III? I have a large tattoo with colors that I am trying to remove and don’t know which laser would work best.

      you need multiple wavelengths i think the trinity or enlighten as long as they have multiplw wavelengthsin the system. Things are harder now because these lasers are so expensive additional wavelengths are optional i think as long as it has a yag , ktp and ruby you will be okay. Just remember white and yellows are resistant

    Dr Messina, I am a physician as well. I enjoy your presentations so much. I currently have a Q-Switched laser and would like to to upgrade my knowledge of the physics and possibly more advanced protocols for using the laser in ways to benefit my patients. I have had a devil of a time finding protocols. I have found training by vendors to be terribly inadequate.

      I know what you mean, I started a long time ago and little was known about laser removal. There is a book by William Kirby D.O. that gets into physics etc. There are courses by the ASLMS ( American Society for laser Medicine & Surgery ) they have an intensive course in their yearly conf that is geared to laser physics and tissue

    Hello I have a tattoo in my neck is a bird with black , red , green and blue. I do every month laser discovery pico(quanta system) and first he do the laser removal and after one minute did me fractional . The red I almost gone from my skin but the green and blue look difficult colour but hope will disappear. I want ask you think is correct to use in every session laser removal and after the session fractional? They told me that help tattoo go away more fast. And with this laser will be the couloyr removal 100%? Thank you

      I am not a big fan of using any fractional system repeatedly over a tattoo because it could change the skin color too much and it’s really not the best method to remove a tattoo. I would see if that laser system has a ruby wavelength in it it might, and I would use that on all the blacks greens and blues, and a 532 on the red.

    My Dr is using a Yag laser but I have a lot of green and light blue on my tattoo. The first session was fantastic and made my tattoo lighter, but after that, I got 3 more sessions and nothing changed. Can you please tell me where is your clinic? Thank you.

      I’m on Long Island ny. The yag is not good for green and slightly better for blue. The Ruby laser or even an alexandrite laser that’s q switched would be better.

    Hi Dr, thank you so much for this video. I use a Q-Switched laser and it has been very effective for my clients but I am finding more so these days people asking about the Picosecond/Picosure. This video is explained very well and it's got lots of great information that I can pass to any future clients. I think there is a lot of marketing hype about the Pico.

      @@quincy189 Hi there, yes so my machine is a Quanta Q Plus C and it's a Q-Switched laser. I have never used a Picoway so I couldn't really give an opnion on this. However my clients have great success with my machine. And I am very happy with it. In my eyes, it's the best! 🙂

      Both can blister and scar. Red needs a 532 wavelength. I like the Q Just make sure the laser is on conservative settings

    My skin is sensitive but I got an eyebrow tattoo removal with Pico laser on my gray brows… I bled a lot but it’s like the blood is under the first layer of the skin it’s not its washable 😅 It’s not like any videos here where people’s brows get light/white right after, mine got super red and it’s like red dots… why could that be? Why aren’t there any pictures or videos with patients like me? 😖

      THe red dots are dermal pinpoint hemorrhage and quite common, the brow hair will get white but recover in a short time.

      Im a Q switched fan, the Starwalker is made by Fotana an old and high quality company so I am partial to that laser as compared to the picos which are very very expensive, meaning you pay more and dont get an appreciable difference in tattoo removal.

      Either will be fine to treat hyperpigmentation, the pico may have a slight advantage but the Q is just fine.

      Picoway is a nice system, this and Q are both photoacoustic so thermal injury is relatively rare with both but can still happen. Its great you are happy with the system you chose to use, that's the best part.

    I think the beauty olace i went to used the ĺaser for hair removal, on my tattoo. Could this be why it got so red and looks like a freshly inked tattoo? It was already bery faded but now it looks like a redish brown outline of the tattoo. Is this a burn

      I can’t see the pic. Laser hair lasers will burn. The pain would be terrible and the risk of scar very high

    Im Using a Picosecond laser as we speaks. i think every machine takes the same amount of session to remove a tattoo am i right???

      In general, if the laser has a wavelength, that’s being absorbed by the tattoo ink, they should all take about the same amount of time. However, if you have, let’s say a blue ink that Laser should have either an alexandrite wave length 755 or a ruby wave length of 695 if you don’t have either of those wave lengths, then it might take significantly longer to get that blue ink out

    Doctor I am prone to keloids but only on my shoulders I have a tattoo on my face neck and arm can I get it removed?

      Yes. But of course the possibility of keloid formation is there but low. Just use lower laser energy and wait 12 weeks between sessions

    Dr, which laser is best for SMP removal on dark brown African skin? I’m very confused which to follow Pico is unsuitable for my skin I have been told? Also is Nd YAG same as qswitched laser?

      Pico may be to prone to pigment loss. A Q YAG 1064 would suit you nicely

      The nD yag is the wavelength. The Q is the rate of firing so make sure that yag is a q switch

    Hi doctor,I had started tatto removal sessions in may 2023 my techinician said that we will continue to the summer then break,but I will need to come in September one more time and I will be done (q switch),i had done 7 sessions in september will be 12 total,is it possible to remove tatto in such short time (year and 4 months long) ,my tatto is black ...?

      A year and a quarter seems about right when using a cue switch laser in fair skin light skin tone every six weeks is a short as you want to do. I always tell my patience to expect to be together for a year and a quarter to a year and a half so it sounds like you’re right in the proper time frame to have a black tattoo removed.

    Hi Doctor, I want to remove a tattoo that’s red and black. The clinic I’m looking at uses the Spectra Lutronic Sysytem. I’m very scared that I’ll develop scarring. What should I be looking for in a provider/clinic that removes tattoos?

      That is my favorite laser. Look for a qualified practitioner, one with a lot of experience and a good local following. Don’t fall for the new “spa” that opened a week ago and now voted #1.

    Hi Doc, I just need a clarity on Carbon peel. To give much information, had 3 sessions of carbon peel within a duration of weeks time but I don't see any improvements as such rather my skin looks more patchy and dark. Could you help me with some suggestions/recommendations to get rid of pigmentation around mouth and on forehead pls..

      Try the peel without the carbon. Lutronic who pioneered the carbon peel call this the soft peel. It delivers less heat to your skin. If you have melisma heat is not your friend so just use the q 1064 and quasi long pulse should they be using the official Lutronic spectra. Of note melisma and hyperpigmentation take many sessions I tell people at least 7.

    I’ve done three sessions to get my black fine line work removed from my arm with a Q-switch, it’s not a very big area & it has faded quite a bit but i want the area to be a bit more cleared up before i get it covered up… wondering if i should switch to a picoway for the next treatment which you say you recommend but i just wanted to see why you think so or if i should just stick with the q switch? The shop with the Q-switch is really close to my house but the picoway is about a 45 minute drive from home but if it’s gonna remove my tattoo quicker then i can justify the drive out.

      @@Luisrivera12672 Sure, when using a Q switched laser the MINIMUM time between sessions is 6 weeks for caucasion and 12 for African American or darker skinned persons, when using Pico second lasers the earliest is 12 weeks to 6 months in darker toned people,

    What’s your knowledge on the picoway system? Removery is buying out all the tattoo shops and bought mine out. They used to operate both picosure and Qswitch , and now they have added the picoway. I hope I’m not wasting my time and money

    All hype. As a laser tech I’ll tell you qswitch is hands down better. We have a picoway and it’s just a pricey unit that is constantly being serviced for big money, it spends more time with our repair tech than it does removing tattoos I swear.

      Yup! It’s horrible the pico didn’t seem to me that it would speed things up. The rate limiting step is your immune system which is already working to capacity. I don’t have a pico but I do have a vascular laser that is in service more than in use

      Absolutely spot on, in fact photo acoustic or opto acoustice means that the light energy creates a sound wave due to photothermal effects which is essentially the same thing but uses more energy. This means a q switch laser can dump more energy per pulse. Using the machine with the correct power and repetition rate is key to removing a tattoo cleanly without burning. Arguably the pico laser can create worse scaring due to the numerous times your skin is being blasted, costs much more and isn't as effective. The colouration of the pigment you're trying to remove determines which wavelength you'll need to remove it so using the correct tip which changes the output wavelength ie 53nm, 755nm, 1320nm.
      I plan to get a q switch for tattoo and mole removal / reduction, tested on myself before I zap the wife; just in case...

      I am partial to the Q , however I believe the enlighten has both so they can start with a Q and finish the tattoo with a pico when the ink is faded.

      Yes fitz 4 will be okay using a Q yag.
      It’s most important though you go to someone competent in the treatment. If desired you can switch to a pico yag later on.

O was worried because the doctor who visited me told me it's better to start with q Switched and that go on with pico and I was worried that she was fooling me to "steal" more sessions from my wallet but now it's clearer to me that she was actually doing the best thing :)

      Thank you thats great. The Q is like a sledgehammer shattering the ink which is important in the early sessions when the ink load is highest. Then if you want you can switch, I do not and use Q for all the sessions. Best of luck

    The place that I have to pay $4,041 USD for a package to fully remove my tattoos are using Picoway laser.

      Yikes! The free sounds tempting but why and what is the quality of their laser. I am normally a Q Switch person but picoway is a good system. See if they will start with the picoway Q arm then switch over. Keep in mind that laser is $250,000 adding to the price.

    Doctor, can the Pico laser work on Hemosiderin? What is the difference between picosecond and pico genesis?

      Picogenesis just has another laser in it thats designed for rejuvenation. Hemosoderian is simply oxidized hemoglobin still in the skin after a brusie. THe picosecond laser especially if a YAG ( 1064 nm) should remove the stain. I use t Q Switched YAG for treating hemosiderin but the pico should do just as well.

    I’m trying to decide on what laser to go with. I found a provider that uses the Q switch and is quoting me 80/ session and saying he has over 15 years experience. On his website he says he uses The Dermo Clarifyer laser but I can’t seem to find information on this. While speaking with him he identified it as being a Q-switch laser that is a step up from the nd-yag laser. I’m also looking at clinics that offer the picosure and pico way laser. This gentleman’s clinic is way cheaper and he has a lot of experience but I’m just not sure. What are your thoughts?

      I always get a laugh when I hear over 15 years experience and they have laser thats not a standard system. First 15 years ago there was only a hand full of practitioners and these days suddenly there were a preponderance of them. That aside that clarify laser , which I could not find either I assure you isnt up to the task may be an import. $80 is very inexpensive and the reason the picos are so much more is the laser costed them close to $300,000 so $80 treatments are not going to cut it. The picos are better than the first guy , Im not a pico fan as you see in my videos because I have been around 15 years and know how laser removal works. Picos were developed and marketed soley on cutting the laser sessions in half. most tattoos require 10-14 sessions , they sold pico as one that could do it in 4-5 which it does not. Since the job was no faster or better I could not justify the cost of using one and also having to charge patients more for nothing with extra benefit . Of the 3 lasers I believe the picoway is the best because it has multiple wavelengths and picosure was horrible. It was terrible on black,

    Is it possible to go every three weeks for laser tattoo removal? My doctor wants me to come every 3 weeks. I got a second opinion and the other doctor told me every 6 weeks. What do you recommend. I don’t want to damage me skin!

      Three weeks is to early and you will be spending more than you need because the immune cells are still stimulated. If it’s Q switched then ever 6 weeks is the earliest and if pico 12 weeks.

    I been getting tattoo removal for a while now and i have lost pigment while removing light blue..why is this? and is this normal? I have been removing black ink but that removal is fine no loss of color.

      The laser for blue may be different than the black ink, black responds to a YAG which is gentler on the skin and will not be as effective on the blue. They may be using a ruby on the blue which can cause a loss of pigment.

      I love the spectra by Lutronic. I have one and it’s an amazing laser for tattoos black red and navy blue. I use Ruby lasers for other blues and green. It also offers the carbon spectra peel. Equally important it’s solid. It dosent have a lot of breakdowns

      @@super.PY14 the difference is the rate at which the laser beam is emitted in a cue, switch laser it fires in a billionth of the second a pico second laser fires in the trillions of a second theory being that the rapid firing of pico creates more of a fine dust, when it comes to breaking down a pigment I know with tattoos in the early phase is the pico does not seem as efficient as far as the nevus of ota goes though they probably are equal I’m not sure because in the United States, where I practice nevus of ota is an extremely rare occurrence so I don’t get to speak to other physicians in the two that I treated I use the Q switched yag

      Where on able to change the pulse width on these type of lasers the cue switch is firing in nanoseconds or a billionth of a second. The pico is firing in pico seconds or 1 trillions of a second but unlike other laces, you can’t really adjust that you could adjust the laser beam size you could adjust the laser beam fluence, and you can adjust the rate at which the laser fires but you cannot adjust the width

      I had a session today. It was my first time and it costed me 50 euro. My second session will be in 8 weeks. The laser that been used is the Pico Derma 4 laser. I also have colors in my tattoo and she said colors is harder to get rid off but we will see how it goes.

      That’s the sales rep speech, they sell it on the photo-acoustic properties and say the Q is photothermal and cause scars and burns. What they don’t know is the Q is the original photo-acoustic laser firing in nanoseconds. The bottom line is both will burn and scar a person if used incorrectly

      Steroids reduce the immune response so in theory they can reduce the effectiveness of your macrophages taking ink particles away. Its not likely a significant impact and on occasion should someone develop severe swelling a short course of steroid will help.

      The Q YAG laser would be an excellent choice to remove a dark thick black tattoo on a dark skinned individual. The risk of getting scars is dependent on your operator, so make sure you go to somebody who knows what they’re doing who’s been doing it a long time and has a good reputation in your community whenever I see scars it’s usually from somebody who is either lasered with the wrong Laser for using the right Lisa, except at two high of an energy level

      Scars are very hard to treat it could very well be that if you had a pico with a 532 wavelength you might be able to soft in the scars in time. There are other lasers that do help soften scars one are fractionated CO2 lasers and the other is a laser called the Cutera EXCELV is designed to reduce redness of rosacea and to take redness out of scars and again overtime it could soften them. They usually take a lot of treatments and they usually don’t just disappear they simply get better
