  • How To Use A Fetal Doppler After 16 Weeks Pregnancy - Dr Gill
    Using a fetal, Doppler seems like one of those very specialist skills. However, I would argue that being able to accurately, and reliably detect a fetal heart rate, and separate that from the maternal heart rate is a clinical skill that all practising doctors should be able to perform.
    In almost all specialities, you will likely come across pregnant women in your care, and regardless of the medical issue for which you're seeing the mother. There, the majority of pregnant women will be more concerned about the unborn child. In those situations, being able to use a fetal, ultrasound probe is a fantastic tool to help a mother, even if it is merely a temporary measure, whilst you are arranging a specialist obstetric review for definitive reassurance
    In this video, we address how you can use the fetal Doppler, in order to listen to a babies, heart sounds.
    We highlight the reasons that you may want to listen to a fetal doppler, and highlight approaches to differentiating the maternal from fetal sounds
    Here is the link to the BMJ article outlining the issues with patients utilising fetal dopplers at home
    www.bmj.com/bmj/section-pdf/1...
    ________
    Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
    However during OSCE assessments. Different medical schools, nursing colleges, and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.
    The examination demonstrated here is derived from Macleod's Clinical Examination - a recognized standard textbook for clinical skills.

    I remember my doctor using the doppler on me quite a few times throughout my pregnancy and I found it amazing how clear you can hear baby's heartbeat on such a small device. I had HG throughout the entire 9 months, I thought it was normal morning sickness! Spent quite a bit of those 9 months in and out of hospital :( Would never wish HG on my worst enemy! As a very very experienced RN who has dealt with many doctors in the ob/gyn area ( personal experience as well, having been through 6 pregnancies), they are fairly antiqued. When I was trying to determine 47 yrs ago if i was pregnant with my son they wouldn't allow home pregnancy tests bc they felt it wouldn't be properly done ( trust me the tests were available but the knowledge of having the them available was not made available!) This doppler business that this doctor is endorsing is the same.

    Dr. Beth, since you’re an OBGYN, how did you go about choosing your OB? I assume you can’t/shouldn’t be your own prenatal care provider.

    • @DrJamesGill
      @DrJamesGill  10 หลายเดือนก่อน +1

      Not to answer for Beth, she will be able to add her personal take, but generally, in the UK, unless you are opting for private medicine, you don’t get to choose your specialist, you are allocated depending on the case
      So a simple shoulder case would go in the general shoulder surgeon pool, but a case of a certain difficulty, would be discussed and a specific surgeon would likely be referred them

    late to this but i do have a question. i know these things can give the wrong impression when people confuse the mother's heartbeat for the babies but wouldn't a normal adult pulse be too low for a baby and a babies pulse be too high for an adult?

    • @DrJamesGill
      @DrJamesGill  7 หลายเดือนก่อน +1

      Yes, however a very anxious mother could have a pulse of 150, which would be what you’d expect of a baby
