Well, this song and video have different meaning but it can be interpreted as smoking. Taylor had an eating disorder back in 2014. After meeting her ex boyfriend Joe Alwyn (they were together for 6 years and broke up in april this year) in 2016, she understood that it's not good for her so she started eating much more and in 2017 she gained normal weight for her. Now she's healthy and happy but still skinnier that she was back then. Anyways, she's good now and that is all what matters.
