    The hair loss community, I feel, is becoming politicised on the topic of medication. I am concerned with what I see. Medication for hair loss is a personal choice. However, it is vital to reinforce a hair transplant with it for long-term value.
    Whether we take Finasteride or Dutasteride or oral Minoxidil it is a personal choice made in conjunction with one's physician. More than ever I see division here, where people on certain medicines are propagated to superiority. Platforms such as Reddit and forums promote this conjecture. This is wrong, and verges on a cult-like ideology.
    I recently highlighted Dutasteride as a step too far and stand by this, although this was more personal experience and personal opinion. I would like to remind people that more is not less in hair restoration medication. And the first line of defence is Finasteride.
    Dutasteride is for benign prostate hyperplasia, and more importantly when Finasteride at clinical dosages much higher than for hairloss isn't enough for BPH. This medicine blocks the 3 iso-enzymes, inhibiting pathways dependent on them in steroid metabolism. The following reactions are known to be catalyzed by 5α-reductase:
    Cholestenone → 5α-Cholestanone
    Progesterone → 5α-Dihydroprogesterone
    3α-Dihydroprogesterone → Allopregnanolone
    3β-Dihydroprogesterone → Isopregnanolone
    Deoxycorticosterone → 5α-Dihydrodeoxycorticosterone
    Corticosterone → 5α-Dihydrocorticosterone
    Aldosterone → 5α-Dihydroaldosterone
    Androstenedione → 5α-Androstanedione
    Testosterone → 5α-Dihydrotestosterone
    Nandrolone → 5α-Dihydronandrolone.
    Every medicine has a downstream knock-on effect. Dutasteride for hair restoration crosses a line.

    Been on it for 4-5 years with no real side effects that I know of
    At 38 years old, I have retained most of my hair and arguably, it's a little better now than it was 5 years ago
    While in comparison, i have cousins and friends who have had hair transplants and one thinking of doing it, and none take or want to take finasteride afraid of the side effects. My hair is better than theirs by a long shot. One in particular is 30 years old and is in a bad shape, resorting to various hair styles to try and cover it up. All thanks to the negative internet stories. Not even realising that without fin a hair transplant is more or less worthless you hust buy a couple of years and the recession makes it look even worse (with grated alive hair at the front then a cm or two behind blad...

    I already got some hair growth with rosemary oil, vitamin E, castor oil, and black seed oil. I use a clean shampoo without chemicals and add these oils to the shampoo. This has helped; I see many new small hairs growing I do wash my hair 2x a day and let the shampoo for 5 minutes before washing out. I am sure Finasteride would help much more, so let's see where it goes. I have been using Minoxidil for two days now and have already contacted a compounding pharmacy to add Finasteride to my Minoxidil. I will use it in a few months, as I need to A/B test with and without Finasteride.

    I’ve been on 1mg/day finasteride (only) from the age of 21. Finasteride slowed the AGA, the loss of hair was managed, but still obviously I was growing bald, especially in the left and right frontals.
    At 38 years old I got a hair transplant at an accredited clinic.
    Results after a year have been stellar. But 13, 14 months post-op the shedding had grown significantly. This isn’t seasonal as it has been ongoing for the past 5 months. Everyday washing my hair I can count at least 12 hairs. The hairs also seem narrow. Obviously it has nothing to do with the transplant. But I am gradually yet noticeably losing the results of the transplant. It is as if finasteride has lost its positive effect. I will soon begin regimenting oral minoxidil 1.25/2.5mg, but I doubt that would take care of the increased shedding.
    What do you think may have changed recently in the biology of testosterone
    /DHT? What therapy do you recommend I change to remedy this increased hair loss?

    • @MedicalSchoolJourney
      @MedicalSchoolJourney  5 หลายเดือนก่อน +1

      There can be synchronized shed at some point in the 1 - 2 year mark of a portion of the transplant. During this period it looks worse overall. It's like an anagan reset at once, if you imagine the hairs from the donor when harvested went into a resting period in synchronization. I have no idea why the shed occurs at this timeline from harvest though.
      There is also possibly another prompt such as have you changed finasteride manufactures recently? Have you encountered increased stress and this raised cortisol etc. is the hair coming out mainly long and terminal - shed, or it's a noticeable portion consisting of shorter and finer hairs? - progressive AGA. Has there also been a change in diet? Have you reduced caloric consumption months back etc.
      Finally, have sebum levels increased by any chance - is the scalp greasier than usually.

    • @MrPopocatepetl
      @MrPopocatepetl 5 หลายเดือนก่อน

      @@MedicalSchoolJourney my operation was on 04/23.
      Regarding the synchronized shed (which I haven't heard of), I can't tell for sure if these are the transplanted hairs or the native hair which are falling off.
      How common is post-op synchronized shed and what should I expect? How long does it take it to stop and will the new hair re-grow, replacing the shed hair?
      There was a period (months 6-11 post-op) when I've used generic forms of Finasteride.
      I have read that sub-optimal response may occur due to the generic form being less potent, and have therefor reverted back to Propecia, on month 11~12ish.
      I've been solely on Propecia for the past 5+ months, nearing 17 months post-op.
      Stress - maybe during February-April. But it has been over 5 months since than with no special events.
      Shed hairs are falling off mostly long. I'd say about 80% of it long.
      Perhaps I cannot account for the short hair because I mostly count it in the foam on my hands as I wash my hair. Shorter are harder to track?
      Change in diet - I've tried to consume slightly less calories. It was inconsistent, on and off, for about 1-2 months. It wasn't a major decrease (-200kcal) and I wasn't consistent.
      Sebum - yes, there was a period when I've felt like I've had more sebum, not sure why. My scalp was itchy and I could see dandruff over my shirt. Not sure for how long it lasted, but I took care of it on August, by incorporating Nizoral 2-3 times/week. My transplanting doctor also noted that I have greasy hair. I've recently read that SLS shampoos my cause increased oiliness, due to striping away all of the natural hair oil, so the hair secretes sebum even more to compensate. Tried switching to a non-SLS shampoo for a while, didn't notice a positive or negative effect over the shed. Felt oilier and went back to SLS-shampoo (Head and Shoulders).
      Thank you so much for replying, much appreciated.

    27M, Im a diffused thinner, mostly around the midscalp and crown area. Have been on fin for 1+ year and dut for 1+ year, but Im still losing ground (scalp more visible) and shedding over 100 hairs daily. The weird thing is I started treatment quite early but my hair has still regressed over the span of 3 years. HT wont work as well for me since Im mostly losing hairs in the midscalp and crown, not hairline. I tried oral min 2.5mg for 7 months as well, but stopped as even that did not work for me. What should I do now? Im at a loss...
    Also to add: I have done a blood test for any vitamin deficiency or thyroid issues, but didnt have those issues. Im not stressed out or anything as well that could trigger a Telogen Effluvium.

      Strip surgery is always a resolution to cover ground lost should your occipital donor be intact. The only issue is with diffuse loss you'll be chasing transplants. While we might hear acceptance is key it's not always an option and surgery should be looked at as a potential avenue for quality of life.

    Hello, thank you for sharing your experience. I would like to ask about one thing. It turns out that the period of effectiveness of finasteride is limited, and depends on the predisposition of a particular person to alopecia, right? What should be done in this case if finasteride loses its effectiveness. I will be grateful for your answer

    • @MedicalSchoolJourney
      @MedicalSchoolJourney  ปีที่แล้ว +3

      Finasteride doesn't lose effectiveness over time as long as the product is manufactured consistently. As hair loss is progressive and mediated primarily by DHT, Finasteride does not inhibit the conversion of testosterone to DHT fully thus we continue to miniaturization but at a slower rate.
      Those individuals with aggressive hairloss will have increased sensitivity to DHT thus more DHT will bind to the androgen receptors and even though we may reduce serum DHT by 70% on Finasteride there will be greater miniaturization in these individuals. It's ultimately about androgen sensitivity for binding DHT which is genetically predetermined and case dependant. Those with less predisposition respond better to Finasteride therapy over those with greater predisposition.

      Women also lose hair. How many 70 year old women do you know with a fantastic full head of hair? Almost none
      Finasteride just brings hair loss rate down closer to a women's hair loss rate. Or say half way between a man and a women loss rate
      So on average you'll probably keep your hair 15-25 years longer using the drugs than not using them
      It's not going to keep your hairline forever. It's just going to slow, sometimes greatly slow, the rate of hair loss
      And that's the best we can Realistically hope for. No one has the hair of a 20 year old at age 80. Not men not women. Maybe one in a million super genetically blessed but until genetic engineering becomes a thing Finasteride is the best tool we have

    Here is the question nobody on TH-cam answers is it safe to take finasteride if you are taking other medication like blood pressure tablets or thinners or type 2 diabetic medication like metmorthin or would you get an adverse reaction? you can go as low as 0.2mgs 3x a week to stop significant hairloss, it wont be as effective as 1mg but it should still let you keep most of your ground for years to come, thats the lowest you can go. Alternatively try topical finasteride, since it has been shown to reduce side effect risks. An experimental anti-androgen would also be RU-558841 with less side effects

      @@MedicalSchoolJourney finasteride absolutely can cause your beard to fall out, it blocks the conversion of Testosterone to DHT. DHT is responsible for thinning head hair but beard growth.Unfortunately I am one of the unlucky ones to develop PFS. Even though I haven’t taken finasteride for 2 years, I experience no scalp hair thinning anymore and like I said, my beard fell out.

    • @MedicalSchoolJourney
      @MedicalSchoolJourney  7 หลายเดือนก่อน +1

      @@timpoos4635once the beard hair follicles have developed fully from dihydrotestosterone, Finasteride will not cause them to become inactive. Their dependency on DHT is during the period of their formation especially. Less DHT can cause body hair to grow at a slower rate, but ultimately not fall out and become "lost". Perhaps there is more at play to your situation and I am sorry to hear you went through it.

      @@MedicalSchoolJourney the alternative is getting transplant(s). Most have enough donor hair. I've seen enough people irl who did transplants any many not taking fina or even on minox. All had great results. But yes you will need multiple transplants if you're not on fina, but totally worth it imo.

    • @MedicalSchoolJourney

      The thing with transplants is most people who do not take Finasteride will have an expanding demand where the supply won't be met. Finasteride reduces the recipient and increases the donor.
