  • This week's livestream question comes from Laurence Galian. He commented:
    “If hypothetically, you had a patient that presented with moderate gonarthrosis, decreased joint spaces with major involvement of the medial, misalignment of the femorotibial eye, periarticular calcifications, retropatellar chondromalacia, prominent tibial spines, would you recommend a knee brace or a knee sleeve? Or only exercise? What type of knee brace or sleeve should the person buy if you recommend them? Many thanks for your help and wisdom…”

    Bone-on-bone knee pain is a common way that people in healthcare describe knee arthritis. It just has to do with the way the knee joint spacing looks like on an x-ray. If the joint surfaces appear to be touching, then you might have very worn-down cartilage as part of your knee arthritis.

    Replaying this. A few years back, I was fitted for a Don Joy brace for my right knee which is I'm pretty knocked knee and is the bone on bone. Doc said this was to take the left side of my knee there and "push" my knee back straight 😳😳 welllll, no could do! 400.00 wasted. I see now I might do better with a non-bracketed brace. My believe is how could that brace push my knee back straight when I've always been knocked kneed in that rt knee? Anyway, thanks again 🙂

      Hey Kathy! Sorry to hear you didn't get much out of the brace. Bracing is such a confusing topic for most healthcare professionals. The right exercise is so much more important for most knee problems, especially being knock kneed. Have you seen our video about knock knees?

      You'll have to see how you feel in your knees when you kneel, bend or squat down using the sleeve.
      It will come down to how much your glutes can support your movements. If you have problems while at work, you need to strengthen your glutes to better tolerate the movements.

    I am going to a gp I was referred to in Qld Australia. He takes blood, spins in & turns it into stem cells & injects the stem cells under the knee at the one appointment . I have spoken to 3 people he has treated, one was in a wheel chair so fingers crossed it will work. I think he uses the reclaim method that was mentioned at the Mayo Clinic. My orthopaedic surgeon didn’t mention this method to me & sent me away & said could back when the pain is so back & have a knee reconstruction.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  2 ปีที่แล้ว

      Hey Jen, that is platelet rich plasma injections you’re talking about. Hopefully it helps and you can find some relief!
      Injections do not fix the mechanical problem of the over pressure in the knee joint. That means that the knee pain is probably going to come back even if this injection gets you some temporary relief.
      You really should address it by doing the right exercises and starting to address how you move. We talk all about it and show you exercises in our videos on this channel. Here is a playlist of our knee arthritis videos in case you need it:

    Hi Dr David. I'm 60 years old and I like running for enjoyment as well as exercise. I have bad knees (meniscus surgery on both knees about 6 years ago) . The doctor says I should have knee replacement soon. Once I have knee replacement, can I continue to run?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  ปีที่แล้ว

      Hey James, great question! Running after a knee replacement is usually not recommended. Surgeons advise against it because of the risk of loosening the hardware and then needing a revision surgery later.
      Although, I know many of my patients that do some interval walking/jogging and feel okay, but they're not walking for more than 45 minutes at a time.
      The better question to ask in this situation is "What can I do to get my knees healthier to avoid replacement surgery and possibly continue to run?"
      Just because the surgeon is recommending surgery doesn't mean that is the only option. Most people with knee arthritis can get a lot better once they start doing the right things...even after having a meniscus problem.
      Next week, we'll be launching a Knee Arthritis Recovery Program that sounds like it might be right for you. Keep up with our channel so you don't miss it!

I love using the treadmill and desperately want to start working out, walking on the treadmill regularly again. Any help or suggestions on a brace would be greatly appreciated.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  ปีที่แล้ว

      Hey Daphne, sorry to hear about your ACL tear, meniscus tear and knee arthritis :-(
      For what you have going on, you'll need more than just a brace. Absolutely you should be in a good brace with brackets like this one:
      Shock Doctor Knee Brace - amzn.to/3hhguVR
      But you cannot rely on the brace alone to help with your knee problems. You'll need to address the underlying root problem that is setting up your knee arthritis, unhealed ACL and unhealed meniscus. The common root problem is a muscle imbalance that causes too much pressure to build up in the knee joint. That means you've got to do the right exercises at the right time.
      Our Knee Arthritis Recovery Program would definitely be the best place to start. That way you can know if this is the right time to be getting on the treadmill. You can learn more about this program here:

      Hey Nirdesh, sorry to hear about your knee pain :-(
      That area near the outer, front part of the knee can have many sources of pain. If it hurts right near the knee joint, then it's likely a cartilage problem, IT band problem, meniscus problem, or an arthritis problem.

      Hi Bill, we are sorry to hear that you've been suffering from bone-on-bone knee issues. We know everyone's experience with it is different but we know the pain and frustration that comes with it.
      We have a Knee Arthritis Recovery Program that can aid in your recovery. To learn more about it please click here: epmanualphysicaltherapy.com/knee-arthritis-recovery-program
      You may already do this but we suggest sleeping on your back with a pillow underneath your knees to help with the stiffness. An exercise that helps tremendously with stiffness is the tailgate swings.
      Our Knee Arthritis Help playlist will have this exercise and other exercises that can be beneficial in your recovery. you can access it here: “Knee Arthritis Pain Help”
      th-cam.com/play/PLJVIPiFvv1maa9YF-WmKLF7oI4VoczH8i.html

      Sorry to hear this A Y :-(
      For a recent surgery, you need to heal a lot at the beginning. Have you been up on your feet a lot? If so, you likely need to rest and let it recover. If you had a knee replacement then you need to get all your motion back as soon as possible. Not all physical therapists get this either, sometimes they over-work surgical knees to adhere to the surgeon's protocols (instructions) despite over-doing it for the joint.

      @epmanualphysicaltherapy  2 ปีที่แล้ว

      Hey Margot! Sorry to hear about your knee pain and lymphedema 😢
      If the knee pain has been there since before the lymphedema then a knee brace could be helpful. But if the knee pain came on after or became worse since the lymphedema began then it is likely that the lymphedema needs to be treated properly for the knee pain to improve.

      Hey Dorothy, sorry to hear that! A hinged brace is usually a very strong brace and can help with the mechanics of the knee. It is very important to build the muscles that are responsible for maintaining these mechanics as well. This is most often in the feet and the glutes. Hi Dr. David, Is there exercise to prevent the knee freezing when I wake up

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  หลายเดือนก่อน

      Hi Ali, we're so sorry to see that you've been dealing with a frozen knee! We currently do not have any resources for this but we will forward your comment to Dr. David so that he can consider it for future content.
      Here are some quick tips for dealing with a frozen knee before getting out of bed in the morning:
      Warm-Up: Apply a warm compress or take a warm shower before getting out of bed to help loosen the muscles and reduce stiffness.
      Gentle Stretching: Perform gentle knee stretches while still lying in bed to improve flexibility and mobility. Slowly bend and straighten your knee several times.
      Use Pillows: Place pillows under your knees while lying down to keep them slightly elevated. This can help reduce swelling and improve circulation.
      Avoid Sudden Movements: Be cautious when getting out of bed to avoid sudden movements that could strain your knee. Take your time and move slowly.
      We'd love to hear how you are doing as you progress.🙌 Please take care!
