I had the privilege to know Professor NRE Fendall. HIs work was fundamental to Alma Ata and the world owes him.

China and Cuba are the only countries successfully achieved the goal of the decleration.

No one has. If anything the world is even further away from where it was in the late 70s towards the goals of Alma-Ata, including Cuba and China. There have been great improvements in child mortality, maternal mortality and life expectancy but those living longer are living worse lives than their predecessors with lots more comorbidities. We failed on putting primary care at the forefront and money continues to be pumped into emergency care, elective surgeries, hospital admission and chronic conditions that could have mostly be prevented by effective health promotion. Health inequalities are worse than ever. Mental health is virtually non existent. Health and Social care professionals are suffering chronic burnout with failing organizations unfit for the growing demands. The system needs a reset but no one has had the guts to do it in half century and I doubt they ever will.
