Thanks for your query, Dr Anshuman. It’s because in Anaemia of Inflammation/AI ( earlier called , Anaemia of chronic disease) : transferrin level is low. So, TIBC level becomes low. If you follow my hotel-room analogy , If your total room number in hotel decreases ; the maximum number of people who can stay in hotel also decreases. The concept that you need to understand in AI/ACD the main problem is that iron is “ locked up” in reticuloendothelial macrophages, making it unavailable to developing red blood cells. So, body don’t need to make more transferrin to “ hunt down “ iron. (Unlike Iron deficiency anaemia. Where body is searching badly to get more iron.) So, transferrin level decreases in Anaemia of inflammation/Anaemia of chronic disease >> concurrently, TIBC decreases in Anaemia of inflammation (earlier called, Anaemia of chronic disease). I have not got time to discuss Anaemia of inflammation here ; but, as you asked I explained here. Hope this clears your doubt.
