My 63 year old wife had a massive heart attack and double bypass surgery on July 21st. Her labs: total cholesterol 154, HDL 54, LDL 68, Tri's 220, Creatinine 1.06, Bun/Crea ratio 17. She has had two CAC scores ten years apart of Zero ! She is a little over weight. We had been walking and never had any symptoms with pain or being winded. She could go up and down stairs with no symptoms. She had Covid in late May which did effect her lungs and recovered in early June. No family hx of heart disease , however has had high BP for a long time...under control. Would like to understand the possibilties of why this happened to her so that we can try and keep it from happening again !!!
One hint might be in her triglycerides over HDL ratio and just her triglycerides number on its own for that matter. Google that one, there lots of info out there on understanding your lipids panel. Supposedly much more important than just looking at cholesterol numbers in isolation. Basically her ratio is suggesting her LDL could be the dangerous type, small particle rather than large. There are other factors of course, but in her case (and mine) this could have been a major contributing factor. Good luck for the future and unless you have a very good Dr, perhaps even if you do have a good Dr, check up on everything they tell you, despite what some may tell you they are not God and don't always know what's required.
Dr. Ovadia is a carnivore heart surgeon. His book is, "How To Stay Off My Operating Table". He has several markers he looks at. I will share three specific ones I remember for women: A waist
@@hollyhock3945 there are several variations on those lipids numbers. One I watched recently wasn't quite as high on hdl at 54, but was much lower for triglycerides at 70 and also included triglycerides over hdl ratio of no more than 2. The idea was if you meet those 3 numbers, which are all on just a standard lipids panel, it virtually guarantees your ldl to be the good type. Worse numbers can still be OK but need more comprehensive tests to find out. Perhaps Dr Ovadia is taking other things or tests into account to get the more modest triglycerides number? Unfortunately many Dr's and cardiologists don't even know these numbers are important, never mind what's good and bad 😞 Here's the video. th-cam.com/video/rdgS3PuSuyg/w-d-xo.htmlsi=bHhsIvlnUF7riVan
I am not a physician. but this is LIkely Covid related to some degree - it's happening a lot and the Cardiologists actually seem to have no explanation.. but Covid does love to hid in Heart tissue and the immune system can dislodge plaque in fairly open arteries when it's trying to hill the Covid Virus. Did she have these high Tri numbers before Covid?? And did she have a fasting insulin number measured in the past?? or now? Had she ever smoked? She could also have a gene related to Clotting issues.
I’m a T1D, diagnosed in 1975. Watching the advances in technology and insulins has been remarkable, and I’m grateful to be able to see how much life has improved because of it. My last A1C was 5.3, and I’ve been following a low carb diet for several years. My CAC, however, was 221, which scared me. My doctor wants me on a statin, and because of that CAC score, I’m conflicted. Up till this test, I felt that my complication risk was low based on my good metabolic health. My labs are like your patient Robert’s, except for the CAC! Where does genetic risk factor in?
"Where does genetic risk factor in?" - Forget it. Metabolic risk is all that matters. What are your inflammation numbers? Also try getting even lower with your blood sugar. 5.3% is not THAT low. Do you measure your fasting insulin?
@@btudrusNever considered a fasting insulin since I don’t make any. 🤷♀️ I figure I know my fasting insulin by how much basal insulin my pump delivers overnight. My inflammation markers are all low, but obviously there’s inflammation.
I love listening to your videos! When you go over bloodwork I ran and get mine to compare! I’ve started a carnivore diet, but sadly it’s not 100% yet, still working on that part. The holidays are the worst. Plus I love to cook and bake new recipes. My family has really improved our health and diet in the last 6 months, our bloodwork has proven we are better! My doctor wanted me to take a statin, and, of course, I said no. I ask my Dr. to request a CAC and he did an I’m happy to say it was “0”. But my WBC is 7.1 Will that go down as I eat better or will my Hashimoto keep it elevated?
As a type 2 diabetic, CAC of 400, I take 20mg Rosuvastatin, not for the cholesterol but to reduce major blood vessel inflammation. What is the problem with that?
I am a T2Diabetic on Mounjaro currently on 2.5. I have not been on Mounjaro long enough to have an A1c, but I notice that glucose average is mid-80s. I controlled diabetes with low carb/higher protein diet to 6.2 and saw such good numbers coming from GLP1s that I asked doctor about starting Mounjaro. I would like to know your stance on IF/longer fasts for T2D, especially those on Mounjaro, or other GLP1s. I am also overweight and found it very difficult to lose weight, but have lost 15# in 3.5 weeks on Mounjaro without changing my diet (was already almost zero carb when I started the med).
Hahaha, I got my blood test results today and I'm unhappy with my HbA1c because for me 5.2% is quite high... I would wish I had rather around 4.8%. 😞 (Not being diabetic) But need to say that the kind of excercise I'm doing is the only thing which really spikes my blood sugar to diabetic range...
The same situation, I do a low-carb routine with fasting and exercise, and I am not satisfied with my blood sugar level. I practice resistance sports and running. What should I do? Should I abstain from exercise?
I am also restricting my carbs, walk everyday for 1 hr, intermittent fasting, no food fasting twice a week and yet I find my glucose sugar, while within normal range, should be lower. I usually walk after i had breakfast and while my 2 hr bg is around 120 mg dl .I find for the kind of effort I put in, it should be way lower. I am not diabetic. However, I am sure I eat more carbs than most low carbs people.😃Do not see much different after exercising. I experiment by taking blood glucose when I eat after 1, 2, and 3 hours, with different foods so I will know what spikes my bs. Only started this carb restrictive diet about 7 weeks ago but have been doing all the other things for over 2 years now. I can definitely say that flour must go. That's the diabetic monster, in my humble opinion😄
It’s very scary to hear that a woman with repeated scores of “0” calcium, can still have a heart attack. I just watched one of Dr Cywes’ videos that talks about how diabetes increases the risk for a heart event. What is your wife’s A1C. Is she even prediabetic-5.7 or up? That’s where I was in September ‘23. I have been on a low-carb meat-based way of eating for 9 weeks now, and expect a new blood draw in about a month. I strongly hope that my eliminating sugar and grain (all baked goods, especially) will have made big differences. I am 73 and also struggle with high bp (some damage to heart and kidneys) and obesity (5’2”, 173 lb). I still allow raw dairy (mostly milk and kefir, not much cheese) and prunes and minimal fruit. I might have to lower the milk and fruit, if the numbers haven’t improved. Oh, just identified with fatty liver.
Dr. Cywes, This might sound silly to you but I'm a bit puzzled. I would like to know if Metformin is a brand name and if Glucophage is generic in the USA. My nephew and niece-in-law from Miami visited us in Toronto for Christmas and we were chatting about medication. They both said, that Metformin is a brand name and Glucophage is generic. I said no, Glucophage is the brand name and Metformin is generic because I've never paid for Metformin but I'm paying some of the cost for Glucophage; I asked my Pharmacists what's the reason and he said, you're paying for the brand name Glucophage (Metformin is generic so you never paid)! They both told me that I was wrong. Doc, can you please clear up this misunderstanding? Thank you. As usual, this is a very educational and very well-explained video; thanks again and God bless you Sir🙏🌹
Dr. Cwyes, I enjoy quite a few cups of black coffee each day and noticed that you do as well. Just read that coffee increases Serum Triglycerides. Is there any proof of this and if so what would be the rationale for why your metabolism increases Serum Triglycerides from the consumption of coffee that does not have the addition of sugar.
CAC - HBA1C - Fasting Insulin - full Lipid panel - liver function panel - Kidney function panel - C- Peptides - IR (Insulin Resistance) or do the IR ratio trigs/HDL in mg/dl form your Lipid results, should be less than 2 and best below 1 and close to 0.5 for good Insulin sensitivity - Blood Pressure - Testosterone (optional ) not a doctor - info only but these are the markers that will plot your metabolic health over the next few years and it is good to have a baseline - said he who didn't and only have tests from after about 5 weeks in - If on meds like BP, diabetes, and insulin keep your doctor in the loop - The BP and insulin usage can drop rapidly and the dosage needs to be closely monitored early on eg I went from 140 a day down to 10 units of insulin inside 3 weeks with only the internet and no doctor - scary and dangerous oh and very stupid but hey Dr Google didn't mention that - Age 73, 20 months keto / carnivore, 36 Kgs loss, HBA1C 5.6 from 8.5, BP 150+ to 128, IR 2.0 from 3.8, LDL up, HDL up, Trigs down, liver / kidney markers all better - Pants size 34 from 46 - Enjoy and best wishes

I wonder how many endochrinologists actually understand the liver makes all the glucose you need. You dont need to eat any. Any you eat is "extra". So baseline for a diabetic should be zero glucose intake.
Moms passed away this year and she had type 2 diabetes then developed Alzheimer’s not long after her diabetes came back. The guideline was for her to have four portions of carbs spread out through her day. She was confused and no matter what she did she got sicker. Eventually gave up and had all the sweets she wanted and since she had Alzheimer’s she couldn’t remember what or how much she was eating. She had no health aid at home even tho she it was ordered and she was on hospice. They couldn’t find any workers in our area. 1 week after she was placed into a facility she died.
Diabetes with Addison’s is almost impossible to manage. On steroids my BG rises into the 300s when I eat, even after minimal carbs. And it can take 5 to 9 hours to get it down, unless I exercise, which I don’t do much due to tiredness. Is there a study that can help me, or do I just go 0 carb.
My opinion isn't worth much, but try zero carb. It's a life-changer. I was in pain with arthritis, had terrible psoriasis, and hashimotos... I've never felt better on this diet. The joint pain is gone, the skin is clear, and the fatigue lifted... Good luck.

Wow, talk about advances in technology and treatment of diabetes, my brother has had diabetes since the 1940's. We had to drive 60 miles into Houston to find a doctor who could treat him. I remember as a child watching him have to take 4 injections a day. Later it dropped to 2. He's 80 yo now, and I understand he's a bit of a rarity. He has his sight and has had no amputations. He also survived a tumor attached to his pancreas. Sadly, he does have Alzheimer's but is hanging in there.
@@robertcywes2966 that is not correct. Only a small fraction of crops and husbandry animals have been genetically modified. The vast majority went through natural breeding and selection. And how is it that the red biotech you mention here is so super helpful and fantastic, applied directly to our bodies. While green biotech is somehow "bad"?
Question- no matter how long I give my body to adjust to zero carbs, when all the carbs go, I CANNOT SLEEP! Why???? I’m 58 yr old female with very little body fat. Don’t eat sugar or processed carbs. What’s happening in my body and I have tried up to 9 months zero carbs but just can’t function on zero sleep and have to add some vegetables back.
you "cannot sleep", or you just sleep less? I find on zero carb I sleep quickly and deeply for 5-6 hours and just have to get up, no laying around. I don't think that's a problem.
Ultralente could be revived. Dr. Bernstein is a big advocate of that. But nobody is going to do that. It’s an old idea and it’s not as sexy as patentable insulin analogs.
I have never heard of it being called HgA1c, I have always known it as HbA1c. I assume these are the same thing. Hg is the symbol for Mercury, so I assume that is why Hemoglobin is abbreviated as Hb.
No matter what I do with my keto/carnivore diet, my a1c remains pre-diabetic and my fatty liver isn't going away after 3 years with no weight loss. But I've got a thyroid tumor with 50% suspicious result after FNA and my doctors are calling for removal. It's growing behind the sternum so an FNA is difficult and also I assume with the surgery. My thyroid hormone levels appear normal after multiple tests but I'm guessing that's behind my poor metabolism. Every doc on the keto/carnivore menu talks about goiters on the assumption that its benign, and not about when its inconclusive and how thyroid cancer affects blood sugar, insulin resistance, metabolism, liver function, and weight control along with sclerosis of my heart valve and bradycardia. All I got from going keto/carnivore is an ever growing suspicious tumor.

Did she take the jab? Lots of clotting and heart issues from it.
Unfortunately many Dr's and cardiologists don't even know these numbers are important, never mind what's good and bad 😞
Here's the video. th-cam.com/video/rdgS3PuSuyg/w-d-xo.htmlsi=bHhsIvlnUF7riVan
Set up a visit

I hear you can put hashimotos to rest ... stay away from gluten ? And up protein?

As a type 2 diabetic, CAC of 400, I take 20mg Rosuvastatin, not for the cholesterol but to reduce major blood vessel inflammation. What is the problem with that?

I'd love to Dr, but I live down under.

Fasting is much better to reduce inflammation then statins, LOL.
Cardiac calcium measures hard plaque (?) what about soft plaque (?)
Hahaha, I got my blood test results today and I'm unhappy with my HbA1c because for me 5.2% is quite high...
I would wish I had rather around 4.8%. 😞 (Not being diabetic)
But need to say that the kind of excercise I'm doing is the only thing which really spikes my blood sugar to diabetic range...
The same situation, I do a low-carb routine with fasting and exercise, and I am not satisfied with my blood sugar level. I practice resistance sports and running. What should I do? Should I abstain from exercise?
Every person is different
Maybe you need this level
Or you get angry easily like me
I am also restricting my carbs, walk everyday for 1 hr, intermittent fasting, no food fasting twice a week and yet I find my glucose sugar, while within normal range, should be lower.
Check your protein. Eat high fat medium protein.
Top effort doc I like your information keep it coming
Dr. Cywes,
This might sound silly to you but I'm a bit puzzled.

I am going to see my Indiana doctor, what blood work should I get to start my Carnivore diet?
Are you aware of Keto Flu - are you a big carb man if so it may be easier to go low carb (above Total 30g carbs a day) for a week or two so the effects of keto flu aren't as bad - also electrolytes - do some youtube search - Dr Cywes and other vlogs will have advice - best wishes

I wonder how many endochrinologists actually understand the liver makes all the glucose you need. You dont need to eat any. Any you eat is "extra". So baseline for a diabetic should be zero glucose intake.
Thanks Doc.
I am sorry for your loss- curious if she was on a Statin??

Good luck.
(I'm *not* a medical professional) You could try going carnivore and see what happens. After all, we each have to find what works for us, ie we are all an experiment where N=1.
carnivore (=zero carb) + time restricted eating
try excercising to exhaustion if you can at least once a week. it's worth it.

Is it correct that BGL (or insulin?) needs to elevate to a certain level to active electrolytes?
I've heard that mentioned serval times in regards to a possible downside for people on carnivore diet, where your BGL and insulin basically flatline, increasing the risk that your body isn't activating the much needed electrolytes?

Wow, talk about advances in technology and treatment of diabetes, my brother has had diabetes since the 1940's. We had to drive 60 miles into Houston to find a doctor who could treat him. He also survived a tumor attached to his pancreas. Sadly, he does have Alzheimer's but is hanging in there.

Ultralente could be revived. Dr. Bernstein is a big advocate of that. But nobody is going to do that. It’s an old idea and it’s not as sexy as patentable insulin analogs.
Richard is excellent but what works for him is prior to the new meds
I have never heard of it being called HgA1c, I have always known it as HbA1c. I assume these are the same thing.
Hg is the symbol for Mercury, so I assume that is why Hemoglobin is abbreviated as Hb.
i believe it is a mistake in the title. HbA1C is how i’ve always seen it referred to.
Do you take your Dexstom off at all?
Not if you have DM
You change out the sensor every 10 days.
And the bright side of ur life is?
