  • Do you still have knee pain after surgery?
    A common question I get is why do I still have pain after knee surgery. One of the first things I look out is whether patient has adequate range of motion. The ability to stream your knee after surgery is essential to properly function. In fact knee knee hyperextension as I described in the video is normally the goal after most surgeries. If you have a loss of the hyperextension it can result in a limp while walking, make it difficult to stand comfortable and generally affect the normal functioning of the knee. We discussed how to do a self-assessment of proper knee hyperextension in the comfort of your home. After that we discuss my top two stretches to regain your range of motion. The first one I describe is in the prop where you position your ankle in an elevated position so the back and he is not touching the floor. You can either use the way the leg to create the stretch or add ankle way to the top of the knee for 3 to 5 minutes or there is a standing stretch can do where you actively stretch your leg in standing. Both of these structures should not make your condition worse but do expect some soreness during the stretch lasting no more than three departments then five minutes after completion.

      6 months of agony and counting 😢. My plan was to recover enough to stay reasonably active ( I am 70 ), that's not going to happen. In the beginning it was do your exercises every day, that turned into you are doing to much. Worst decision of my life.

    I had right TKA in 2016 and then a revision due to loose patellar component in 5/19. Some spacers were also implanted. Post-op PT went well, and I made excellent progress. About the 3rd week of PT, the sharp pain I had felt preop, returned. I have full ROM, but still have the pain, particularly on stairs, getting up from chair, etc. I've been told "There's nothing wrong mechanically", and my pain is simply "nerve memory". I even had some test injections to evaluate my suitability for nerve ablation.
    I didn't go through with it because I REALLY didn't buy that explanation. Before the pain returned at the end of PT, I was doing great. I know about phantom limb pain, but is "nerve memory" a plausible explanation for what I'm feeling? I remain very active despite pain in both knees. I've been told I "need" a replacement of my left knee, but after my experience with 1st replacement, I'm very wary. I try to keep up with the exercises I learned in PT, along with ones I've learned elsewhere. Should I have my right knee reevaluated, or settle for the explanation I've been handed. I'm 62 years old and have been physically active my entire life.

    So awesome!!! Thanks I used to be an Advanced athlete in HS. I had a knee injury where my medial meniscus was torn and my acl was partially torn.
    I had surgery about 8 months later! Now about two years down the line I have really worked on the pain and I try to strengthen it as well, but it feels like as I squat, my femur actually “slides” across my lower leg bones. At about 30 degrees it happens and I feel like it isn’t “tight” now this really sucks and I have dealt with a lot of depression because sports and physical activity were a huge part of my life.

      You should get a evaluation from a physical therapist to see if you can correct that .

    Hey Tom. I had Two Surgeries on my left knee. One of them being because a bone in my knee was too far over to the right and was supposed to be over to the left. So they did two long cuts on my knee to get in there and push it over and do what they needed to do. I did Physical therapy to be able to use my leg again and fully bend it which was successful. Left knee has been bothering me a bit with pain so i go back to get an X-ray. The surgery they did wasn't 100% successful because the bone wasn't as close as they wanted it to be. They did put screws in my knee by the way. So They think the pain is coming from the screws so they do a second surgery to take them out. Surgery was a success and after recovery I still had the same pain in my left knee. I can't sit on my left knee because it will hurt and it's overall weaker than my right knee. My Right Knee has the same problem as the left knee did (bone being too far to the right) and I've been able to use it just fine without it having surgery like my left knee. I feel like the surgery messed my left knee up more than it was already messed up because before i got the surgery i had no pain. I'm planning on going into the Military but if I can't sit on my knees without pain then I'm Afraid they would disqualify me. Is there any way i can get rid of that pain? It's not constant pain, it's just when pressure is applied to the area of surgery (The Knee.)

    Been 16 months since ACL reconstruction with Patella tendon. Had minor meniscus tear, left as is. Audible sounds from the knee. Along with pain. Sounds like zelcro, or crunching.

    I had Acl surgery 10 months ago the biggest problem that i am facing now is the cut it self still hurts me and it is always cold sometimes when i press on that cut i feel some sort of electric shock or a pulse in that area . I do have full extension but i also feel some sort of cramping in the back of my hip when i bend my leg

    I had ACL surgery 2-1-2021 . I am 2 degrees extension when pushing my knee down. When I try to overextend my knee, my partially torn MCL hurts substantially.
    I am walking very oddly because I feel my knee locks or pops when walking. I am trying to figure out why this is happening.
    My P.T had me using a leg brace for 7 weeks due to weak quads & poor extension. I am currently trying to regain a normal walk, & just realized my knee is locking up/popping.
    •Do you have any tips or ideas on why this is happening, how to fix the problem & regain a normal walking gait?
    •ROM is 120- 125

    Question: I broke my tibia shaft 4 weeks ago (closed oblique fracture, IM nail) and have knee pain when trying to stretch the knee after it's bent too long or the other way around - once warmed up it turns into almost unnoticable discomfort but if I don't move it for 1-3 hours it becomes excrutiating to move it. Is this because of the fat pad or patellar tendon or something else? Will it ever go away and are there exercices for it? (not getting better or worse for weeks so far)?
    I can partially weight bear without pain and could even limp unassisted 3.5 days after surgery but haven't tried since (doc orders), movement/loading ironically helps a lot, x ray is normal (one half healed very fast in 2 weeks lol)

    I got PCL reconstruction surgery 4 months ago....I got zero extension...But before surgery I was in hyperextension.
    My unoperated leg is in hyperextension but operated leg is in zero extension position. What should I do ??
    Do I should get hyperextension in my operated leg as well???
    Or just leave it in zero extension???
    Please suggest 🙏

    Hi Tom. I have done surgery on May 17th on my acl and meniscus. Almost 2 months later and I can’t strech my knee. When I try to, I feel massive pain, just like I’m forcing the new grapht to stretch. The pain is similar to the pain I felt when I originally ruptured my Acl.
    I’m in P.T. and doing various exercices, but I feel like I’m not getting better.

    Hello I twisted my knee 5 months ago. Had an arthroscopy 2 weeks ago. And finding it difficult to regain hyperextension. I had a lot of atrophy on my quad and tightness on the back of the knee and pain on the lateral side.

I had my right meniscus cleaned it was torn it’s been 9 weeks but I still can’t bend my knee fully and my doc recommended me to walk without crutches but obviously I’m still limping how long till I can fully bend my knee and obviously for it be at a good stage I’m quite young and it’s really frustrating, it still clicks which I knew but not as severe and still can’t put weight down whilst walking down the stairs

      If you have swelling still present that can limit full fleixon. Are You working with a physical therapist right now?

      That is still early in the recovery. Are you being seen by physical therapist?
      In some cases you have to adjust the progression of the exercises if it’s too much for the person . Talk to your PT and surgeon if you have any concerns.

    I am located in NYC and I had arthroscopic surgery on my right dominate knee back in April. I keep having pain in my knee in between the joints . I spoke to the doctor and he keeps giving me PT , but it’s not helping and stairs are my worst enemy. Any tips ? Thanks

    Hey I'm 6 weeks post surgery and doing physio 2x a week. I feel like I'm progressing well, my issue is trying to reduce knee pain. I get pain on the right(inside) and left(outside) of my knee and under the scar at the front. It feels like pain in the MCL and LCL but I have no idea. Physio addresses my balance and strength well but not the pain. I continue to ice it everyday to reduce swelling. Thanks

      Keep your physio posted on pain levels so they can make adjustments in your program. As you get stronger in balance improves should have decreased pain. It also depends on the type of surgery you had

    Im 3 months of pattella surgery, last week i started to feel the wire on patella really squeaking my knee so hard to a point where i feared to walk normal, expecially after the last therapy..im gonna have to get checked out sooner by my doctor, but maybe you can tell me if thats normal because of regual exercise ? Now im good after some days of deep rest, but before it was just frustrating. Also, for a month now, i cant flex my knee past 95 degrees,(after i was done using machine at fiziotherapy), even after regular exercising i dont seem to improve the flexing...hope i just need some time and that thats nothing to worry about too.
    I’d be very gratefull if you can tell me some advice about it, keep up the good work!

      I’m more concerned that you seem to have plateaued at 95°. you should have a discussion with your surgeon and physical therapist about how to change your stretching routine. Tell us how you do

      @@apexmovebetter yeah im progressing really slow now, getting checked by my doctor in a week. I also use exercise bike which really helpes me, but doesnt increase the knee bend as they told me it should. Im 22 years young so i should get normal by summer hopefully;)
      One question, does forearm (head) stand affect negatively on the pattela or overal rehabilitation? When i do it, i can actually flex my knee way more freely but thats just excuse for doing it just because i love yoga;) thats why i need to get fully healed by september, when im signing up for a hathayoga course.
      Just wanted to add that, didnt want to be that long;)

    Hi Tom, I tore my acl, mcl and meniscus last year ( March 2020) and can’t seem to gain my strength back in my leg. I’ve tried doing squats, leg press etc and nothing seems to bring the mass back.. what should I do? Whenever I try to really push my leg it gets really stiff and tight and hurts when I try to bend it again

      You may want to try some unilateral strengthening exercises to help you and also work on the knee flexion if it’s still tight

    Hi, I'm 3 years post op. I can do lots of activities already. Deadlifting and jogging for a long period of time. But after those activities I feel pain on my left knee. I had ACL Surgery and meniscus. I havent seen my dr for a long time and didnt have a good rehab. Also, wasnt able to do xray after my surgery. I'm scared that I might get another surgery 🥺

    I had 3 surgeries on my left knee due to being it weak and painful. My recent one was about 4 months ago and I still can’t bend my knee completely. it still hurts just like before ;( though the last surgery was a bit more complicated than the last 2 surgeries but this ones taking a really time to get back to normal.
    What can I do? :)

      Talk to your surgeon. Don't do these exercises untiled ok’d by Surgeion.
      It can take 2-3months in some cases

    my extention of knee before acl construction was not full and now it been 6 weeks from my acl construction surgery. I still doing different movement to gain my flexion. my flextion is about 100 degrees and the extention is the same as before acl reconstruction surgery. I would like to know as I had problem with my knee extention which is about 178 degree, do you think I still able to gain my full extention with trying these movements in this clip?

    I have undergone acl surgery before 3 weeks. While doing exercise like pressing the pillow kept under the knee, backside of knee is very painful now i cant do any of the exercise , cant extend the leg , it feels like muscle catch but pain pointed in tendons of backside knee.

    I had a medial meniscus tear. They removed part and fixed. I also had a microfracture at the same time. I'm 9 weeks out and pain is getting worse instead of better. I now have swelling in my whole leg not just the knee. I am so frustrated. I'm supposed to go back to work soon but at this point I'm not even sure I can. Any suggestions

      I would talk to your therapist but you may need to pull back on certain exercises that may be contributing to the swelling. Also if it significant you want to make sure it is a not a blood clot. See your doctor or call them asap to make sure that is not the case. This can lead to a life threatening condition. It may be nothing but should not be ignored

      You may try another PT that specializes in sports rehab after a second opinion from a orthopedic surgeon.

    First time watching. I had knee surgery in April and feel like a golf ball pain below the knee,it is so tight and bothers me all the time. At night the pain hurts and I have to toss and turn to get in a postion so it doesn’t hurt. I’m told give it a year by my PT. My knee still isn’t straight. My other knee was done and didn’t have any problems like this one. The nerve endings below the knee are super sensitive. One PT worked on the pain by some kind of massage, it hurt but felt better. My major PT won’t do it just ignores it. What do I do for the pain??

      May want to discuss with Md expectations if you still have a loss of motion. Maybe another PT. Swelling can vary but range of motion depending on surgery should be attained in first 6-8weeks

      @@fouzhanshanu9392 that should slowly get better. I might be better to see a Physical therapist live to see if you have any loss of motion that is more subtle like tibial rotation.

      I would get a good evaluation in person to make sure there isn’t pain being referred from lower back. Or hip. Then look at strength deficits. Aloos 2nd opinion on TKR from another orthopedic surgeon.

    hi, i had acl and meniscus surgery 6 weeks ago and i am doing the rehab. i managed to reach full extension of the knee/slight hypertension. but each time i try to extend the knee fully again after time away from it, i have to force myself again to get the knee to hyperextend which i reach again. does that mean i haven’t gained the full extension yet or is it normal to strugggle with it after few hours away from it. thanks

      That will occur but you should start keeping the full extension without continuing to work on it. If you have continued swelling that can also prevent the full hyper extension between bouts of stretching.

    Hi Tom ! 2 and a half months ago i got my lateral meniscus trimmed and plyca removed on my left knee. I torn my meniscus while dancing 2 months prior to surgery. Since last week i had been doing my physio regularly and saw that most of my swelling had reduced was able slowly get back to workouts. Although this one week i was unwell and couldn't continue my physio and was also sleeping on my stomach bending my surgical knee. Two days ago the swelling came back with a bit of stiffness and discomfort with pain on my hip as well (the hip pain doesn't seem to be going away). Im really worried can you please give me some advice on this. I really want to get back to my training and dancing.

      At this point everything is healed but occasionally you have bouts of swelling that will rise after increased activity or even just weight-bearing.

      @@apexmovebetter I had ACL + external meniscus injury at same time. From what I was told, the operation was not 100% successful. After a few months I was dispatched by the insurance company because the ceiling € ended. Got treatment by my football club at the time.
      I still can play but I feel a bit conditioned and I wanna find ways to get better because im still young and I'm worried about future. Im 22 btw

      You should see changes in 6-8!weeks. It also depends on if you’ve been exercising at all on your own the relative change and strength you’ll see

      That is still early in overall recovery but you must continue To progress the exercises of strengthening and range of motion.

    Hello. So starting last ski season I began to have some knee pain in my left knee. I was able to ski through it and after the season ended it went away. Didn’t come back until summer when I was on a backpacking trip. About a week in the knee pain came back and it was extremely painful. I was forced to limp at some points during the trip. Once again I powered through and didn’t get it checked by a doctor. After this whenever I went on runs(one of my main forms of exercise) I would make it a few miles and the pain would immediately come back. I stopped running and the pain went away. In the past few weeks I’ve skied a couple times and the pain came back. I decided to go to a physical therapist and he says nothing is torn but after multiple sessions of pt pain isn’t better at all and I have no confidence in knee. Any advice?

We decided to start doing the knees over toes guy basic workout and within 3 days of doing this daily my knee already feels way better. Plan to continue with Ben Patrick’s program through the dense level.

    Hi Tom, I am about 9 months out of an ACL surgery, and still experiencing what I think is a lot of pain compared to what I "should" be experiencing with the amount of pt I have been doing and strengthening. About a month ago I began doing new exercises for my knee that apparently is slightly slanted outwards (An MRI showed that I had that and a tiny cyst of some sort of a really small muscle placed on the backside of the knee). However, I don't feel any difference what so ever from doing the new exercises. I am able to very very slightly overextend my surgical knee, but that comes with a lot of pain under my kneecap. The pain I have is always right below the knee and on the inside (the side towards my other leg). Also, over the past 2 weeks I have gotten a bruise right on this spot without having bumped into anything, so I was wondering if that could be a sign of some internal issues? Lastly, I hear a clicking sound every time I tense up my quad to fully extend my knee. I guess I am just wondering if there is anything specific I could be doing, as I have been told all my pain is due to my kneecap being slanted (even though I see nothing of that, but ofc I am not a doctor so what do I know), so if I "just" loosen up my IT-band and strengthen my quad it should go away. So yeah if you have any exercises or could think of a reason why I am experiencing these very specific pains, I would love to know because to me it just seems like a pain that will never disappear and the assessments I get from my surgeron are always quick and I practically had to beg to get an MRI to make sure nothing was seriously seriously wrong.
    (also in case it helps I am 21, a girl and since I checked 1 month ago the size difference of my thigh/quad is 1cm)

      @@apexmovebetter Thank you so much for answering, after watching the video again and comparing it to myself I would say I am very very close to full extension as I am able to slightly overextend my knee as explained in the video. It is lacking a little compared to my "healthy" leg but it is very close, and I am able to "lift" my heel off the ground while the rest of my leg is still on the floor like you showed (I am just not able to lift it as high as the other leg). It is painful to do, but I am able to so I wouldn't say that is my issue. I can send you pictures if you want that.

    I had partial knee replacement surgery on May 3 2021. I do therapy 3 times a week. And my knee still not straightened out it only at a 0 degree angel. If it don’t straightened out by July 13 cause that when the doctor is a virtual call with me then she will have to ho and effusion to make the knee straightened out to. Any ideas. It straightened out to a 5 degree angel from a 6 it has yo gd at 0 degree angel

      Is it possible to get proper flexion after a year of surgery becoz herd that becoz of no blood flow in that area it will not heal by it's own.

      @@thymos6575 a couple of things. First check loss of knee range of motion. Second maybe adding closed chain strengthening like a leg press, forward reaches, Deadlifts , etc.

      @@thymos6575 The ability to run will partly be determined by your tolerance for closed kinetic chain exercises. like squats that will load the joint . I would start with bodyweight exercises to assess tolerance then progress to loaded exercises. Eventually moving to jogging. You may need more guidance from a physical therapist who can do a in person evaluation. Good Luck! Tell us how you do.

    it's not the knee - after ORIF /ACL tear. It's the thigh, hip, tendons throughout thigh, feels like twisted tendons, horrific agony pain. In the groin area, sciatica , hip. Knee is fine. a bit wooden. Also, Bi-malleolar fracture with plate - wooden, arthritic pain but not so agonizing as groin, thigh, hip. Why?

    Sir i had acl op 6 months ago on my right knee. My right knee is little bit hyperxetended than my left knee. And i feel my knee is loose and while walking i feel like something is catchimg in between my joints and that feels very irritating to me

      You may have slightly loose Or needs more strengthening. Consult your surgeon or physical therapist. Don’t do more stretching until you do. Good luck!

But whenever I do exercises I feel much better..I think we both should stick to our strengthinng exercises consistently than we hv been doing
