  • Join me and my husband Mark for a day in the life of a doctor in the hospital. We’ve flown up to Sault St. Marie, Ontario to help with the doctor shortage. Learn about septic shock, acute kidney injury, hyperkalemia (high potassium) and some very unusual heart rhythms!

    Ok so of course we all know she is a literal angel on earth, but can we talk about how SO IS HER HUSBAND?!?!?!?!?! "let's do something nice for the nurses" my heart melted. Literally a perfect couple of completely angelic people.

      Truly. And so much more detail than an American doctor. I mean I know we have HIPAA and everything but obviously there's some way to de-identify the patient.
      Anyway it's much more interesting than just watching someone walk from here to there to somewhere else and tell us what they had for lunch (and how they put on their makeup, which is really boring)
      By the way I remember my father's saying Sault Ste. Marie when I was a kid. I'm not sure quite where it is but it seemed, years later as if it was way off track for where we were going. We went from Boston to Montreal for expo 68 (originally 67 but we were a little late) And then for some reason we ended up in Detroit. I remember going to some restaurant and keeping a napkin so I would remember that I had been in Detroit, but apparently I lost it. So it was decades before I remembered that I've been to Detroit. Just briefly I'm sure. Sort of like Chicago, although we went there to see Donny Osmond in 94 and then off to a Quantum Leap convention in Indianapolis. (And that's most of my experience in the Midwest except for the 4 hours we spent at O'Hare waiting for another flight to join us on our way to Denver)

      Some do. I follow some residents and dotors who are in the U.S. I am impressed they let Dr. Max Feinstein use an OR to record though.(Obviously when it's not in use) If you're interested in learning more about anesthesiology, his channel is great.

      @@skypie1 unfortunately, that's the way every hospital is. Some docs have the world revolve around them, some docs are real team players. I've worked at 3 hospitals so far and that's been the case for all of them.

    I love that you are so kind to the nurses! I have a vascular surgeon who is fantastic with the nurses that work with her and it makes me respect her even more! She relies on them and uses their expertise a lot!

    Thanks for a couple nurse shout-outs!
    So many strong team members in a hospital. Often I think house-keeping are some of the most under-rated heroes.
    PS. I've seen dextracardia twice and it is kinda cool. Once we could not get an ECG that made sense until the older, laid-back head of the department suggested we reverse the leads. Another time, a parent of a child with known cardiac issues told us as we were putting on monitors so that we wouldn't be confused.

      I agree regarding the housekeepers being underrated😢my mom is one at a hospital and sometimes she feels invisible. But I tell her that her job is very important.

      Great to see a shout out to the nurses. I have only seen dextracardia once in almost 21 years of nursing and it was so fascinating. The patient knew they had it and was amused at us trying to figure out the ecg 🤣

    I love having access to my medical records. When I had a chemical pregnancy back in September the ER doctor didn't believe I was pregnant. The urine strips were negative... but low and behold, when the blood work came back. It confirmed I had indeed conceived and was going through an early loss of pregnancy. It made it hurt more to the point I filed a complaint with Alberta Health. But I was glad it was a confirmed loss.

    We lived in Moose Factory Ontario for 5 years and locum or visiting physicians were really what allowed our communities to have access to healthcare so thank you for doing what you do and bringing awareness to these parts of healthcare in Ontario that are often overlooked.

    Edit: close. I didn't know individual organs could be on the wrong side. I thought it was an all or nothing condition.

    I absolutely love that there are doctors dentist and all kinds of different people in the medical field that want to share their experiences and their lives working in the fields that they choose to people on the internet. I've learned so much from watching your videos other people's videos letter in the medical field and I think it's so cool how much that we don't know and how much we can soak in and learn from people like you. It's also cool to see what's real versus what you see on these TV shows like Grey's Anatomy and things like that. It's so fascinating to me to see all the things you go through and what it's like really to be a doctor and to learn about these patients and how to help them. Thank you so much for what you do. I look forward to every single video you post. I hope you continue to share and give us the knowledge that we could definitely use and need.

    I take care of patients with ESRD on dialysis. I have a patient who consistently has a K+ between 7 and 9! No symptoms. She insists she doesn’t eat fruits or veggies. She doesn’t have constipation (which can cause hyperK+ in ESRD). Loved having Mark share screen time with you!

    This video VERY MUCH is like looking through my cases of sepsis!!! I've had 4 rounds of sepsis in the last 3 years. 2 of them being septic shock within 7 months of each other. My worst one was because I had a kidney stone the size of a kernel of corn on the cob that was blocking ALL function to my kidney and started shutting down my other organs. I also had a nephrostomy tube placed because of the blockage but I was already in septic shock when the tube was placed because I was too unstable to have the doctors remove the stone surgically. Due to the kidney stone shutting down my other organs I was also hooked up to dialysis for 5 days straight 24/7 for 5 days! I had slipped into a coma at home for 7 hours before my husband got home from work and had called 911 to rush me to the hospital and stayed in a coma for the first 6 days. Then I just completely came out of it all on my own. I was even extubated only 6 hours after coming out of it and then was discharged from the ICU and got to go up to the main floor by that afternoon. Doctor's cannot explain how I recovered and even how I survived this, especially when I had just gone through my first round of septic shock 7 months prior *which was due to a UTI that had gone septic because I have a subra pubic catheter and was put into a medically induced coma for 4 days*
    Add that with a spinal cord injury that happened 14 years ago that happened because I had a MRSA infection in my spinal cord that was all stemmed from an ingrown hair under my right arm! ALL of my doctors and entire medical team have absolutely NO IDEA how I am still alive and thriving with NO long term damage to any of my organs! I'm also only 37 years old and it's always interesting to watch your videos and see things from the doctors point of view! Even though I've been through all of that, any many other personal trauma/tragedies over the last 5 and a half years I'm still here, fighting every day, thriving every day, smiling every day and living my life the way I want to live it. I'm 100% independent and don't need any help getting around, driving, personal care, etc is all done on my own! I've worked my butt off for years to get to this point and even though it's NEVER easy, I've made the best of it and have learned how to live with it and work around it instead of it defining who I am and making my injury the main aspect of my life which I'm proud to say it's NOT!
    I absolutely LOVE watching your videos and how you explain and normalize so many things that can seem REALLY scary to most people. It takes a lot to scare me nowadays because I've been through so much medically so big needles, central lines, procedures, etc I feel are ALMOST a walk in the park nowadays LOL! If you'd ever like to chat about doing a day in the life or collaborating on a video together, I would be honored. I live in Michigan and always love when you get to speak with patients directly on camera to show both sides of the topic of conversation. Much like when you followed the NICU nurse and got to speak with a parent of one of the babies in the NICU!

    I have gastroparesis had a pyloroplasty the Dr. Was supposed to be great he did do great but I ended up with a fungal infection that ended up abscess I was transported bk to hospital in another. State where the surgery was performed it was closest dr that could do the surgery (pyloroplasty) so he did a drain and left me with a open hole in abdomen to pack and fill he failed to cure the infection he stated he had never dealt with this kinda infection so I was not confident he would do it right I found a local dr while septic on home hospice to remove the fistula and sinus well the abscess hole in muscle wall of abdomen was never closed because the skin closed over it and. Now I have to have surgery again to close that or I could get a hernia well I'm still very sick BP stays low and my iron is bad I have severe anemia my body won't absorb it even with infusions I went to er couple weeks ago and was released with a 21.5 white count I feel the same as when I was septic extreme fatigue I'm afraid it wasnt all cured and infection has came bk with a white count that high and the er thinks I'm crazy but my white count doesn't do that unless I have infection it took 2 weeks in beginning to figure out it was fungal and not bacterial infection so I'm worried they r just passing me over. Cus I have had many health issues I'm willing to sign a release medical if u would look at my case concerning my possible infection I'm worried

    I am not gonna lie here, that heart patient got me completely by surprise and the same goes for the patient that had the heart on the opposite side of their chest in the chest X-ray. That was really cool to see honestly!! And, if I may Siobhan, I really like your yellow coat, it's really nice!! :D Anway, Siobhan, and Mark, you guys are fantastic, and keep up the absolutely amazing work!!!! Have a safe flight home as well!! Finally, (I know I kind of ended it there) referencing one of your donating blood videos honestly moved me enough to give it a go and next Tuesday my school is having a blood drive and I'm donating. I feel excited about helping people but I'm honestly kind of scared as just the sight of blood makes me squeamish but I'm certain I will get through it!!

    You must be the most chipper doctor I've ever seen, and I've seen a lot of doctors (as a patient). I wanted to be a doctor when I was young, but as I got older, I started thinking that being around sick people all day would probably depress me. I think that's why I'm amazed at your upbeat attitude. Anyway, good job on the videos. Medicine always interests me, and since I'm at the hospital so often, I often wonder what's happening behind the scenes.

    your videos are so educational and entertaining! I will admit i laughed out loud when you asked if they checked again and did the BP on the other arm!! as an er rn i repeated the BP 7 times, 4 different cuffs, atleast 2 machines, did a manual and said at least 14 prayers before i called the Dr 😂

    What a fun post! Great to see the both of you working. I will repeat myself again, any pt that has either of you to care for them is very lucky. You both are so thorough when you are with them. You remind me of a couple of drs that I got to work with for 10 yrs, very positive, wicked smart and truly compassionate when caring for their pts. Kudos to you both!

    I love your videos, and I know you've always had to make some concessions with how you make your videos because of patient confidentiality. With the ads and how extremely obvious the nurse moments were scripted, it just didn't seem as genuine as previous videos.
    Seriously, just trying to give some constructive criticism. I'll definitely keep subbing and watching your videos.

    I am a systems engineer. I went straight to the signal interference. The only thought I had different was what's called a "Ground loop" where currents flow through the equipment (patient) where it shouldn't. Usually ground loops create a hum noise. Or problems with communicating from one thing to the other. It's detected by measuring current between different parts and seeing that it's not zero. The worst case is infamous in rock performance. A musicians microphone was connected on one side of the stage and the electric guitar on the other. He stepped up for his performance, grabbed the mic stand with one hand the guitar fret board with the other. He then writhed and gyrated, there was a loud humming, the crowd roared with his exciting performance, and he finally fell to the stage electrocuted.

    Me, a former telemetry tech: *immediately accurately guesses it*
    Didn't know the brand, obviously, but still! Lol after working in telemetry, you may not become an expert in cardiology (obviously) but you do become an expert in identifying artifact! 😂
    Such an underrated department of the hospital because we'd get such minimal patient interaction (same with lab techs, sterile techs, etc)
    I cannot even begin to tell you the number of times nurses would be rushing to a room to respond to a "VTach" only for us to tell them IT'S NOT REAL! It's just Ashman phenomenon, or afib with abberency, or artifact! (And we'd be right)
    OR on the other hand, *real* VTach and nurses would say "maybe they were just brushing their teeth?" Noooo, it was REAL VTach! (And we'd be right)
    And we'd see all types of things! Bifascular blocks with p axis deviation, rate dependent bundle branch blocks, torsades, ventricular escape beats, etc
    Not to mention that we not only aided in the intervention of many medical emergencies, but we also aided it determining time of death when a patient did expire.
    Telemetry is great. Even if we do annoy the hell out of nurses 😉

    i keep getting concerned lately, because i keep hearing the helicopter come in to the county hospital. obviously its used to lift people to University of Iowa, when they cant take an ambulance up there. not sure why so many people are apparently needing lifts...

    One of the ultrasounds of our identical twin boys revealed that one of the boys’ heart was facing the opposite way on the other side. The other twin was normal. This is the only other time I had heard about something like that!

    IM LOVING how you are making your videos show a lot of the interdisciplinary team involved with doctors, especially the nurses! I feel like a lot of medical vlogs never include that and as a nursing student myself I love seeing this team work in real practice

    I forgot to mention, I Love watching your videos! I was diagnosed with MDS in November.Also, I have RA and Systemic Scleroderma. I have learned so many things from your videos. I forgot. I also have Von Willie brand disease. Keep-up the positivity and great outlook into medicine! ❤

    Yazbut.... Patient access to medical records is in general a very good thing. There's one problem, though - Some non-urgent results, e.g., routine labwork might be reported at midnight or 2-3AM. Patients access the report, and find that their ALT is one point above "normal" or one point below; neither has ANY clinical significance. But then the patient calls the answering service with an emergency call because of abnormal labs that the physician hasn't had time to review in the middle of the night. Typically, the lab will either call or text if there's a "panic number" so little chance of a patient being harmed. The net result is a lousy night's sleep for the doc, for no reason. Been there, though I'm now retired. Also had middle of the night calls for non-endocrine issues (I'm an endocrinologist) to discuss non-endocrine results ordered by someone else because their primary MD is "grumpy at night."

    Hey,
    I love all your videos but this felt quite personal to me. I had vesicoureteral reflux as a child I was told I’d grow out of it. I didn’t. My ureter kept blocking and had several nephrostomies. My kidney was eventually removed. Now I have CKD stage 3b on the remaining kidney. Which now leads me onto my potassium levels. They got too high so I too had the dextrose treatment. Now I’m following a low potassium diet and taking sodium bicarbonate and fingers crossed it’s working. Xx

    Not a medical profeasional in any form, but ...Did the patient with dextrocardia (spelling?) have the other organs flipped as well or was the xray just zoomed in? It also looked like more tissue/organs and/or shadowing on both sides. Is this just so we could see it better or is something else going on too?
    I'm a systems analyst so I look at alot of data vs. visual results for a living. I overthink everything.
    P.S. I love all your videos. You are so beautiful and intelligent and you look just like my Mama. More than i do, lol. Ya'll could be twins!

    I just found your page, I was a paramedic in a hospital and on the truck. I have been out of EMS for about 3 years now and its really weird not working 24 hour shifts anymore or knowing when you are going to get to eat or sleep. Everytime your pager goes off I get that little adrenaline rush again! Please keep posting your videos are awesome!

    Septic shock is no joke. I've had a few people who have had septic shock, and the amount of work that is needed to get ahead of it and manage it appropriately is insane. Even from a pre-hospital standpoint, we need two large bore IV lines (which is not easy when they are so hypotensive), IV fluid boluses to maintain BP, pressors in case the fluids do not work, and potentially airway management if they are so altered from normal consciousness that they can't protect their own airway.
    Long story short, don't play around with it lol.

    I love watching your videos as you just get started in your career! I am a retired X-ray tech with 30+ years in the hospital, and then retired due to cardiac sarcoidosis and advanced heart failure and all that goes along with that. Keep posting your videos, I love them!

    I wonder how much of this is re-enacted. Not that it's a bad thing, but having to do it a second time seems like a lot of work! :D I went to medical training as well it would be insane to have to do and record all these conversations with other people in the healthcare delivery team.

    My sister’s baby had his heart and other organ’s on the opposite side. He passed at 6 mos after a pediatrician misdiagnosed a bowel obstruction as a virus. I thought there was now a named syndrome for this condition? Love your videos!
