    I actually used ABC/DEFG at work the day after I saw this! I work in a small animal emergency vet and remembered to pull out our glucometer as I was setting up for a stat. Its amazing how much overlap there is between vet and human medicine even with all the differences!

    As an oncology nurse/ FNP student, as soon as you said she started a new medication for RA, I automatically thought the pancytopenia was due to the immunosuppressive medication! Love to see how far my critical thinking has come. Thank you for being such a great teacher!! Love your videos

    When I heard the patient that had mouth ulcers I thought “maybe it’s from methotrexate “ as I have RA I’ve been on methotrexate (injections) and although I never had those ulcers I really felt for that patient. My rheumatologist was very clear on the dosing of my medication but I can see how it could get confusing, especially in tablet form. I had to discontinue MTX for other reasons…basically wasn’t tolerating other side effects 💉

      I had that thought too, but from the other side as a pharmacist. We have lots of alerts to make sure methotrexate is written and dosed correctly, but patients forget sometimes.
      Mouth ulcers, pancytopenia, rheumatology, and pneumonia all point to some sort of overdose of immunosuppressant! Glad my guess was right :)

    When I was on methotrexate, it was a weekly injection done in the office, and a blood test EVERY month.

    wow! good student… when I had very low red and white counts (2 tests) no followup. When diagnosed with IBS in 1995, no followup or suggestion I join a study (found one in newspaper, disqualified because I had Amoxicillin in previous month). No followup with my leaky valves…

    As a 911 dispatcher my longest call ... car vs pole we did have to get a trauma surgeon to the scene and there was a need for amputation there because EMS and fire were unable to extricate the patient. Yes I can talk about it because it made the news. That was a 5 hour scene call plus post call team debrief. Alcohol and street racing do not mix.

    I watched this because I wanted to see the shoulder injection, but like always, I enjoyed the whole video. I just started working in a pain clinic where they do injections, but I haven't had the opportunity to see an injection (I'm the first nurse the patient sees, and I do the IVs, but I don't see the fun parts 😀.

    Being disabled and having to utilize a wheelchair for 48 years and going in and out of hospitals here in the us, i prefer to teach a new student about me, nurse or doctor

      Hi! Good job keeping yourself and others safe by masking up! I'd recommend (if you haven't already) upgrading to an N95 if possible, as they are vastly more protective than surgical masks or cloth masks (even when compared to double masking).
      If you're interested, one that people often like is the 3M Aura 9210+ (or the 9205+ which is the same mask but with a less durable band), but your mileage may vary and other ones may work better for you!
      KN95/KF94s are also better than surgical or cloth masks, but generally fit less well than N95s due to using ear-loops instead of the around-the-head band, therefore giving you less protection, but even so they're still an improvement over surgical/cloth. Be careful about counterfeits!
      If cost is a concern for you (it is for many of us unfortunately), there are still options available to get improved respiratory protection.
      If you're in the US, Project N95 gives away free high-filtration masks (www.projectn95.org/free-masks/).
      If you're in Canada, Donate A Mask gives away free high-filtration masks (donatemask.ca/)
      There are also many local "mask blocs" which are activist groups that offer free high-filtration masks, so try searching up for a mask bloc in your area!
      There may also be other organizations in your area that give out free high-filtration masks.
      And if you're concerned about mask use and the environment, you may consider getting a reusable P100 mask. They have even higher filtration than an N95 does, and are less waste, having to only replace the filters occasionally, rather than the entire mask.

    At least Grey's Anatomy mostly works in the hospital. The Night Shift was always flying off to somewhere to do medical things out in the field. When I got the chance to rewatch it It occurred to me that I just didn't want to. I prefer seeing things in the hospital. And starting on the 2nd season of Code Black they got Rob Lowe and he was always jumping out of helicopters and climbing up on cranes and I didn't like that either. If I want to see EMTs I will watch them. If I want to see a hospital I would like to find it in a hospital drama.
    And of course the people on house did all their own tests and I would swear occasionally they did surgery as well although only one of them was a surgeon. And they transport the patients and all of that stuff. ER was a lot better at mostly realistically showing who was doing what. But Grey's Anatomy is still my favorite. One year we had seven medical dramas. But there was one that was mostly a woman fighting with her father over Which one of them was going to be the chief of surgery. And the father was very childish. I didn't like it.
    Okay back to reality.

    of patients with diabetic neuropathy 😒Those taking Gabapentin... within 5 years, a substantial
    number of patients who experienced Myocardial Infarcts, Strokes, Heart failure, Peripheral vascular disease, Deep vein thrombosis
    Pulmonary Embolism,... I have to add that this is only my reflection on the content of the report / study and has it has to be remembered that WHO and WHAT other medications, medical conditions etc HAVE to be taken into consideration that may well have influenced or caused the conditions shown to have occurred is not indicative of an absolute nay or yah. BUT it is nevertheless interesting to someone who is taking Gabapentin especially if their dose has just been increased to 3 x 300mgs DAILY.. ??

    I'm 70 years old. I have arthritis in both legs and cervical spine. I have pain and stiffness in my.legs sometimes in.my.knees and wrists and cervical. I take Walmart 8hr 650mg arthritis strength and a 325mg aspirin every day for pain.

      That's how work life at an university hospital works, you're taught by somene senior to you and with each step you take up the ranks it continues to work like that. Med students are taught by junior residents who are taught by fellows who are taught by attending physicians. Or like in this case.

    For a risky drug like methotrexate, shouldn't the pharmacist talk to the patient to make sure they understand the dosage schedule and the risks? Or did that happen in this case, and the patient didn't understand?
    I was impressed by how Emily got to the bottom of the problem. It required some outside-the-box thinking.

      In Australia, we usually get fact sheets and the pharmacist will briefly go over it too

      I'm guessing the patient misunderstood the directions or mixed up the doseage schedule with another drug. You wouldn't believe the amount of times that happened when I worked in a pharmacy

      When I was on it, I had friends telling me I must have heard the doctor wrong. If I was in that much pain, why was I only taking my medication once a week... I tend to listen to my doctor not my inexperienced friends!

    I guessed it correctly. Thought the patient was taking a dose that was too high but wow, daily dose of what's supposed to be a weekly dose.
    On the topic of medication instructions confusion, I've never forgotten an episode of ER where a patient didn't understand English so when they read "once a day" on the bottle, to them it was "11 a day" when read in Spanish.

    I’ve had a bunch of those shoulder joint injections as well as spinal injections into my cervical and lumbar spine. They hurt like hell , but with physical therapy, really help the pain

    I loved getting the opportunity to see you teach a medical student because I ended up learning a lot as well. That's one of the main reasons I watch your videos. I'm not in the medical field but I've always had a strong interest in learning things surrounding that field and your videos help me do that, especially when you make videos with other people that you're teaching or working with on certain days.

    I am a certified (advanced practice) nurse midwife. We are considered healthcare providers in the US. I have been working in nursing and medicine for over 40 years. I would have loved you mentoring me back in the day. You are awesome in so many ways. I’m 72 years old, but if I was 30, I’d wanna be you when I grew up 😋
