He was administered Valium and taken to River Valley Centre. Mr. Smith first presented at emergency two weeks ago. By the time he arrived, he was much calmer and his behavior had normalized. He was apparently suffering a psychotic episode.

Depends on the patient circumstances. For instance, you could use the chronological order for a normal referral where introduction would be followed by past medical and social history and then development of patient case (A to Z). Whereas, in case of an emergency letter, your introduction must immediately be followed by the current situation.

First of all I should say you are amazing with teaching OET. You make things simple. I am very thankful. I just want to share and ask about something. Something might be wrong with structure. In the real life, doctors are focus on to share story of patient before the examination findings. Normally, the structure is composed of history of the disease, medical and surgical history, family history, examination findings and laboratory findings respectively. In the finger example physical examination finding which shows swelling could be take place after patients complaint. When it comes to writing referral in OET, it could be better to stick to your recommendation.

Mr. Smith's moods have been fluctuating. When he first presented, he was calm. A day later, he seemed rather agitated. Today, he seemed calm again.
