Hai Sir, some are telling that the date is mentioned in the question. For example, in this letter the datebhasvto be the discharge date. Is that correct. I'm so confused about this. Is that wrong if we right the discharge date?

Let me first tell you I had no idea whatsoever what was the test all about. I registered 20 days before the exam. I checked you tube too many people did videos but Jay's were the best for me.
Regarding speaking it was fine though I had some stumbling "grammatically" and I corrected it immediately. I didn't write down notes though afterwards I thought it might have been useful to do so. My advice to stay calm and confident through it. I literally thought I screwed it but guess what it was my highest score😄
2)writing : it was a referral letter, I took Jay's advice regarding summarizing and general layout. It was not hard, though I revised my capital letters and punctuation which i found some mistakes in them.
For me the hardest section was reading, was tough and I had to stick to exclusion of the wrong answers. I was left with almost 2 closely similar answers which I had to go with my intuition for them😁 time was tight on that section and I was delusional that I would have enough time to recheck some answers (lightly shaded them) but once I was done, time was already up

I have two questions about this sample.
1- Shouldn't there be an article "a" before "recent surgery" in the purpose paragraph?
2- In the request paragraph, why was it used the preposition "of" with the verb "to provide"? Could the author have used the prepositions "to" or "for" instead?
Surgery is uncountable.. so can't use a before surgery.

In some case notes it's not possible to write the paragraph of purpose expanded after the paragraph of purpose because there's more relevant information in the patient's past medical history which has to be included before the detailed purpose.

It depends on the purpose and also the reader .Here reader is a physiotherapist and he is not needed to inform about other clinical conditions which he can’t help with..

Yes. It's important to mention the full name of the patient in the introduction paragraph. Later on, mention them with a second name.
