As a physician (albeit not a vascular specialist or hepatologist or pulmonologist) a few of the medical points are wrong. His nails don’t look like clubbing although you couldn’t be sure without a proper physical exam by a specialist. The point about hyperaldacteronism is conjecture and you can’t be sure without checking a renal panel. And the point about enlarged umbilical veins is inaccurate, enlarged porto-umbilical veins (caput medusae) is an end stage sign of decompensated cirrhosis. While steroids certainly cause liver damage and can increase the risk of certain neoplasms, no bodybuilder would be able to maintain even a fraction of their physique (due to the protein wasting and inability for the liver to synthesize SHBG and other important proteins) if they got to the point where they have portosystemic shunting like that. For reference, the pts who I’ve cared for who have cirrhosis with caput medusae were extremely frail and on the brink of death waiting for a transplant. Great video but keep in mind the medical points here are just theories from someone who is well versed in bodybuilding science and general health but not in medical conditions. Happy to hear points from medical professionals (physicians, PAs, nurses in these specialties) and what your opinions are about these points.
The veins which are outlined appear as the inferior epigastric veins in which there are signs of contortion and migration towards the umbilicus, their presentation seem to be a recent change based on previous pictures from years ago in similar conditions. Their presentation does not align with stigmata of liver disease, no conclusion is drawn by this gross presentation, however, this feature should not be completely disregarded as it does appear atypical based on the anatomy of the inferior epigastric vein. Could it be nothing ? Possibly. Could it be an early presentation of peripheral artery disease (such asan aorto-iliac occlusive disorder)? I've seen similar presentations in the literature in case reports. The pre test probability is not insignificant given the present risk factors

These are definitely the inferior epigastric veins, however I would not even characterize these as varicosities, they're just prominent branch veins in a person with low body fat percentage. I map inferior epigastric vasculature for my vascular surgery colleagues and in my personal opinion this is within normal physiologic limits. You would need a CT angiogram to diagnose this, a visual conjecture is not sufficient. In the CTAs I've read for patients for arterial atherosclerotic disease, when it gets to the point where there is occlusive aorto-iliac disease, the patient has almost always required numerous lower extremity arterial angioplasties, stents, and bypasses. If you're referring to IVC (inferior vena cava obstruction), that is also a huge conjecture with a severe clinical picture with severe lower extremity edema , and enlarged collateral deep and superficial veins. You would need a CT venogram to diagnose this, a visual inspection is not close to sufficient. I have only seen a couple cases of IVC occlusion on the thousands of CTs I have read in sick, hospitalized patients and the imaging and clinical scenario is severe. I asked Mike why BB instead of powerlifting, and he replied it's his dream. I think he'd be a much better powerlifter, but each to their own. I'm neither a BB or powerlifter, just a weightlifter that really enjoys the gym.

As someone who suffers from severe anxiety and panic disorder I can already assume Mike won’t reply on this video. I can tell he loves bodybuilding but this will hit him very hard(maybe too hard). But I do hope he prioritizes his health first. Anxiety is terrible especially when you’re just a normal guy and it comes out of nowhere then never leaves. It seems if he pushed any harder his overall health will decline rapidly(physically/mentally). I love his content and hope he is still around long enough to inspire the youth even if he can’t hit his dream goal. He’s already a pillar in the community

It's a huge risk if something goes wrong, but training for and being a safe skydiver is just as healthy as not doing skydiving.

Sure, there are some new rules that may come into effect, but it's unwise to say they haven't factored this in and are attempting to continue providing the services they do to others. I'm not even on TRT, but they help me with a ton of other issues I have and wouldn't know about if it weren't for them. The screening-first pipeline they use is second to none, really. I've never experienced anything like it in years of care as a normal not-rich guy. Thankful for him every day. When it comes to knowledge and wisdom regarding training he is the GOAT. No influencer ever had such a big influence on the way I train. And he is funny af. But Greg is right, he needs to stop the juicing and competing ASAP.
Yeah just because he is very knowledgeable about the subject doesn’t mean he himself should be doing it. Unfortunately some people are just not meant for competing and would be better off just being coaches. Hey Greg, 100mg of is not usually given for heart failure. Usually we dose at 12.5-25mg/day titrating up to a maximum of 50mg/day for that indication due to the risk of hyperkalemia. At that dose we're probably looking at potential treatment of ascites due to cirrhosis, hypertension, or primary aldosteronism. I would perhaps suspect cirrhosis if there's clubbing of the fingers but this is all speculative. Hope this helps :)
I think it shows how dedicated Mike is that even though his genetics aren't great he is still pushing towards his goals. I'm also sure he is aware of the risks Mike is definitely a big dude and knows his stuff. I hope he is able to get his pro card this time around
I wish these guys the best man. They walked so we could run. But I will say something. Chemically induced anxiety, paranoia and anger is fucking scary. I’ve been on antibiotics for 4 weeks and had another dose lined up for two weeks time. I was so down, mood would swing like a pendulum. I thought I was going crazy, even got therapy penned in, which I will still attend. I had five days off this antibiotic. I felt incredible, stable. Strong. Started back up again, symptoms returned. So I googled the side effects… rare, but, paranoia, panic attacks, mood swings, depression - well. That certainly answers some things. So I feel for these guys when they say they get this shit from chemicals. It’s mad. Also, shout out to the Manlets out there!!

I suffer from chronic colitis, even with a very strict diet. What steroids do to your guts alone is unreal. All the liver and kidney swelling also puts stress on your intestines. Not to mention heart issues.
Dr. Mike has given so much helpful lifting advice. Helped me make great gains. That said, I totally thought he was already in his mid-late 40s. I gotta say, I don't see him getting his card. For someone lifting for 20+ years, his physique has almost certainly peaked. If that is the case, he's not a top 3.
I love Dr. Mike. I'm rooting for him to get his pro card and get out. Anyone who's ever had a goal you've strived for a VERY long time to obtain should understand and respect Mike's decision to pursue his goal. With that said, everyone who loves Dr. Mike wants to see him healthy and live a long life. His sense of humor is so refreshing, and the knowledge he passes down to everyone is invaluable! I feel the same way about Coach Greg. He's the first person that came along on YT when I started lifting and eating more healthy 18 months ago.

Low dose testosterone like 200mg a week and train 2-4 days a week with weights, depending on your training programme. You don’t need to go to failure every set but you should have a couple of sets in there that you do fail on. Progressive overload, more weights, more reps, extra set, etc
All the positive and encouraging things coach said about mike in this video (and there where lots of them) will go unnoticed. People only commenting they fighting, forehead this, belly that…
I think Mike is essentially a man of science. As a scientist, he care about doing actual experiment and being right about the hypothesis than anything. You can say it's a mental ego, intellectual desire, but it's possible that it's not money-related.

@@tvsa7 the thing is, Dr. Mike has a pretty academic way of presenting his ideas. He (almost) always has a powerpoint, and take care to argue his assertions beforehand for the sake of refutability (specific to scientists). (it looks like a thesis presentation, but with d-jokes and Arnold impersonations 😄) This minimizes the risk of misunderstanding and unintentional drama. But doesn't stop the intentional drama 🙂
No joke i just had a cardiologist appointment this morning and i got another 2 week monitor. Watching this out of coincidence is not helping my worries(not on steroids I'm just a string bean with possibly a genetic heart issue). That clubbed finger part is scary
Every pro body builder is literally putting their health at risk for the silly sport. I respect the craft and the people doing it, it is brutally hard. but I’m also calling them all silly risking their health to win a modeling contest.
To you it’s a beauty contest but for us who compete it’s a goal that we’re passionate about. U probably have a hobby or something ur passionate about that others think is the dumbest thing ever. Just means it’s not for them.
A modelling contest is no different than spending your life chasing success in a running contest or a jumping contest. Every single sport is inherently meaningless if you break it down like this. It doesn't make one less silly than the other. Money is surely the biggest contributing factor here but since he likely has enough of it so I'm assuming it's purely for the kick of it.
This is pretty depressing because I honestly think Mike is a great guy. His knowledge and advice on training, diet, and overall pharmacology is very useful. I just hope he stops all the blasting of gear before it’s too late. 250 LBS of muscle for a man who’s 5’6 is just insane. A lot of pressure on the heart for sure.
I’m a moderately big man and I’m a shade under 6ft at 225lb. When I hit 240lb I felt like absolute garbage. It was awful. Taking 15lb off completely changed how I feel. So I cannot imagine 250lb at 5ft 6in. That’s insane.

Not really. A lot of what Greg said in the debate was true. Mike was just asking him to site his sources and Greg didn’t have good sources. But at the end of the day, the research is usually behind people who are actually practicing the profession especially in a sport like body building, and that showed itself true over time when Greg was proven right about most things in that debate
Hey Greg, my fingernails are similar. I went to a couple doctors who ran a bunch of tests and everything came back normal. I never took a lot of GH, and never stayed on GH for more then a few months. One possible reason could be sleep apnea and lack of O2. When my weight goes down, my sleep apnea lessens and my finger beds/nails look much healthier.

Regardless of the health risk, Dr. Mike knows the risks and his condition, and he does it knowing it as is his right. He loves doing it and has a goal. That’s personal freedom, and he has set a date where he’s gonna stop, regardless of reaching his goal. Very calculated. Is it the healtiest option. No, but he knows that.

Sacrifice isn't limited to "getting up early and working hard", it often involves sacrificing your health and you don't sacrifice just for your "family and community", you can sacrifice for yourself. People sacrifice some of their health to achieve their goals all the time, his goal is to get a master's pro card and he is willing to sacrifice his health (potentially) to achieve that goal.
Gym was meant to get you healthy. Now people have body dismorphia and do multiple terrible things to themself trying to look something they will never be. That is sad.

I was a fat disgusting slob before losing over 100+ lbs. Took me 12 years to get a decent body. That entire time I would have ABSOLUTELY taken gear if I knew a source. Glad I didn't. I'm 45 and look much younger than both these guys. The struggle is part of the game, my dudes.

I mean in mike's case money could def. be a factor but if I were bodybuilding for the few compliments I get in the summer months about my body I would probably not continue doing as that is faar to little positive feedback lol. “It’s never enough.” This is known as avarice. Bodybuilders - more than anyone - should know only so much is needed to have success in life. Only so many reps, so many sets, so many workouts, so much protein/carbs/fats, so many calories, so much cardio, etc.,. bodybuilders live by the principle of sufficiency. Probably because you take low amounts. I’m the same, I take low dose test and came off my antidepressant. I’m way calmer and controlled. But I don’t blast huge amounts. Huge amounts would probably swing you the other way.
honestly, I can relate to what Mike said. I would like to meet some of these IG commenters in real life to see how fast they run away from me, since they most likely will not be so brave if they actually have to see me in person... that goes for any social media platform where people feel like they can say anything they want no matter how negative. Nothing will ever be enough for worldly people, because the void left in the soul from living for the world makes people insatiable. Nothing is ever enough. A goal is never a destiny, but a tease, always leaving people thirstier and hungrier. No matter what people accomplish, someone will always be better to them. Someone will always have something they can't have. They will never feel #1. Only one person can fill that void, and then, just existing will be more fullfilling and satisfying than all the fame and money in the world. What you do and what you own has as much worth as what you deposit in the toilet. Bodybuilding is fun, and competitions are interesting to watch, but in the end, as everything in life, it was worth nothing and just an exercise in futility, just entertainment before death. Then is the time to show your true accomplishments and investments. What do you have to show for? You arrived naked and poor, because nothing you ever did before losing your heartbeat counted for after you lost it. Take care of your health. Exercise. Have fun. However, don't live your life for your body. Live it for when you finally lose it.

Just discovered Mike's videos not long ago, he is such a nice and helpful guy with a great sense of humor who puts a lot of the fake and Hollywood fitness in its place and his honesty is truly, truly appreciated, however it saddens me to see a person like him damaging his health like that and getting himself in what can perfectly be a miserable late adult life and shortened lifespan (he is free to do so but I am also free to say so)
As someone who works in a peptides laboratory these are extremely worrying signs. These are things that would have alarms ringing for any one of these symptoms. The fact he ticks every box speaks volumes about what can happen when you abuse peptides like GH, IGF1 and Insulin

As a physician (albeit not a vascular specialist or hepatologist or pulmonologist) a few of the medical points are wrong. His nails don’t look like clubbing although you couldn’t be sure without a proper physical exam by a specialist. The point about hyperaldacteronism is conjecture and you can’t be sure without checking a renal panel. And the point about enlarged umbilical veins is inaccurate, enlarged porto-umbilical veins (caput medusae) is an end stage sign of decompensated cirrhosis. While steroids certainly cause liver damage and can increase the risk of certain neoplasms, no bodybuilder would be able to maintain even a fraction of their physique (due to the protein wasting and inability for the liver to synthesize SHBG and other important proteins) if they got to the point where they have portosystemic shunting like that. For reference, the pts who I’ve cared for who have cirrhosis with caput medusae were extremely frail and on the brink of death waiting for a transplant. Great video but keep in mind the medical points here are just theories from someone who is well versed in bodybuilding science and general health but not in medical conditions. Happy to hear points from medical professionals (physicians, PAs, nurses in these specialties) and what your opinions are about these points.
The veins which are outlined appear as the inferior epigastric veins in which there are signs of contortion and migration towards the umbilicus, their presentation seem to be a recent change based on previous pictures from years ago in similar conditions. Their presentation does not align with stigmata of liver disease, no conclusion is drawn by this gross presentation, however, this feature should not be completely disregarded as it does appear atypical based on the anatomy of the inferior epigastric vein. Could it be nothing ? Possibly. Could it be an early presentation of peripheral artery disease (such asan aorto-iliac occlusive disorder)? I've seen similar presentations in the literature in case reports. The pre test probability is not insignificant given the present risk factors

These are definitely the inferior epigastric veins, however I would not even characterize these as varicosities, they're just prominent branch veins in a person with low body fat percentage. I map inferior epigastric vasculature for my vascular surgery colleagues and in my personal opinion this is within normal physiologic limits. You would need a CT angiogram to diagnose this, a visual conjecture is not sufficient. In the CTAs I've read for patients for arterial atherosclerotic disease, when it gets to the point where there is occlusive aorto-iliac disease, the patient has almost always required numerous lower extremity arterial angioplasties, stents, and bypasses.
If you're referring to IVC (inferior vena cava obstruction), that is also a huge conjecture with a severe clinical picture with severe lower extremity edema , and enlarged collateral deep and superficial veins. You would need a CT venogram to diagnose this, a visual inspection is not close to sufficient. I have only seen a couple cases of IVC occlusion on the thousands of CTs I have read in sick, hospitalized patients and the imaging and clinical scenario is severe.
Again, I would caution suggesting severe diagnoses for people when you are not their medical professional who is privy to their medical history.

I asked Mike why BB instead of powerlifting, and he replied it's his dream. I think he'd be a much better powerlifter, but each to their own. I'm neither a BB or powerlifter, just a weightlifter that really enjoys the gym.

As someone who suffers from severe anxiety and panic disorder I can already assume Mike won’t reply on this video. I can tell he loves bodybuilding but this will hit him very hard(maybe too hard). But I do hope he prioritizes his health first. Anxiety is terrible especially when you’re just a normal guy and it comes out of nowhere then never leaves. It seems if he pushed any harder his overall health will decline rapidly(physically/mentally). I love his content and hope he is still around long enough to inspire the youth even if he can’t hit his dream goal. He’s already a pillar in the community

I'm not even on TRT, but they help me with a ton of other issues I have and wouldn't know about if it weren't for them. The screening-first pipeline they use is second to none, really. I've never experienced anything like it in years of care as a normal not-rich guy. Thankful for him every day.

Hey Greg, 100mg of is not usually given for heart failure. Usually we dose at 12.5-25mg/day titrating up to a maximum of 50mg/day for that indication due to the risk of hyperkalemia. At that dose we're probably looking at potential treatment of ascites due to cirrhosis, hypertension, or primary aldosteronism. I would perhaps suspect cirrhosis if there's clubbing of the fingers but this is all speculative. Hope this helps :)

I’ve been on antibiotics for 4 weeks and had another dose lined up for two weeks time. I was so down, mood would swing like a pendulum. I thought I was going crazy, even got therapy penned in, which I will still attend. I had five days off this antibiotic. I felt incredible, stable. Strong. Started back up again, symptoms returned. So I googled the side effects… rare, but, paranoia, panic attacks, mood swings, depression - well. That certainly answers some things. So I feel for these guys when they say they get this shit from chemicals. It’s mad.

I suffer from chronic colitis, even with a very strict diet. What steroids do to your guts alone is unreal. All the liver and kidney swelling also puts stress on your intestines. Not to mention heart issues.
Dr. Mike has given so much helpful lifting advice. Helped me make great gains.
That said, I totally thought he was already in his mid-late 40s.
I gotta say, I don't see him getting his card. For someone lifting for 20+ years, his physique has almost certainly peaked. If that is the case, he's not a top 3.
I love Dr. Mike. I'm rooting for him to get his pro card and get out. Anyone who's ever had a goal you've strived for a VERY long time to obtain should understand and respect Mike's decision to pursue his goal. With that said, everyone who loves Dr. Mike wants to see him healthy and live a long life. His sense of humor is so refreshing, and the knowledge he passes down to everyone is invaluable! Low dose testosterone like 200mg a week and train 2-4 days a week with weights, depending on your training programme. You don’t need to go to failure every set but you should have a couple of sets in there that you do fail on. Progressive overload, more weights, more reps, extra set, etc

I think Mike is essentially a man of science. As a scientist, he care about doing actual experiment and being right about the hypothesis than anything. You can say it's a mental ego, intellectual desire, but it's possible that it's not money-related.

This is pretty depressing because I honestly think Mike is a great guy. His knowledge and advice on training, diet, and overall pharmacology is very useful. I just hope he stops all the blasting of gear before it’s too late. 250 LBS of muscle for a man who’s 5’6 is just insane. A lot of pressure on the heart for sure.

I’m a moderately big man and I’m a shade under 6ft at 225lb. When I hit 240lb I felt like absolute garbage. It was awful. Taking 15lb off completely changed how I feel. So I cannot imagine 250lb at 5ft 6in. That’s insane.

Not really. A lot of what Greg said in the debate was true. Mike was just asking him to site his sources and Greg didn’t have good sources. But at the end of the day, the research is usually behind people who are actually practicing the profession especially in a sport like body building, and that showed itself true over time when Greg was proven right about most things in that debate
Hey Greg, my fingernails are similar. I went to a couple doctors who ran a bunch of tests and everything came back normal. I never took a lot of GH, and never stayed on GH for more then a few months. One possible reason could be sleep apnea and lack of O2. When my weight goes down, my sleep apnea lessens and my finger beds/nails look much healthier.

Nothing will ever be enough for worldly people, because the void left in the soul from living for the world makes people insatiable. Nothing is ever enough. A goal is never a destiny, but a tease, always leaving people thirstier and hungrier.
No matter what people accomplish, someone will always be better to them. Someone will always have something they can't have. They will never feel #1.
Only one person can fill that void, and then, just existing will be more fullfilling and satisfying than all the fame and money in the world. What you do and what you own has as much worth as what you deposit in the toilet.
Bodybuilding is fun, and competitions are interesting to watch, but in the end, as everything in life, it was worth nothing and just an exercise in futility, just entertainment before death.
Then is the time to show your true accomplishments and investments. What do you have to show for? You arrived naked and poor, because nothing you ever did before losing your heartbeat counted for after you lost it.
Take care of your health. Exercise. Have fun. However, don't live your life for your body. Live it for when you finally lose it.

Just discovered Mike's videos not long ago, he is such a nice and helpful guy with a great sense of humor who puts a lot of the fake and Hollywood fitness in its place and his honesty is truly, truly appreciated, however it saddens me to see a person like him damaging his health like that and getting himself in what can perfectly be a miserable late adult life and shortened lifespan (he is free to do so but I am also free to say so)

As someone who works in a peptides laboratory these are extremely worrying signs. These are things that would have alarms ringing for any one of these symptoms. The fact he ticks every box speaks volumes about what can happen when you abuse peptides like GH, IGF1 and Insulin
