    Thank you for caring and all you do for us. Speaking for myself, you are my go to person. I can’t afford physical therapy. Yes the truth is I get frustrated at times but I’m not giving up. You are a godsend. Have a great day. 🤗

      You are not alone. I get lots of messages from people saying the same thing (they can not get physical therapy). That is why I do this. I am glad it is helping and I hope the second video cleared up any confusion from the first video. Hang in there and as you hear me say in many videos, focus on that 1% improvement. You CAN do this. :)

      I really love your videos. I am a hemiplegic, left side since 2011, I am working on myself everyday. I had an aneurysm first due to being bulimic, purging, then after the vein burst then the stroke happened. I would like more ankle strengthening exercises. Thank you for doing these videos.     Controlling my breathing helps relax the "monster" aka tight muscles.
    FOR MY ARM using my cane to "bench press" has helped my reach. Now I can use a 10 pound barbell to bench press. She is right it's more about practice then strength but always control your breathing!!!

    I had a stroke in August 2015, a bleed into my right frontal lobe left me paralyzed on my left side. With great rehab I was placed on a road to recovery. However my gait, balance and walking stamina have been slow to improve. Your vid’s have helped me tremendously. Thank you, thank you!

    I found your channel recently and I am obsessed! My father had a stroke a year ago and we have made progress, but things are still pretty hard. He does have some PT but 45 minutes a week each of PT and OT doesn't do much. I have been trying to use my self-taught knowledge of physiology and exercise (20+ years of being a nerd and working out) to help retrain him on my own. It is slow and frustrating. I have him walking without a cane again, poorly and he still doesn't have stability in his effected leg and his balance is 90% I would say (he balances well but his brain doesn't quite realize it and have confidence).
    In the 10 days I have been watching your videos I have realized that I was on the right path with some things, some things I totally missed some important distinctions (like disassociation) and have learned a ton of exercises that break movement patterns down even more than I had thought.
    I have had him putting his effected leg up on a chair to help with disassociation and after just 4-5 minutes a day for 2 days, when I get him to walk, I am already convinced it is improving his stride.
    Sorry for the long story - but Thank you! I know enough to recognize who does and who doesn't know their stuff. You are THE expert IMO. I wish his PT and OT therapists were this good.

      Your dad is very lucky. I can tell he is in good hands with you. :) The fact that you picked up on the dissociation component, applied it, and see the results gives me confidence that you will continue to find creative ways to challenge your father. Use these videos to add to what you already know and understand.

      @@rehabhqofficial I only picked up on it because of your videos. Also, understanding spasticity more has been a huge help. As you can tell, I could go on and on about my story. I am a little scared to say that I don't think his therapists know half of the things you do. It is obvious you are not only extremely knowledgeable, but also unusually caring and kind. I hope you know how much you help people in need, going through difficult times with little support.

    I had a light bulb moment this morning, whilst watching this video. After a disabling stroke five years ago, I lost all function in my left side. I can now walk with the aid of an electrical stimulator, but still have no function in my left arm and hand. I used to be left handed. However, I have learned to write and do embroidery with my right hand, which is very satisfying. However, my physiotherapist has always told me to slow down when doing my exercises and now I know why. I cannot get my hand flat yet, despite using my splint, but I will not give up hope. I stretch out my arm every day and use a variety of different exercises for different parts of my body. The fact that I am still improving gives me hope for the future! Thank you for your inspiring videos. I believe I will get some movement in this appendage that used to be a functioning arm!

    I love your videos and use your exercises for my rehab. I had a spinal cord injury 2 years ago and was nearly paralyzed
    I have hyperspaticity drag my right foot and hyperextend my knees when walking . Dry needling has helped with stretching and strengthening.. your rehab videos have helped me with balance and coordination

    Thank you so much. Recently suffered a Brain Stem Stroke on January 24, 2021. Right sided numbness but gained mobility on the 4th day. Now, I’m experiencing spasticity. I keep my hand open and stretch my arms. Thank you so much for such great info ❤️

    You have the best stroke recovery channel on youtube tara!,im a 10 year severe subarachnoid hemorhage survivor and ive gained some back from my left limbs but my main issue is my left fingers and toes"fine motor skills"my vision has field cuts on the left side and when i look in the mirror my eyes look walleyed "out if alighnment"or crooked,i love love love your videos as you put in in a great perspective for us survivors or caregivers to understand,please keep making videos,you are doing gods work tara!you are gifted and im learning a lot from your videos!only if therapists knew your knowledge,you should hold seminars for the up and coming therapists,i wish i could go back to the begining stages of my recovery to watch all of your videos,i cant thank you enough for your work!❤❤

    than you so much I watched your knee kick back video yesterday I was able to bend my knee while walking after doing those exercises than you so much although today my heal is quite sore from my plantar fascilitis keep up the great work

    I had brain surgery due to tumors removed my right arm and hand was paralyzed before only my issue is my hand get fisted involuntary. I stopped the therapy session due to financial issues, I don’t have money to pay and I lost my job after the operation (July 2020) I would like to say thank you for this wonderful video, I learned a lot, this video is very informative.

      Hi Tara. Thank you for sharing your story. With ALS it is really important to protect your joints and prevent muscles from becoming fatigued. That being said, bracing is an extremely valuable tool. I hope that helps.

    Thank you Dr Tara. This is such a helpful video. I’ve recently come across your channel and subscribed. I have spasticity in my EHL (muscle that controls big toe) as a result of an axonal shear brain injury I had when I was 14. I broke my leg in the accident and was in a coma for 2 weeks. I spent 3 months in hospital. Bad shape. That was 35 years ago and I’ve coped with spasticity since - competing at sport in some cases to a very high level, but accommodating my spasticity. I don’t know whether just getting older or during lockdown i now seem to have developed severe pain in the tendons around the ankle and walking/running can be painful. I’ve been diagnosed with both drop foot and spasticity (I think it’s the latter based on your advice) and I’ve found such inspiration from your videos and advice. Thank you. I’m not ready to throw the towel in just yet and I’m keen to learn as much as I can to make things better. No one has explained things that I experience as well as you. I have so many questions still. Richard.

      I have primary progressive multiple sclerosis. Had bad fall and landed hard on both my knees. Almost a year ago. I cannot walk with rollator and can't stand up. Can't straighten either leg. Currently have in home PT coming out but still can't straighten legs

    Thank you so much for discussing spacticity. When you go to therapy there is no time to talk about it. The videos have explained what I do right and wrong. I take botox shots and I am way overdue. You sure made me understand the way I walk and how to overcome it. I thought I could start walking and sooner or later my walk would improve. I think you are the best and you have a spot reserved in heaven.

    Miss Tara Tobias I've suffered from my tbi since 2016 and my hand spasticity has gotten better barely I suffer from trigger finger. But, either way this is the best stuff I've heard and seen yet!!! Thank you!!!

    My wife and I love your videos. You are so knowledgeable! Is there a wrist/hand splint you recommend aside from the air splint? My wife has should and elbow movement coming back, and saw one video where you strongly recommended a splint for the hand/fingers.

    This was s helpful, what is recommended to help relax the muscles just above the elbow joint in the front of the arm. Seems like the more I walk the tighter the arm draws up

    Dr T Tobias, please help on how to disassociate the movements of my upper limb to my lower limb. During my walking exercise my fingers clench and wrist and elbow bend inwardly. Even during brushing my teeth my left hand bends upward then slowly moving down after that. Thank you.

    Thank you so much for your great videos I have greatly improved but it seems my major problem is my left fist it is always clenched with my nails diggin my palms and if I try to straighten it, it folds back to the same position meanwhile I have heard from my neurosurgeon and therapists that the hand is always the last to get fixed but I’m trying so hard to beat that assumption and I believe I will get there soon

    I suffered SIDS at the hands of a negligent nurse after birth and was diagnosed with cerebral hypoxia. My issue is not really as noticeable until I get nervous. Then I tense up and my mild spasticity is there. In a year, I tripped 7 times over rugs in my house to the point I now have minor tendinitis in my wrists from landing on them. What do you suggest to do to help avoid tripping? Also, another real problem is that I cannot carry a full cup or full beverages on trays. I spontaneously tense my lower body without wanting to and spill the beverage by squeezing the cup or tensing uncontrollably, or even jerking my hand without wanting to. I was told I have considerable strength in my wrists, however, the jerking, tensing never seems to go away. My right side is effected and my right hand has difficulty forming a sphere of the hand - meaning my ring and pinky finger stick out and palm sticks up when I try to pick up a ball for example. But how can I stop spilling my drink when I either walk with a drink or even attempting to grab a drink when I'm sitting? Thank you!

    Do you have any suggestions for exercises/stretches for toes curling under on affected foot? Thanks and I just found your channel and am catching up on all the good info. My wife had a pontine stroke about 6 months ago and is slowly but surely making progress with arm/hand and leg/foot.

    What is the best hand splint in your opinion? Would you recommend an elbow splint as well if the elbow is constantly flexed especially during walking? I have terrible flexor tone in my biceps

    I have spasticity on my left arm/fingers,my muscles are strong though but my fingers are somewhat stiff and I have difficulty in using a fork and leaving a cup of water on the table,it seems like my fingers are not leaving the cup ,a sensation of stiffness,do you advice me to do push ups or lift light weights,which I feel I could do or is it dangerous to do so?

    Hey Doc. I have a huge confusion. See before my stroke. I never used to send commands to get my hand down or swing it. But now when I walk hand tend to come to my tm thigh area and I have to send command to use my triceps to get it down again. Will I regain the involuntary action again? I believe that one of the Desires of Patient suffering from paralysis

    What about AFO to promote ankle dorsi flexion position and to avoid toes making contact with floor first. We use it often.

    Than you for the videos. I have a question regarding my affected arm, In order to avoid atrophy I try to extend it by itself without any assistance. Whenever I do this, my hand (which most of the time is fisting) trembles rapidly. Is that alright or a normal reaction?

    Hello Dr Tara, I am struggling to understand if I have spasticity because my doctor didn't use that word, but I have heard from people that I have it and my legs do not feel like your explanation. I had stroke and my remaining limbs to come back are my thighs in the sense that I can.t walk faster. am able to do thins like mow my yard etc. My question is, does the condition fit into the definition of spasticity? Am starting physical therapy tomorrow. Thank you.

    TO THE POINT...I have a partial (PSCI) I always see videos on SCI, but not partial. It is 2022 now I had a neck fusion in 2002 when they found out what I had. I think it was a C4-5 injury, to the outside edge of my SC. I have spasticity & clonus., (my condition could have been a lot worse), but on the other hand I can do almost anything SLOWLY. I have some weakness from atrophy all over my body, mostly my left side and legs. I did not have a stroke or any other disease. I have been as active as I can be, walking, hiking, stretching, yard work, etc. I'm a woodworker-designer. I do less building and more designing now, "From the Chair work''. I found that in order to improve my physical condition, I should quit designing and become a yoga teacher, where I can spend the entire day stretching and breathing, LOL Do you have any information or sources regarding PSCI and what I would call mild spasticity and clonus? I like your videos and have been sending them to a friend who lives around Sarasota, FL. I'm in NJ. He is in much worse condition than I am. He had a lumbar fusion, (because of pain), He was active and could do anything before the fusion. Now he can't walk or control bowel movements. He has been in PT for a bout a year now, with some improvements,. I sent him your video on the functional stem unit. I thought maybe that unit could jump the damaged part of his nerve that got messed up during the fusion operation. Any information for me and my PSCI-spasticity-clonus and my friends condition would be greatly appreciated, birdiemiller@comcast.net Thanks for making your PT channel. BTW my daughter became a PT, she works at Kessler, in NJ, working with brain and SC issues.

    Running out of superlatives. If you have a quick answer, my dear friend that I recently met at the gym, suffered a stroke 4 yrs ago. His arm, wrist and fingers have the classic severe spacisity. Is this permanent or can it be improved with proper stretching over time?

      Thank you. I splinting and stretching is essential. It definitely will not go away by letting it stay in the "fisted" position. :)

      Fabian Wilson that is an excellent question. There are so many factors that need to be considered. 1. The severity of the brain damage, 2. How long spasticity was present prior to treating it. 3. How long since the stroke/brain injury. Time is the killer. most people wait too long before they really start taking their rehab seriously. I think many people initially are in denial or think (in the back of their head) they are just going to wake up in a week and be better. That being said many don’t really start taking their rehab seriously until reality sets in. That can really have a major impact on how far they can recover. What makes for a good outcome? 1. Treat spasticity early 2. Take your rehab exercises seriously from day one 3. Stay consistent with your exercises 4. Believe in yourself and your medical team. I hope this helps

    Hello doctor , Your videos are just awesome covering lots of minute information.. In my case My fingers move involuntarily when I am kind a tensed (like when in new place / in a place with lots of people around) .My hand/fingers stay normal otherwise .
    I have made it an habit to hold onto something(usually my mobile) in my affected hand to stop the involuntary moment of my fingers. Not sure if that's going to make things worsen
    Request your kind advice on how to deal with it. #strokearm

      Thanks for watching! Generally speaking, it is good to "block" any involuntary movements to prevent muscles from adaptively shortening (which will make it harder to use the hand). Splinting is really important for this. However, using your phone is not a bad idea :)

    I have MS and after being hospitalized last January for double pneumonia as well as aspiration pneumonia. Requiring me to be on the ventilator for a while. I always seem to take a long time to get my self back again. I have to work really hard to be able to handle a fork, pick up a glass etc. one thing new happened this time. I seem to get vocal spasms. It’ll make it impossible for me to form sentences without a lot of pauses and stuttering. Is this related at all maybe spasticity of the larynx?

    Hi I've seemed to have lost skills I once had since stroke. In the early days I could do a lot more with my hand than I can do now. I've not let up on using the hand but it is definitely going backwards. Any idea why.?

    I just sunscribed to your channel and my queztipn is o had a stroke back in 2016 i have no movement on my leftside i had contracrure release on my left elbow and have wrist contractur also but i also have shoulder sunlaxation on my left affected side so what could i be doing to get thT shoulder back up ive just been told there is nothing i can do about it my bra strap wont stay up on my left side and my shirts fall off of my shoulder also when im woth family they are always pulling my shirt up over my shoulder What Can i do to try to get that shoulder back up and moving please help

    I never did have spasticity until I was over 60 yrs old but i was having trouble my sciatic nerve so the spasticity started up idid have 2 strokes the day after my. Birth. I was diagnosed with mild spastic cerebral palsy at 6 weeks old.

    My mom 91 had a major stroke and made it to rehab. But it took 4 months to get there. Now she has a leg/knee that (frogs) up to the side and she is also experiencing pain in her hip and upper leg joint during rehab exercises. Insurance will only cover rehab for so long, especially if she doesn't show signs of certain progression. Do you have anything insight to help her please? She is becoming more cognitive and showing signs of determination to get up and move more and more each week, but it seems like we are racing against time. Any help and insight would be greatly appreciated.

    Pls does stroke affect vision because I had stroke about 6 months back and I noticed that my vision is getting bad all the time I don’t know if glasses can be of help or should I keep on waiting for my brain to heal but I also had brain surgery craniotomy

      Sometimes i experience double vision sort of like looking into a magnifying lens and sometimes my visiom is kinda blurry i do need to look right to keep balance and to register my vision to a clear mode. Usually, when i look down, i feel dizzy or the floor is too far. I cant distinguish clearly a far object though i see it in general. All these vision problem usually is from my left eye. But if i cover one eye alternately, i can see clearly and normally. Thank you so much for your time. I've been following some of your therapy and exorcises. It was of great help for my recovery 💗

    what about botox for fingers an arm? It's been 3 months I can walk almost well I can move my arm and elbow limited but not bad but I don't have any out move on my wrist and fingers. do you recommend botox? I'm 33 years old

      Be patient, don't use botox,be determined, you're young,get saeboflex, if your arm 💪 flex up, get saeboreach, you gonna be fine. By God's grace you gonna be back on your feet.

      Stroke for me in early 50s. Did botox on arm and fingers, it helped big time. Once i could release the spasticity, i concentrated on strengthening it. Over time , i was able to decrease botox treatment. I havent done any for almost 2 years. Botox in conjunction with exercise worked for me. Botox isnt for everyone, it is expensive and doesn't always work , plus it can have serious life threatening consequences.

      Botox has helped me tremendously.I survived a massive cerebral hemorrhage /stroke 20 years ago.I had recovered 80% of functionality in my left hand until I developed an infection in the middle finger in 2019. After lots of treatment and minor surgery I had extreme spasticity and contracture in the left hand and even up to the shoulder. If began having botox injections every three months and after 3 treatments I was much better! It may not be for everyone but I am going to continue.

    I think I have better understanding about working through my spasticity for my arm. But my ankle/foot is a different story. I suffer from " role ankle ". However, my toes doesn't point down. When I go to bend my leg my foot goes up enough to clear the floor but my ankle stills rolls. When I place my foot down, the bottom/ outside of my foot hit floor first before it goes flat. And When try to focus on placing my foot down heel first, my knee snaps back. What can I do to try to strengthen the ankle to get the foot to straighten up .

    I got hemiperalysis 7 months ago on my left side of the body. My legs recovered within a week but arm started recovering after 3 months. Now I can move my arm to a limited motion but my wrist and fingers doesn't move. Can you help?

    Hei Dr Tara Tobias I live in Norway I just saw your video about spasticity. It's about my son 19 years old he suffering from side effects of radiation he had brain cancer. He has stiffness in his left side. We are doing stretching every morning. And wax bath on his hands. But we don't have a professional physiotherapist (little complicated we the health system here).     Thank you! Have you noticed a relapse or worsening of spasticity after contracting covid? That is what my mom is going through. She was doing amazing but after catching covid her spasticity went into overdrive

    I had a scheming stroke 08/2018on my right side. Left side is weekWhen having tightness in your hand the same as having spasticity? Also trying to work my left and hand the same like the right hand. Trying to get back to being normal again as well. & Yes it is very frustrating
