Doctor here. Some, people have reported altered liver enzyme levels outside normal range due to long term Tretinoin use though, and we know Vitamin A is toxic in high dosage. I am still not convinced it is good to use long term or as an anti-aging alternative.

So helpful. I found that trentinoin works on my very mature 71 year old skin. Then I see a popular TH-cam channel mentioning this. I get so tired of people trying something and if it doesn’t work for them they decide it is dangerous and bad for everyone! Thank you for sharing ❤️

Yep tretinoin has a ton of solid data for safety and efficacy for anti-ageing. Dermatologist Dr Natalia Spierings' mother has been using it nightly for 40 years and has fantastic skin. She got onto it on the advice of her husband who is a neurologist.

Association is not causation. Yes. This phrase reminds me of an article I read that was titled Increased death and hypertension linked to napping during the day. Well everything I know about napping during the day previously had always suggested it was healthy. When I read the article, I learned that it’s not necessarily the napping causing high blood pressure and death, it’s the correlation between people napping during the day having poor sleep patterns at night, the nap itself was not the issue but the lifestyle link of people who need to nap during the day may not be getting enough sleep or adequate sleep at night.

My skin loves Tretinoin. I’ve been using almost a year, and my skin is so much smoother and with less sun damage. It’s even slightly less oily; it’s a game changer. Once you go Tretinoin you don’t go back! It’s truly a great drug.

The fundamental problem is obviously that the patients were already ill. If they were also smokers, which seems to be the case, I don't see how this study has much value. I think they need to follow healthy subjects and see if a pattern emerges. Considering how prolific use of Tretinoin is this is frustrating.

I started using Tretinoin 0.25 for acne in 2009. However, these days, I am nearing 50 now. I get red splotches on my face. So I don't use it anymore. I use Olay and other creams. I had a pretty smooth forehead despite a high-tension job. After Mom passed away in 2020, forehead lines started appearing pretty fast - almost overnight, and even my hair started growing white and losing hair. I am still mourning her.

I have used Tretinoin for 20 yrs. Still alive at 74...I even have a ten year 3 packs a day cigarette history I am not proud of from age 20 to 30. Most everyone smoked then...I promised myself when I finished grad school I would stop and I used the psych techniques I had learned to get off cigarettes. Essentially I knew I had a behavior chain of how cigarette use was triggered in my life ie associations with other daily actions and habits : first cup of coffee light my first cigarette. Put the keys in the car ignition in the am headed to uni, push in the cigarette lighter, start the car light a cigarette and much of it was study related as well. I stopped cold turkey on a trip to a conference to present a paper. I realized I would be in a great city I wanted to see and EVERYTHING would be different including coffee with chicory at Cafe du Monde each morning...four days of New Orleans cured me...I got on the plane a nonsmoker for the rest of my life. I jokingly call it the N'Awlins cure.
I am very thankful for Tretinoin...my skin is wonderful. I had some acne scars not deep, I had crows feet, my pores were enlarged but I now have very fine smooth skin on my face and neck and even backs of hands the age spots were gone in 6mos. I used to avoid upper eyelids, neck, above and underlip areas but worked into those as fine lines appeared by using moisturizer followed by every other night use slowly, gradually working up to every night following Dr. DRAYS advice!

Thank you for clarifying this subject. As a veteran I receive my health care from the VA, and would never trust their doctors, or the results of any research they conduct. VA: Tretinoin causes cancer. Also VA: “It has never been shown that PTSD is relevant to the mission of the Veteran’s Administration” ( VA response to the PTSD study proposal submitted by Dr. Bessel Van Der Kolk MD. ) Dr. Van Der Kolk left the VA after his proposal was rejected, then went on to work for Harvard University’s Massachusetts Mental Health Center, and become a leading trauma expert. The VA still hasn’t gotten the memo. Enough said.

My nephew was a combat medic in Afghanistan, and suffered a coma and PTSD as the result of being the sole survivor of an attack on the Humvee he was riding in. The VA is not at all doing a decent job of treating his headaches and PTSD. However, most psychologist/psychiatrist will NOT accept insurance, and their rates range from $300-500/hr! Who but a multi-millionaire can afford that?? The VA has my nephew drugged within an inch of his life!

    • @madaleinemulrey8545
      @madaleinemulrey8545 2 ปีที่แล้ว +17

@@silvereight6054 I’m happy to see that you are satisfied with the care your family members have received at the VA Healthcare System. That said, please understand that many veterans do not share this experience, which is why the *reported* veteran suicide rate is #22ADay. ( The reported rate is based on data provided by only 21 states, which do not include California, Illinois, New York, or Texas. The actual veteran suicide rate is estimated to be more than 60 per day.) My point being that in 1978 the VA was fortunate to have the doctor who would later become one of the leading trauma experts on the planet working for their agency, but as is typically the case with the best, and brightest doctors, the VA bureaucracy met his dedication with contempt. Even now, the VA bureaucracy refuses to implement Dr. Van Der Kolk’s treatment protocol, along with other options which are proven to be effective, to the detriment of many veterans suffering from PTSD. This is shameful, unacceptable, and a national disgrace. As Dr. Van Der Kolk stated on his website:
“I currently treat only a few veterans and I am not intimately familiar with what is happening at the Veterans Administration. The VA is a pretty closed system-people who work at the VA rarely attend my workshops or lectures, or those of my close colleagues.
I hear that their main focus is on cognitive behavioral treatments, and on desensitizing people to their memories. These treatments, research shows, are not particularly effective. That is not surprising, given that being traumatized is not an issue of faulty cognition-the problems emanate from parts of the brain that have little to do with cognition.”

The VA has gotten a LOT wrong, we do not use the VA facilities, we have other Dr's, my brother in law does use the VA and they miss a LOT in his health, I had to tell him to get them to biopsy several spots on him that DID turn out to be skin cancers...but a retired Dermatologist also got one spot on me wrong too, I told her that this one spot bked a lot, she said I was just too rough on my skin, told her I really wasn't, finally went to a plastic surgeon who removed the spot on my face that ny gut told me was a problem, it came back to be cancerous, so can't say it's just the VA in things...some Dr's just assume too much and DO NOT listen to the patient...and that seems to be getting worse across the board even more so recently...I've use Retin-A a long time, and even though I do smoke its not had any bad affect with me, granted I don't smoke a lot, but its still never affected me, my lungs are very good...I dont drink, or take a ton of prescription drugs and I think they over look the 2 combined factors that smoking AND drinking have greater incidents of lung cancer, or smoking and opioid uses also seem to be more causation in lung cancers...smoking is NOT good for ur lungs but its not the main cause of lung cancers in my age group of 60+ to 75 years old group, I've paid attention to ppl who DID get lung cancer and in that group of ppl it was smoking plus alcohol use at a very high rate...
I dont trust VA studies, I know of one person who was in some sort of trials thru the military of experiments that has become psychotic, hes young and the tests caused him to be mentally ill, he thinks the military/government is bugging his phone, his walls have cameras in them, but it was only after he was in an experiment group of late 20's ppl who now have mental illnesses due to that study...my favorite family Dr was retired military, so its not the Drs per say just the more link of the Dr's NOW not truly listening to patients concerns...

This ties into my recent question for you, Dr. Dray! “The absorption through the skin is negligible”, so why is it so controversial in pregnancy and nursing? I know that it’s not been studied but some research has shown it is not harmful during pregnancy. I wish there was more discussion about this for women that are struggling with acne

It’s because it’s a class c drug: no animal studies have shown harm to a fetus however there haven’t been any well-controlled studies actually done on pregnant women to prove it’s for sure safe. Typically class c drugs are given to pregnant women if the benefit outweighs the risk. I suppose in this case they don’t assume controlling acne is enough of a benefit to justify the potential risk.

Veterans are exposed to conditions that are unique to veterans and these conditions often increase veterans health risks (ie burn pits, agent orange, chemicals etc.). Add to that tremendous stress, sleep disruption, questionable food quality--all of these factors increase the health risks to veterans.

I am a veteran and lost my job during the 2008 economic crash so i lost my health care benefits and had to go to the VA for the first time, i was in my fifties. After 8 months of being enrolled i had 2 upper respiratory infections that they refused to give me antibiotics for even though i went back several times, each time until i was so sick that both times they told me to go to the ER; makes no sense. At this point they took me off of all medications that i had been on for decades cold turkey which caused so many other issues; i became very ill and ended up in the ER several times with high blood pressure stroke alerts. They also thought i had reproductive cancer so i had a surgery to take it all out but there was not a thing wrong and than they refused all hormone treatment except estrogen only which is dangerous. So i would not take anything the VA has to say seriously; they do not like to give medications and i feel come up with "studies" they have done with ridiculous results to back themselves up because every time i challenged them that is exactly what they did, recite some ridiculous study that they did that makes no sense at all. I had to go to a civilian doctor and pay out of pocket to get proper care. I have been on retin a for over 30 years and i am just fine. Do not take the VA seriously; they do not know what they are doing from my experience with them.

Due to the advanced ages of the study participants it brings to mind "agent orange". There is a severe form of adult acne that so many of our veterans were stricken with. Along with lung cancer and damage to many other body systems. I cannot fathom why veterans in this age group would be chosen for a tretinoin study. Seems rather strange.

Here is a tip from someone who has been using RetinA 0.05% for the last 40 years on his face ..wear nitrile gloves when you apply and massage the creme ! because the skin on your finger tip absorbs about as much as the skin on your face, so you waste 50% of that precious tube into your finger..also reduces overall systemic absorption. Buy nitrile gloves one size larger and you can re-use the same glove many times.

These studies sometimes conflict and I wonder if generally the role of antioxidant’s in potentially exasperating lung cancers is something that we have yet to fully understand.

I heard of the study that said vitamin E increases risk of lung cancer in smokers. The possible theory to explain this was that smoking damages cells and their dna over the years, and damaged cells are often programed to die, apoptosis. The antioxidants may repair the damage just enough to prevent apoptosis, but the DNA is still damaged and a those damaged cell have a higher chance of developing into cancer. Basically, antioxidants may save a cell that should've died. That is a logical possibility.
With regard to current cancer patients getting chemo, antioxidants may interfere with the ability of the ability of the chemo, a poison, to kill the cancer cells. It may rescue them from death in a similar way.

Within 2 months my face started breaking out again, like everyday 3 new pimples popped up, I got back on those meds so fast..it’s been 4 months since I stopped and started again and my skin has finally started easing up on breaking out. Dr Drey. I've been following you for a while a few years ago my neck started aging prematurely.. I started using retinol nightly and after 2 months I was able to reverse the aging,, however recently I had to have eye surgery, for a retinal tear and during my research for a proper recovery I found that retinols can cause problems with your tear ducts, I was wondering if you could do a video and educate people on what would be a good alternative if you're predisposition to eye disease to use for sun damage skin other than retinol. Thank you for educating us on skin care.

did you experience the "retinol uglies" or did you get your skin used to it over time by starting with a lower concentration, then going to higher? how much time do you spend outdoors, and has it ever been an issue even with SPF? I'm going to start tretinoin soon (age 30) and I'm an outdoors lover, and recently I got a retinol burn even while wearing sunscreen and I'm just nervous!!!

Hello! 😎 Love your channel! Please comment on Tretinoin and dry eye- I’ve used Tret for 15 years or so and I was just diagnosed with dry eye. Some studies say Tret can dry up your Meibomian glands over time which is a huge component of dry eye and to stop using it. Help! I love my Tret but having a dry eye flare up was petrifying. Thank you Dr Dray! ❤️

I also had these symptoms swelling and styles. It went away when I stopped retinol.
Worst part is continuous use may cause permanant atropy of meibomian glands leading to permanant dryness.
All opthalmologists i consulted asked to avoid it :(
I am in same situation, nothing works like tretinoin but dry eyes look like really scary condition

Oh My! I have long suspected this! After I put the cream ( mixed with CerVe) on my legs, I got extremely dangerous brusing and blood clots! I tried the experiment five more times ( I was in such disbelief!) and it happened each time! For 20 years now, I have struggled to get up stairs..even a simple walk! I have lung cancer in the family, but never smoked, but lived with smokers...I also used Accutane 2x and then started retin`a , 38 years ago! So, perhaps, that combined with a genetic thing, can indeed cause it. I Love the effects of my retinoids~huge fan! I guess I should be trying something else?? Sad news..but, since the problems started after I really got into my retinoids, I may be one of the unlucky ones. 😥😳
