    I've been on testosterone topical medication for many years. My doctor at work first diagnosed what she thought was the problem and I talked to my PCP. He agreed and put me on topical gel. At that point my T level was way below normal. Since the time when I started the medication, my T levels have increased but at best are just average. I do have more energy and my libido has definitely increased. I came down with a urinary tract infection last fall and my PSA numbers skyrocketed. This is not unusual. My PCP took me off the meds and referred me to a urologist. He explained the situation and performed a blood draw. By that time the infection was over and my PSA was back to normal levels. I went back on the medication at that point in time. Great video. Thanks for showing it.

    My testosterone levels were in the normal range for my age. But my free testosterone was on the low side. Cypionate every two weeks and I’m golden 💪🏼. Definitely need to stay active for results. Or as active as your particular age allows.

      Yes, I had to practically beg my primary care doctor to include these in my blood test. She was not willing to do testosterone treatment. Thankfully my testosterone has been even lower the less couple of blood tests, also found high prolactin levels. she’s finally ordering an MRI of my brain. checking for tumors of the pituitary gland. Depending on what the MRI results are, I will either follow up with my primary care, or she will refer me to endocrinologist.

    Oral T is NOT desroyed in the stomach. It's absorbed into the blood, but something like 80% of it is metabolized by the liver on the furst pass, so you have to keep redosing all day to maintain a level which overworks the liver and leads to temptation to use too much which just exacerbates the danger.
    Injections or cutaneous are a more controlled and steady release. I think cutaneous can give more of a natural cycle when applied in the morning.
    The problem with supplemental T is that adding T screws up the feedback loop of the H-P-A axis. Your "thermostat" is telling your body yoyre making pkenty, because that's what it sees, so it makes less.
    If you adjust it up again, it just lowers your natural output. This leads to a downward spiral of how much of your own T you make vs. how much you supplement.
    Often, as in my own case, there's a rise in PSA levels and the docs want to start doing prostate biopsies before they look at the history of your levels and when they rose and how fast but a good urologist will consider this before insisting on a biopsy.
    Stopping the supplemental T should tesult in PSA returing in-range within 2 years, according to the science...as it did with me, saving an unnecesary biopsy.
    My uroligist works for NASA now.
    Barring some reason like your testes not working or you having an issue with the endocrine system that can't be fixed or illness that requires meds that lower T production, etc, you can raise your natural T by eating right, abd that does NOT mean a low fat diet.
    All sex hormones come from cholesterol, going through several other intermediate hormones first, like DHA and androstenedione.
    Both strenth training and aerobic training are beneficial. A balanced diet is critical to complement the training to work toward a healthy body composition as is lowering sugars and alcohol, and other empty calories.
    Evem manipulating ones own mood, making an efgirt to feel piwerful and confident gives you more T, which in turn makes you more powerful, confident, ambitious, clear-headed and focused, alliwing you to ficus and accoplish whatever your goals are, which in turn makes you more confident, more in control of your environment and gives you more T production.
    Beat your chest like a gorilla, stomp around like a drill seargeant some. Back down a mean dog, complete that project at work, help someone with a problem, do something you've been putting off...accomplish anything, esoecialky if it was sonething you thought you couldnt do.. then tear your shirt off and scream like the Hulk.
    Gorilla stomps are your friend. Cruciferous vegetables eaten raw or lightly steam help too.
    Its like a smile being contagious...the more you tell yourself you can, even if you feel at first you can't the closer you get to where you will... and you'll have all the T you need.
    It all feeds on itself and builds momentum whichever way you have it going , up or down.
    Or continue on the downhill slope with your metabolic snowball until you approach metabolic syndrome, extreme obesity, lack of drive and focus.
    Body fat is a metabolic organ that converts T to estrogens via an enzyme called aromatase. Estrogen makes you fatter, that's why big girls have big boobs, and why bodybuiders that overdo the T get tits....gynomastia
    With the mood component, you really can "fake it 'till you make it". The more success you see the more you'll feel and so on.
    I took the same bloodwork every 3 mos recovering my natural T production as I had when I was using the Androgel because when my GP took me off the supplement I told him I'd been doing some reading about restoring your production naturally and if course ge said no hsrm in tracking progress ...my test results had me back in-range the upper end of natural levels within a year and my PSA normal in 2 years, just as the science shows.
    Mood DOES affect T because stress increases cortisol and cortisol makes belly fat, converting some T you have to estradiols, making more fat. Making an effort to boost your mood lowers stress and ...well if you've read this far, you get it.
    Knowledge is power! You're in the biggest library ever in existence, use it.

    A lot of good information. The “normal” total testosterone in a male ranges from 350ng/dl to 1,150ng/dl. The 26yr old bodybuilder Dallas McCarver who died in 1991 had a total testosterone level of 5,500ng/dl at autopsy, and believe high doses may have attributed to his heart issues. I mention this only because most urologists will try and target a man 45 years and older at around 700ng/dl. My total testosterone was at 30ng/dl before starting therapy. Note that most medical insurance companies consider TRT as a non necessary treatment, and the Andeogel gel packs are over $600/box for a one 30day supply w/o insurance, whereas a 10cc/200/mg bottle of injectable testosterone is only $50/bottle for a 30 day supply. It was recommended by my GP that I start TRT when I turned 50, due to being tired and lack of sex drive. I took it for five years and felt great until I turned 55, and started getting severe tendinitis in both knees. When I turned 60, I was working under my car in the driveway, and when I went to get up I blew the quadriceps tendon in both knees. The first thing my o/s ( orthopedic surgeon) asked as i was a 60 yr old male who presented with bilateral QTR (Quadricep Tendon Ruptures) was I on TRT (Testosterone Replacement Therapy). She said nearly 99% of the Bilateral QTR repairs she does are on men over 45 who have been on long term TRT, and over 80% of unilateral QTR repairs are also performed on men over 45 who are on long term TRT. My o/s explained that long term use of TRT can cause the tendons to weaken. This can occur in other tendons throughout the body, such as elbows, but is more prevalent in the quad tendons pulling away from the patella, as it takes the most stress, and is used throughout the day. I belong to a Facebook Group for patients recovering from QTR surgery. It was started only 3 yrs ago, and already have over 1,000 members. Over 95% of the group are men over 45 who were on TRT. What is truly odd is that Bilateral QTR’s should be statistically rare in cases outside of an accident, and yet 30% of our group are bilateral. Apparently, if the tendons are weakens and pull away from the patella, performing any daily activity like standing up, walking down stairs, or running can cause the quad tendon to just pull away from the patella without any warning. Recovery and rehabilitation from this injury is supposed to take six months,however it appears most members are taking anywhere from 1-2 years before they feel anywhere close to pre injury. However you are dealing with patients who are generally all over 45, and so it would naturally take longer to recover from. Anyway, I would strongly suggest that anyone considering TRT, do their due diligence and ask questions.

      Dallas McCarver had a 550ng/mL which is a whopping 55,000ng/dL. His T/E ratio of 130:1 further backs up how incredibly high his test levels were at the time of death. Even if scaling was linear, this would indicate between 10 and 15 grams of testosterone per week. A ML is 100x smaller than a DL. Most pro bodybuilders have a 5500ng/dl. from 1.5 -2 grams of testosterone a week.

    Hi Docs, Great subject and Dr. Casey was right on. I’m a perfect example. Four years ago I was diagnosed with dangerously low testosterone. My symptoms were not pleasant. Fatigue…no energy…absolutely lethargic…wanted to sleep and took long naps…my libido was missing in action…I was losing muscle mass but putting weight on…and worst of all, I was losing hair. You guys no about that. My curly locks were going down the drain little by little. Full disclosure, I’m 73, 5’10”, 172lbs but I wasn’t getting enough exercise. So my MD gave a choice to me. Gel or injections. I chose the needle. Every two weeks my wife says bend over so she can practice her darts. I do try to make sure she’s happy and for good reasons. Happy wife, happy life and my derrière is thankful for that. Getting the right dosage took a couple/three months to maintain a healthy level. The good news is that the shot is an easy one. Unless you’ve angered your better half. The needle is 1 1/2” 22 gauge and there’s not much feeling in the rear as long as the shot is in the right spot. And the best news is my libido wasn’t dead after all, it was just taking a long nap. 😉🙋‍♂️

However I was low-T and working out did nothing. I'm familiar with TRT so I went the supplement route and I feel a world of difference everyday since. I take Weider Test booster along with L-Arginine and L-Citrulline in the morning. I take Creatine HCL. I also take magnesium glycate which was the missing piece because the calcium from the test booster was not getting carried throughout my body and I had stiffness and magnesium fixed that. I'm shopping for a new doctor but this route changed my life. Literally doing a 180. I cut my portions in half, fasted, and tried to do all the work around the house I can. I saw someone I hadn't seen in years and they were like you really lost weight from the last time I saw you. I don't snack. I reduced how much sugar I eat. One thing which is never mentioned regarding men who although might have normal range testosterone levels, but want peak testosterone levels. I’m 36, healthly, not a bodybuilder but I consider myself one level down from that. I recently got my blood work done, and my PCP insisted I don’t need to take testosterone because my levels were in “normal” range. That being said, I’m certainly not producing the peak levels of testosterone that I was a decade earlier at 26, and I notice the lack/plateau of strength that I used to have at those peak levels when I was younger. Why is it seemingly frowned upon or discouraged to start taking testosterone in order to achieve peak levels again? Especially when done the right way through a doctor with supervision? I’m not asking for super human levels outside of a healthy range, I would just rather do something to go from normal to peak normal. Any thoughts???

    been doing TRT for 9 yrs here in Ontario, I was tested ( before 10 am ) and had Total T levels below the normal range... and all I got from several DR's here was lectures about juiced up athletes and dire warnings of heart attacks and strokes....so i have paid out of pocket from my treaments for my deficiency... significant improvements in health and well being.. including weight loss retention of muscle mass ( continue to WO several times a week) ... for which my regular DR said .."it's just a placebo effect" and continues to refuse to acknowledge improvements ....I am not a DR so this is not medical advice just personal experience.. I also tested relatively high for estradiol...bring that down significant improved what I would call chronic prostate discomfort.. for which my DR was happy to prescribe "antibiotics" for yrs... Have not touch an AB since either.. as far as hemo goes monitored 2 x a year if it gets a little elevated.. simply donate blood and all is fine .. am 66 .. am not 25 anymore but i am the best 66 I can be...healthy and physically active and sleep well .. the only other med is a thyroid pill.. I have no doubt without the T they would have me on all sorts of pills for depression sleep, prostate pain.. and since I would be fat and lazy... how about diabetes or other metabolic disease... not to forget higher risk for heart disease and cancer ......sadly the mythology created by a long discredited and redacted study published in JAMA in November 2013 that said TRT was associated with increase risk of heart attack and stroke still has some negative impact ( although as mentioned has been discredited and deleted from publication) ......In my experience properly monitored and supported it has been a significant improvement to my quality of life . I am not "juiced up" and well within normal ranges.. no DR could ever tell from my blood work that I am on TRT ( except when my Hemo count gets a little on the high side, but as mentioned easily and effectively managed through blood donation.. win/win for all concerned). My perception is the Ministry of Health in Ontario would prefer to see me sick and on a selection of pills...

    - if someone is young, 40 years old or you get, and has low T, it’s imperative a work up for a pituitary related issue is completed. If fact, anyone younger than 40 years old should be considered for an MRI to rule out a pituitary issue. I was surprised the urologist did not mention low T in young persons is much different than low T in older people.

    I have been on TRT in my 50s for several years now and it makes a huge difference. I pick up the symptoms when I miss a dose for a few weeks. I was surprised that I had low T because I have a large muscle mass from strength training naturally (T of 4n mols) But the symptoms for me were lethargy, depression, apathy , and low libido (nil). It is like night and day when on the medication. The problem with waiting too long to do the injection is the apathy because you can’t be bothered anymore. I had to experiment to get the dosing and frequency right. Too much and libido is too high as well as haematocrit. The gels didn’t work because my skin would flare up from all the alcohol in the product. Hypogonadism is a real disease like menopause but is not treated the same. The stigma attaches to TRT for men as steroid drug addicts. It is a real medical condition that even a lot of doctors including senior endocrinologists do not believe. An endocrinologist told me that I don’t need TRT because my body doesn’t need T, that he had another patient with no T who was just fine, that we need to save Medicare in Australia from paying for all the HRT. This was the head of a hospital endocrinology department. I’m glad I got a second opinion.

      Was thinking of getting on it but would have to pay out of pocket (I'm in the U.S). The medication isn't expensive but paying a doctor to watch my back is so it's prevented me from getting on. But I'm 43 and in the past 5 or 6 years I've noticed a huge drop in all the things listed. And I am muscular, work out often, been a ski coach for 20 years and a ww rafting guide as well. I mention these things only to put my body type and lifestyle into context (not fat, lazy, heavy drinkers, etc.) The other hiccup is that I am absolutely terrified of needles, especially blood draws. I have a horrible reaction when I black out, sweat profusely and vomit, followed by hours of malaise and a general feeling of being unwell. I've considered going to a hypnotist to see if I can overcome it. I genuinely want to get on TRT and hearing your experience only reinforces this. Have you noticed any health side effects, mood changes, acne, loss of hair, etc?

    • @FartyHarley
      @FartyHarley 11 หลายเดือนก่อน +2

      @@weirdbeard1980I haven’t had any negative side effects from TRT. If you stay within physiological doses you are simply replacing what you once had. Hairloss comes from high DHT levels which occur in higher testosterone levels. I wouldn’t consider taking testosterone unless your endogenous levels were low. That is the pathology test you should take. If you boost testosterone with exogenous testosterone on already normal levels then you won’t see any difference in mood or energy (you will however have the sex drive of an 18yo). I experimented with dosing and timing because I have a science degree in physiology and I wanted to treat my symptoms. I wanted to see exactly how the dose affected my symptoms and confirmed that my symptoms were not just psychological but consistently returned when I passed the required injection time. With regards to needles, I don’t like needles but the small gauge needles are barely perceptible. Also, it is an IM injection and not into a vein so negligible bleeding. Glute medius is my preferred site.

However, in my mid 50's the wheels fell off! I diagnosed myself with low T and went to my GP who agreed to conduct tests. Sure enough my DHEA and Testosterone were off the bottom of the chart. I started taking ANDROFORTE 5 Testosterone cream and brought back my "natural" mood. My PSA started to rocket up at one point and I went and had a biopsy - no sign of cancer. I refused to go off testosterone replacement and started taking a natural herb remedy - Prostate Support [Saw Palmeto and Stinging nettle root] and my PSA is now well down below normal.

Went in to the specialist and one test was 243 and the third was 213. Globulin is at 13 and LH is 3. I have moderately high blood pressure which is treated with atenanol/chlorthaladone 25 mg/day. Hemocrit is at 47.3. Prolactin is normal, hemoglobin is 15.7. I don't drink or smoke. Have to watch what i eat so i don't get another flare up of gout in my right big toe. Consequently I'm drinking a lot of water with lemon. I would love to loose at least 50 lbs. I have no use for erections. My brother died of prostate cancer after battling it for 15 years. Not sure if i want to monkey with TRT.

    Hey doctor, amazing video. I have a quick question. Does low testosterone cause constipation?

  • @jaredstrauss617

    Have you seen too much exercise cause low testosterone? I do 7-10 hours of cardio per week and recently tested at 166 ng/dl with low RBC, HGB and HCT. My doctor wants to retest in a month.

      Well testosterone is converted to DHT which is implicated in male pattern baldness. May just have been his time though

      When a man wants testosterone there are risks. When a woman wants testosterone to masculinize there are no risks at all. It's a lie doctors have to tell to keep the state medical boards and employers from going after their licenses and jobs. Facts don't matter in this arena sadly.

    I started TRT injections in January, I was at 236. My T is now in the 550 range, but the injections have given me nothing but side effects and stress from keeping your E2 in check without feeling like death. I also have started severe itching anytime I got how or did physical activities. So I am planning on stopping. What would you recommend the best way of weening off? I currently am taking 120mg injections every 4 days.

      Sorry to hear that. The weaning schedule definitely should be discussed with your doctor. Best of luck

    Would testicle trauma be a factor in dropping T levels? I have “grown”two large cysts I guess after having one of my testicles ruptured and now they are very sensitive. Also have noticed a few small, hard knots on the epidenti……..🤷🏻‍♂️the tubes and stuff the sperm travels through.

    Thanks guys the video's are always so good. Quick question, what about the OTC for low testosterone? I see a lot of pill form, and as Dr. Casey pointed out, pill form are useless and go right through the body. I looked and found only a couple topical types none are approved or even tested by health Canada but would they be trash snake oil too or would they have some benefit?

    • @DrAJ_LatinAmerica
      @DrAJ_LatinAmerica ปีที่แล้ว

      99% in US and Canada sold OTC, are junk unless! OTC in Mexico, Colombia, Panama, Thailand,.....where they are allowed to sell the good stuff without a prescription is very different. True some guys only need SARMS (be very careful with the source and quality of the product), peptides or natural herbs like tongkat ali or fadogia agrestis mixed with some L arginine and L citrulline, maybe some nacin, zinc and boron. However, with any issue, it will depend on the root cause of the issue (overweight, stress, poor nutrition, diabetic, hormone imbalance, or more serious health issue). With any health related, it all starts with Data: Complete blood work and being honest about your lifestyle habits.

    I am 66. I recently went thru rectal and anal cancer , emergency surgery, chemo and radiation. I am cancer free now, but I am TIRED all the time. It's been 2 years since the final radiation. A friend of mine (49) started using testoserone from UG freak. He said he feels better than he has felt since he was 25. I am in good shape (construction all my life, and very active) but now, I have to fight to get out of bed...i golf a lot, still doing construction, but my health insurance is no longer in effect. So, my question...I can buy this stuff on line...get the needles and wing it!...with my friends help...I am a cautious person, but, I want to feel like working again, I want to have some strength AND the energy and desire to get up and go instead of having to force everything! Any advise is appreciated.

    My total testosterone came back at 887 and the normal range is 250 to 827 ng/dl. Should I be concerned ? Also my creatine kinase total came back at 252 meanwhile normal range is 44 to 196 u/l. I don’t know what’s going on! But I am gaining weight, loosing muscle and super bloated all the damn times. I wake up bloated, feeling full and I get more bloated even by drinking water.

    Please don't forget, Total T is not the most important number, it is only one of many factors. Estrogen levels in men also play a major role in libido. TRT dosages / Testosterone will not help if the person has low androgen receptor count. "Low" T is not a big deal, "IF" the patient has correct Free T levels, estrogen, prolactin and progesterone levels. It is a balancing act. Just adding TRT / Test, short acting or long acting will not solve these issues. And "IF" anyone starts TRT, please ensure your doctor understands the effects on your liver (if orals), skin if a gel and lungs, blood, and HDL-c if injectables. Yes, lifestyle intervention FIRST. No smoking, no drinking, get 8 hours of quality sleep, nutrition is a critical, correct body weight (obese / over fat will cause estrogen to go up), morning sunlight, and reduced stress. Lastly, most doctors will not tell you, but ensure you have a hot wife or hot girlfriend, the ugly or fat ones will kill your libido. Seen many guys with "erectile dysfunction" then we bring in a hot young hard body surrogate and like magic, they get erect.

      @DrAJ_LatinAmerica so what you're saying, looking at my total T and free T levels is not enough. There's also other factors to look at. Okay. That means 90% doctors won't get it right, so I need to get to a doctor who really understands the big picture. So the trick is finding such a dr.

    • @DrAJ_LatinAmerica
      @DrAJ_LatinAmerica 9 หลายเดือนก่อน

      @@contrarian717 correct. Not enough. Start with, does the patient need it or want it? No judgement but your doctor needs to find out. Next try all interventions without meds. For many losing weight, better / more sleep and diet gets them closer to their goal. Complete cancer screening is important!! The test wouldn't cause it, but can help it grow if already present. Your doctor needs to check CBC, CMP, complete thyroid panel, Total T, Free T, DHEA, SHBG, and all the major hormones like progesterone, prolactin, estradiol,...they must look at LH and FSH, PSA, Hba1c, Hscrp, fasting insulin,..... All these items are a starting point. Must ensure the person understands, once the process starts, monthly visits and monthly blood work for the first year, during the adjustment period (everyone's body is going to react differently). No guessing, verify. Extremely important the patient likes needles and injecting, (helps if they have tattoos, generally they are more relaxed around needles). Helps if your doctor promotes frequent micro dosing vs bolus dose. They should also explain that not everyone needs to start with testosterone. Some guys may only need lifestyle changes and some HCG. Must develop a very complete patient profile/ big picture. Close communication. Be ready to spend lots of money for best results (meds, visits, blood work) and plan on a possible lifelong process. Many guys 50+, once they start, natural function, natural test production never returns. Have fun and best of luck. Ps - never never let the doctor talk you into BS creams, gels, pellets,.....very problematic and if you have any allergic reaction to an injection, keep in mind it could be the Brand / maker of the product, try a different brand that uses a different carrier oil. The reactions are to the oil, not the Test.
