I put my 4 month old in a bunting bag yesterday, I buckled her up in her car seat and it was that loose that the strap came undone from the back of the car seat and out the hole. We were just going for a walk and I put the car seat in the stroller. But just a warning the straps can also come undone from the back! Edit: I also had the straps on the car seat as tight as they could go and they still came loose!
It doesn't matter how cold it is. Keeping a thick coat on in the car seat is never safe. You can bundle them up in blankets to carry them to the car and ideally you would warm up the car before getting into it.
To make sure the clothing your child is wearing is not too bulky to be worn in the vehicle. During a crash, bulky clothing compresses significantly, and a child can be partially ejected from the harness due to this.

The child should be tightly harnessed anytime they are in the car seat, even if just going for a stroll.
