An advice to anyone doing this mission. DO NOT use the keys until level 22. These are leveled chest. Anything you open in Death's Tombs will be the same level as Death.

You might have gotten it before and just thought you didn't, that happens to me ALL THE TIME. But if you've checked them all, resetting the dungeon might work.

Trackers for collectibles are a Godsend for perfectionists like me. Like holy hell, just seeing that number go up is soooooo good. Especially when you finish it and it goes from a bright white to a dark grey, so you know you don't have to worry about it.

The 2nd stone dude isn't there in the Scar for me 😬 Do I need to leave and reset the dungeon maybe?

It's a story event. If you don't have it now, just progress the story
At the end, the chest in the middle wasn't there for me. Why is that?
You need to break all the jars to get it.
