I work as a pharmacy intern at Walgreens and I swear the amount of prior authorizations we get for ADHD medications is ridiculous especially when patients have taking it before
My mother has just about strangled Walgreen's pharmacists over her prescriptions.
@@ElvenRaptor I’m not surprised but it’s not easy when a lot of medications are on back order or companies put a hold on production of certain meds, even now some ADHD meds are on backorder still and it’s insane

Same, I'm Mark's age, dxed at 14 and I only recently started pursuing animation and stuff I want to do bc as a teen I was convinced ADHD people couldn't do things that require any actual concerted effort. I essentially have procrastinated making internet content for almost TWENTY years because I thought my ADHD wouldn't allow it... come to find out that like 1/3 of my favorite creators including Mark have ADHD.

I feel Mark's pain. My mother had to wait three years before her insurance would cover new COPD medicine. The insurance company literally told her doctor, "No, she's fine on the other stuff."

The process really needs to change so people can be helped faster.
I literally had to try Vyvanse instead because of the Adderall shortage. My only saving grace is I not only choose to skip one day a week for tolerance reasons, but I carelessly miss doses so often that I have accumulated an extra month of meds as a result of the ADHD tendancy to be inconsistent.

@Lil_Yuri RELATABLE. I was that way in high school. Now I usually have like 5 extra by the end of the month, but my lateness in requesting refills each time ends to cancel that out LOL
@@antoniocasias5545 the fact that Mark made himself consistently call every few days, do all the steps he needed to (like get ahold of his doctor for the note) basically asap, can be hard for people with ADHD. Sometimes it's hard to find motivation to do things, even if you want to do them, but ESPECIALLY if it's something annoying, frustrating, or stressful

This happens to me every month, it feels like, with my insulin. I have to tell them that I'm type 1 diabetic and what do you mean I can't have my insulin right now? Do you know how insulin works??! I feel this story on a personal level.
Oh God, and that's worse because you likely won't die from not getting Adderall (unless it causes you to get in a car wreck or something where being distracted is no good), I'm no doctor but I'm pretty sure you can't exactly just drink extra coffee and get extra naps or breaks or something in place of not having insulin.

Dysautonomia patient here. I feel you, because every time I need to buy my Astonin I need the authorization from my doctor, and unless it's EXTREMELY DETAILED, they refuse to accept the authorization, and it takes like two months for the pills to arrive, and getting a visit to my doctor can take up to another month. So yeah, getting the medication is almost an adventure.
Well it's a good thing that's not an important prescription that your life depends on or anything.

It isn't just the insurance, who I can understand a bit why they're slow on the uptick (although they have been known to actively defy doctors when life-saving medicine is needed), but also the pharmacist, so actually idiotic to not express this to Mark sooner.

@@Lil_Yuri its also a case of its the shiny new drug and there are alot of bad doctors that treat people like meal tickets. here order this brand new drug you can get it from our phramacy. alot of people are taking pills that they dont actually need or could be replace with cheaper medication.

Whenever I'm off my meds (usually because the pharmacy is being stupid with my Focalin again), the first sign of withdrawal for me (because Focalin is a good ole amphetamine) is I get cold. Really really cold. Pair that with my ADHD primal urge to fidget, and you've got a miserable, shivering wreck of a person desperately pleading with CVS

I kind of feel for Mark because I’ve been around that spectrum of the autism ADHD Asperger’s community for pretty much my entire life so I know how it feels being a high functioning person with autism myself I know what it means, I know what it represents and I know there are places out there that can help people with disabilities, but it’s still hard because sometimes certain things can occur to cause my disability to rear its head and the biggest thing that sets me off or more specifically sets my disability off is change, I cannot handle change very well and it’s gotten worse as I’ve gotten older because in a lot of respect I am very much like a pet, whether it be a dog a cat whatever because lets say for example when you’re late coming home for work and you forgot to feed your pet beforehand you’ll find it sitting at the door yelling at you to feed them. I’m kind of that way I thrive off of having a structured environment that has very little deviation in routine and if any major thing changes I will pitch a fit. Now granted this isn’t as prevalent when there is minor change but even so if minor change does occur you will notice a change in my demeanor immediately even though it’s not as subtle. another thing about me is when I’m tired or low on energy I don’t usually respond in a very concise way as I tend to say things in a lot of short sharp sentences because I don’t have the energy to make concise statements

Also pretty sure I have undiagnosed ADHD and this scenario is one of the reasons why I haven't gotten diagnosed or anything for it lol. I’ve been a Type 1 Diabetic for 24 years and I’m currently a medical student, and let me tell ya that Mark’s assessment of prior auth is 100% on the nose.
It’s just one more hoop insurance makes you jump through to hopefully make you give up before filling a script

I think what Mark said at the beginning of the video is important, I haven’t been diagnosed ADHD but I do feel like I have it. I have a hard time focusing on stuff especially recently.

"'Insurance prefers [Medication B] or [Medication C].' Oh I'm sorry, It's not like I went to school for a million years to know what to prescribe"
the reason why prior authorizations require a doctor to do it is because it isn't just "doctor says yes, so we cover."
It's actually "doctor has to give justification/argue with insurance provider because the insurance provider doesnt want to pay."

God... when Mark said Pre-Authorization or Pre-Auth's as I call them I nearly spit out my drink because I knew that it was coming based on how he was telling the story. I've occasonally needed medication for medical purposes and I have never needed a Pre-Auth before hand and when I hear that others have needed a Pre-Auth for medication or anything that they actually need I just loose my mind because it seems like such a stupid factor as part of an Insurance benifit... I work for a Chiropractor as their X-Ray Technician/ Radiology Operator and Biller which means I deal with a lot of the Insurance aspect of healthcare from the providers side and holy shit do Pre-Auth's suck shit.
I've seen a handful of benefits fall on my desk (from the Insurance Verifications I do) have Pre-Auth Required after a set ammount of visits which is usually like 4-6 or a Medical Necessity Review (which is also a Pre-Auth but a softer variant somewhat) before the first visit and/or after a certain ammout visits. Majority of plans I have seen that have Pre-Auth's riddled in the benefits is any coverage that is with ASHlink (ASHlink is a cost-containment company for insurances) which includes Cigna. A good tip is if you have unlimited ammount of visits, there is likley to be a Pre-Auth or MNR before you can use those benefits.
Don't get me wrong, Pre-Auth's and MNR's can be helpful but I see them to be more problematic than anything else. It's just a cost-containment tool so they don't have to remburse your practitioner as much. The pro's are the visits are mitigated and are thus more potentially worth it because the insurance is making sure it's medically necessary. The con's is the insurance has control over your benefits, they can deny you and your doctor coverage when they want because THEY don't think its medically necessary even if you and your doctor think otherwise, should they decide to deny coverage because it didn't seem medically necessary the costs will most likley be coming out of your pockets on the doctors fee schedule which could be more or less the insurance and you don't have protection provided by the insurance if the doctor is doing things not medically necessary.
My personal advice to people who are looking for a new insurance, just got one, or something else; Do your research on the medical insurance or any insurance in general you have or planning to get The more you know, the less suprises you'll have. There are a few insurances that I can say totally go for becuse the coverage is pretty decent with few strings attatched that I can see and there are some I would say avoid but it really depends on what's affordable. Another advice I have is get two insurances if possible, one does fine but if they decide they don't want to pay then you are screwed but having two is good but just a little more expensive. Having two means you pay less out of pocket when seeing a medical professional, and if one refuses to pay the other will pick up what the other does not and pay on it and either use their primary benefits or use the others primary benefits and have you pay on either or. Also if you have a primary with a deductible and a secondary with out your primary deductible will be paid by the secondary as you go for visits in care then in some cases you'll just pay your patient responsibility on the secondaries primary benefits. Or it might be the primarys benefits depending on whats the primary and secondary.

funny story as a kid we had a meeting about what we should do about my adhd. and everyone was talking about how they didn't want to up the dosage due to risks so my 11 year old brain chimed in with "why don't we lower the dosage since my brain would be use to a higher dosage?" the funniest part is that it worked again.
now this could either be for 2 reasons
a. that actually raised the dosage anyway
or
b. placebo effect took over and the lower dosage actually worked
either way its a core memory of mine that i love sharing

I've had a very similar problem happen to me recently, which is also why I have very little faith in modern insurance. I recently got put on one of the major healthcare insurance platforms to help pay for expenses because they were racking up - the kind that a guy like me who's been unemployed for a while and can't otherwise afford paying out of pocket, needs. So i get that done, get me signed up with a doctor, get some tests done, and I'm good. Within a month I get a doctor's bill - I'm told that I have to pay for the lab tests and the previous visit. I'm sitting here thinking "insurance should have covered that, right?" And they said they'd check in on it. I get no response back, another bill and a warning that a collector's service was incoming. At this point I'm outraged - the whole point of the inusrance coverage that I got was that it was supposed to help cover costs I couldn't afford, and I felt like I was being given a blind eye.
Turns out the insurance provider wouldn't greenlit me and my file, because their records showed I had another insurance on file. Said insurance? A child's insurance card attached to my dad's company insurance from when I was 12 years old. They still had records from insurance that I didn't have anymore from almost two decades ago and nobody bothered to tell me about it. Nobody contacted me, nobody on their end let me know I had a problem, they just completely ignored and gaslit me. In the end I was able to get it fixed but by then I got a call from the collector's agency and I just wound up paying the bill out of pocket anyway, because I was getting fed up with it.
Moral of the story is - don't ever trust modern bureaucracy, especailly when it's advertised as free.
I'm having similar problems with one of my medications right now.

9:10 as a person with A.D.H.D this would be the most infuriating thing to hear. Like I haven't even had an encounter with a pharmacy remotely close to this and yet hearing it made my left eyebrow twitch in anger.

I had where I got the prescription in, and the pharmacist refused to let me pick it up because it was apparently against his religious beliefs. It was birth control, and I’m not even on it for the main reason, I’m on it because I’ve got PCOS and had problems with ovarian cysts and extremely painful and irregular periods for almost 7 years until I was 18 and my mom finally let me see a doctor about it. I’d had multiple cysts rupture and it hurt so bad that I couldn’t really do anything for a while, plus I pretty much always felt horrible because I was anemic from having multiple heavy periods in a month. I also played sports and this was when there was talk of hormone checks for athletes and women with PCOS were getting flagged for high testosterone levels, and mine had always been really high, so I was worried about getting banned from sports too. So anyway, the prescription was sent to a local pharmacy that my family had been using for years, and when I went to pick it up, the pharmacist first asked me if I had a father, boyfriend, or husband and if he was okay with it, which first of all isn’t okay at all. But it’s a small town in Texas, that sort of thing is kind of normal unfortunately. So I told him yeah, my dad’s fine with it, but he told me it went against his religious beliefs for a woman to take birth control pills and he wouldn’t allow me to pick it up unless my dad was there to basically give permission. So I had to go home and ask my dad to go with me, and he’s not a misogynist but he is a jerk, so he said he’d go with me in the morning because he was watching one of his tv shows. So we went the next day and got the same pharmacist, who asked my dad about it and then proceeded to tell him he was raising a harlot. In front of about 10 other people who were waiting in line. Needless to say, I talked to my doctor and asked for future prescriptions to be sent to a different pharmacy.

I know this exact situation, because I go through this practically every time I need to REFILL a prescription
As someone with ADHD and have been taking medication for years (I dunno why it took me til so much later til I got diagnosed, but it took me so long, despite the fact that I get it from both my parents), I relate to this so fucking heavily. I’m just so lucky that I have two pharmacies near me, and my doctor is not only near me, but he’s also a really solid doctor who’s receptive and kind to me. And so is my therapist, since it was her that got me to finally get the script, and not just think “I’m just not a good worker, I’m just depressed and tired”, while actively having massive impulse problems and being unable to focus unless it’s hours at a time. I’m super lucky, and whenever I hear stories like this about ADHD meds, or stories about not being able to get Ozempic when you really need it (like one of my family members, who needs it for health management), it’s just… it’s frustrating, since the reasons why are people exploiting the medication, richer people buying it for their own fun use instead of therapeutic reasons, and people selling it instead of using it. It’s just frustrating, since these drugs are necessary, but people use them as quick fixes/ recreational reasons, when there are people who really need it, and it makes pharmacies and even doctors feel suspicious about giving the scripts, since “how do I know you really need it”.
As someone with ADHD this hits home. I've had my pharmacy (that I have almost always used so they should auto order my meds there) not order my medication in for 2 weeks in a row before. It's hell.

Totally off topic but it’s refreshing to see someone who was also diagnosed later in life. For me I straight up just didn’t ever think I had it because my brother has adhd too and I always compared myself with him not realizing that adhd presents differently in everyone. He was more hyperactive while I’m more inattentive. Anyway my sophomore year of college I was doing homework while on the phone with a friend and I in passing mentioned how rare it was for me to be doing homework cause I just never got motivated to do it no matter how much I WANTED to do homework and fix my grades. She mentioned adhd in passing and I’m like nahhhh then she had me take like those silly online tests… and yea it said I had it bad lol. Talked with my therapist (already diagnosed with depression at that point so was a frequent flyer in therapy) and she tested me and was like YUP you got adhd and now I’m medicated and it’s great :).
If you can find a small/family pharmacy instead of the chain ones, they're a million times better about this. I was running out of Vyvanse and they only had 7 pills of my prescription, so they went "tell you what take these now and we'll order in and give you the other 23 later." I had to pay in advance of course but that's still vastly preferable, even missing a day can really throw off my rhythm/habits.

I can only imagine if i someday got a prescription for those kinds of medications and they drive me to look them in the eyes and go. "Would you care to find OUT why i need these medications?" "Cause if you keep driving me up the wall, inevitably i'm going to crash back down like an asteroid smashing right into this dang Pharmacy" xD
Seriously though, I HATE our country when it comes to medical care and how taxes are handled...among other reasons. But seriously. It is the most mind bogglingly backwards thing imaginable.
Ah yes. Let's allow even a basic checkup to cost 100s if not thousands of dollars for no real reason. Even in cases of emergency and on top of that, Let's make affording healthcare even harder by making it taxing on your SANITY to boot.
But yeah. USA healthcare might be the most aggravating in the world. I don't know.

With my insurance I get a new wheelchair. With a company I’ve been with literally since my first chair at 3 or 4 years old (I’m 27 now) still need a prior authorization. From a doctor I’ve only been seeing maybe 3 years.
Getting ADHD medication, for some reason, has always been a cursed task for me and my parents for the past two decades.
Sometimes the phramacy would forget, sometimes the doctor forgot, or sometimes the insurance would just *forget that they cover my medication and make us pay more only to realize that oh yeah they DO cover it.*
I’ve had my doctor admit that the note to remind him to fill my prescription would be buried under all his other papers. Or he would randomly be out of office when I call, so I would have to wait .
I’ve had my doctor accidentally send my prescription to my hometown pharmacy instead of the pharmacy in my college town. So I had to go two days without my medication because it was a “controlled substance” and couldn’t just be transferred between pharmacies. NOPE, I had to call my doctor again, wait again, and PRAY it actually gets to me before the weekend.
Needless to say, I’ve developed a bit of a trust issue with this medication.

I find it funny that I watched this two part series on Markiplier's painful experience with CVS for the first time a few weeks after I was doing a peer evaluation on a paper that talked about CVS's poor corporate culture, so I already knew something bad was going to happen, that's just pure coincidence and rather humorous for me

6:47 it seems to be really difficult to actually make markiplier angry, but you can just feel the anger eradiating from the screen as he says that line about having to get a prescription for his prescription
The “I figured I had adhd, everyone said I did, but I never went through with a diagnosis and I went and topped out the meter” is exactly what my autism diagnosis was, a lot of people, friends, family kept saying it and finally I went to a diagnosis and they were like “yea you are very autistic but you really developed these coping mechanisms that helped you cope until now”
Also, his fight with insurance is so common for anyone with a rare disease. My disease CAN be treated with a cheap drug, $15, but it has a 10% efficacy rate for my disease. This drug caused me to vomit for 16 hours. It’s a drug you take every 12 hours. I can’t take it. The biologic alternative first was enbrel for 5k a month. I ran that out because it stops working after 7-10 years (I luckily got 10 years somehow). Now my drugs are 36k a month (ilaris) and my company asks every year “have you tried that cheap pill? Seems good?” And a prior authorization fight that lasts weeks. The drugs shouldn’t be this expensive and this fight shouldn’t exist.
My wife has cluster migraines, a new drug came out and they just wouldn’t cover it because it was new despite the promising results of the studies. Because not enough people have taken it.. like how are we going to get anywhere on it then?!

As someone going through a similar fiasco, it's nice to know I'm not the only one who sees the insanity for what it is.
This is why I always talk to them in person. Force them to do their job with awkward unbroken eye contact, and enough annoyance that they finally do anything except the absolute bare minimum they're being paid to perform. People are dumb and need to be pumped full of anxiety and stress in order to do what they need to do sometimes. Scare them, they deserve it.
