  • In this video Doctor O'Donovan explains key facts you need to know about a medication called METFORMIN used to treat type 2 diabetes (also known as Glucophage, Fortamet, Axpinet, Diagemet, Glucient, Metabet).
    In the video we cover:
    What is metformin? (0.36)
Who can and can't take it? (1.34)
    How to take it (2.36)
    Side effects (4.49)
    Use in pregnancy (7.40)
    Interactions (8.15)
    Key facts
    - Not everyone gets diabetes symptoms, so taking metformin may not make you feel any different. That does not mean it is not working, so keep on taking it.
    - It's best to take metformin with a meal to reduce the side effects.
The most common side effects are feeling and being sick, diarrhoea, stomach ache and loss of appetite. Vitamin B12 deficiency is also a common side effect of taking metformin in higher doses or for long periods.
    - Metformin does not cause weight gain, unlike some other diabetes medicines.
    - If you have diabetes, you can get free prescriptions for all of your medicines, not just your diabetes ones. You'll need to have a medical exemption certificate.

My dr. wanted me to take the Metf. I didn't want to - instead through nutrition and exercise I lowered my A1C on my own. In 4 months, I'm now at 5.8 and am working to reduce to 5.0%. WTH are dr.s so quick to get their patients on drugs instead of looking at the root cause and reversing diabetes. As a patient this is so frustrating. It's like all I am is a # now and all the Dr. is is a pharmacy. Never once was I told that I can reverse the disease. I had to educate myself and figure things out on my own. WTH kind of medical system do we have now? Very frustrating!

Can you elaborate a little more, for example, and mainly:
What did you do? What was your daily routine like? Did you check your sugar first thing in the morning? Did you eat three times a day and what did you eat? What kind of activities did you do?
Next, what were your sugar readings before and what were your sugar readings after how many times did you check your sugar and what was your A1c before and what was your A1c after?
Honestly, honestly if you answer that you might be able to help a lot of people reading this, including myself. I need it. Maybe you can help me improve.

@@w0251605 I got labs done to get an update on blood work so didn't check them myself. I eat 2 meals per day and doing Intermittent fasting. I eat mainly plant based w some protein. Approx 9 cups of water daily. Stripped out sugar, processed food and all that crap; eating super clean. Little to no alcohol. Have a non sugar treat now and again. Activities: currently walking approx. 7 - 8 km every other day. A1C: Aug. '23 - 6.6; Dec. '23 - 6.1; Jan 10th ' 24 - 5.8. Being very consistent and super strict has landed where I am today. I've struggled w carb cravings all my life. When I lowered my carbs (less than 75g) the cravings went away in less than 5 days. I watched Dr. Jason Fung's videos or education/research.

I know its reversable and I'm getting there, well done on your type 2 control, your doing well.

I’m 69, and just got diagnosed type 2 this week. Obviously I’ve been diabetic a while as my A1c is 10.1. Endocrinologist put me on Metformin and Glipizide twice a day. I take Metformin with meals. So far no side effects. I was scared to take it but the meds have taken my blood sugar way down. I’ve heard bad things about Metformin. I don’t want insulin so I’m sticking to the meds.

My doctor prescribed metformin 500 ml and I’m pre diabetic.I do not use it everyday since fasting helps me to lower my sugar.i exercise after meal and I changed my diet to pre plant based.Food portion is very important I lost weight and cut on carbs and sugar. Although I crave for sweets I choose the diabetes friendly choice.

I've been using metformin 500mg for 2 weeks, the doctor suggested diet and exercise, but I said I wanted to use this drug because my doctor suggested this to me and now I'm wondering if I'm using metformin for 1 year, will it harm my kidneys?

For type 2 diabetes which can be controlled with lifestyle there’s no need for metformin. Keto diet plus fasting plus exercise is enough. I brought my insulin levels under control with this. The doctor was going to put me on medication for high triglycerides without giving any of these suggestions

I have been taking 2x1000mgs of Metformin since I was 35 years old for type 2 diabetes...I never have diarrhea . However, in the last 3 or 4 years I have became often constipated. I suppose It could be from long term Metformin or, more likely, just from getting old.......I take a stool softener to fix that problem and continue taking the Metformin, in part because I may be into the longevity side effect of Metformin, as I am now 80. At any rate if the Metformin is going to kill me early....it's already too late..it helps me maintain a good A1C and to eat some things I otherwise might not have. I have regular kidney checks and no problems, B-12 level is normal...I know I took the easy way......but for me, it has worked just fine.....Also, I drink a no sugar (used to be called diet) soda whenever I want one and I have since "Tab" came out many years ago. I remember the reports about the artificial sweetener in Tab causing cancer. I also remember reading the follow-up a year of so later saying the mice they used to test the sweetener were given the human equivalent of 600 bottles of Tab daily. I was careful not to do that then, and now with the Sam's Cola, Coke Zero or whatever as I continue to drink no more than a couple per day.......

Very informative. Is it safe for a normal/prediabetic person? Is a doctor's prescription necessary?

    • @DoctorODonovan
      @DoctorODonovan  ปีที่แล้ว +3

Thanks - glad you found it helpful. This is what current NICE guidelines say; NICE suggest that a clinician should use clinical judgement on whether (and when) to offer metformin to support lifestyle change for people whose HbA1c or fasting plasma glucose blood test results have deteriorated if:
this has happened despite their participation in intensive lifestyle-change programmes or
they are unable to participate in an intensive lifestyle-change programme, particularly if they have a BMI greater than 35
Check renal function before starting treatment, and then twice yearly (more often if they are older or if deterioration is suspected).
      Dose:
start with a low dose (for example, 500 mg once daily) and then increase gradually as tolerated, to 1500-2000 mg daily. If the person is intolerant of standard metformin consider using modified-release metformin
      Duration:
prescribe metformin for 6-12 months initially. Monitor the person's fasting plasma glucose or HbA1c levels at 3-month intervals and stop the drug if no effect is seen (1).

If you cut carbs and sugar too much, your body will begin converting your stored fat into acetate to create the glucose for energy and you risk metabolic acidosis or ketosis. I'm not a doctor. But I think this is what can happen with too aggressive a use of Merformin. In fact, as with other chemicals, when you ingest them chronically, you can permanently damage your organs and the truth is, no doctor can ever tell you whether the drug they prescribes you is causing the organ damage; or whether it is your supposed original illness. That is because a) they cannot tell you the truth about a drug causing your illness; and b) they don't want to risk being sued for malpractice and c) they simply don't know.

You should probably start with one tablespoon and see what works for you, my wife mixed up like 4 doses (8 table spoons of olive oil and 8 tablespoons of vinegar) in a bottle and I drank it all thinking it was one dose and vomited from probably all the vinegar, but still.

    Eat less often
    Main takeaways:
    1- Eat less often.
2- Avoid sugary drinks and foods.
3- Reduce meat intake for longevity.
4- Its never too late for dietary changes.
5- Find veggies that have been stressed out (organic, colorful veggies)
6-Never start a meal with sugars, if you do eat them at last (desert).
    EAT LESS OFTEN
-There is a genetic pathway that gets triggered by low energy. These genes (sirtuins) respond to low energy (low glucose levels-caloric restriction), high heat, low amino acids, high salt (stressors). -----They take care of the DNA repair and the stabilization of the epigenome.
-When the body is hungry, the body makes NAD (fuel for the sirtuins).
-Two ways to activate sirtuins synthetically:
    1) Use some molecules
    2) NAD booster.
    Longevity genes:
Mtor: protein that registers amino acids. Allows the cell to build things. Not a recipe for longevity. Activates autophagy.
Ampk: goes up when hungry. Makes more mitochondria (important for making energy)
    Sirtuins
    Fasting:
    - Metformin: mimics fasting.
    - Time Restricted Feeding: At least 16 hours.
    - Liver compensates for the lack of food. Glucose neo-genesis. It overcomes the feeling of hunger. The liver starts making glucose at a steady level.
Sugar: big killer. It spikes glucose. Glucose shut down sirtuins and ampk. Defenses against diseases and aging shut down.
Protein: we should avoid high protein red meat diet for longevity. It inhibits mtor.
Amino acids: three important. Leucine, isoleucine, and valine: BCAAs. Are used to sense protein intake. The sensor is mtor. The mtor activates when you get them. Mtor repairs and build muscle. In the short run, this is good for building muscle etc. But in the long run, mtor is the responsible for autophagy, which is key to longevity.
Plant based diets: the more vegetarian you are, the longer you live. The best, Okinawa diet, is plant base diet with fish - (probably because of Essential fatty acids)
Humans grew COLD and HUNGRY. Now we live at warm houses with food available all the time.
