    THIS IS AN OLDER VERSION OF THIS SKILL. View how this skill should look when performing for the Florida CNA State Exam. View to proper way to empty a urinary drainage bag, measure and record the contents and properly clean and store your supplies. This skill has close-ups of important tasks, including measuring the urine, for better comprehension. This skill requires comprehension of infection control procedures to reduce the possibility of bacterial infection resulting in a urinary tract infection for the patient.     I passed my skills test!! And I thank you for it!! There was a certain skill that kept bugging me out so the night before I kept watching that skill over and over, your video of course.....I go in the next morning, and she asked me to perform THAT TEST lol. I was dying inside lol. "Making an occupied bed". Which no it's not hard at all but when I know I'm being watched for an exam and there are certain ways to complete this task I get nervous. But I did great! I take my state test the 16th of August. I'll be back and let y'all know!

    Passed my test last Thursday. I am in Florida, this was one of my skills along with wheel chair transfer and nail care. Thank you for these videos. I didn’t go to school.

    For those of you questioning the floor, there is nothing on the checklist that states that you cannot touch the floor with your scrubs! In the real world there are many times you will have to get the the floor in order to do the things that you need to do. What matters is that she hit every checkpoint for the state exam! This was absolutely done correctly. And your gloves are allowed to become contaminated. That's the purpose of wearing them! Faucets are ALWAYS dirty and so are our hand until we wash them! So that is why us instructor also teach to turn the faucets off using a paper towel! I tell all of my students to watch these video to reinforce what I teach them in class and never have my students failed due to a technique taught in any of these videos. In fact, 4yourcna is the ONLY videos that I will approve them to watch. Great job Patty!

      There is nothing on the list that says no touching the floor with scrubs on. At this point, your scrubs are contaminated just from entering the room. Touching the faucet with your gloves is acceptable. We just can't contaminate clean hands by not using a paper towel to turn it off. When you wash your hands your dirty hands are turning on a dirty sink. I agree wholeheartedly.

    Yep, you sure did! Great critical thinking! We want to limit the possibility of bacterial contamination of the urinary drainage bag that may eventually cause a urinary tract infection (UTI). This is a common complication with catheters and the bacteria will multiply on the journey to the bladder so by the time they arrive in the bladder, the bacterial infection is overwhelming for the patient and can be very difficult to control. These infections can even cause death in rare cases. Good luck!

    That is a good practice and you may kneel on the barrier on the floor, if you wish. These videos demonstrate MINIMUM testing standards and since this is not a testing point, it is not demonstrated. These videos are to be used as a guide for testing standards, not as a substitute for training where those points can be discussed at length. But good observation. Good luck!

    Thanks for your comment. Kneeling on the floor does not affect your performance for the exam. Many people would have difficulty balancing while squatting for that amount of time, which may cause them to spill the urine. Your uniform is considered unclean after beginning work, because it touches unclean surfaces like the privacy curtain, bed, sink, etc. The floor is just one more unclean surface that you touch.     The CNA pulled the urine container out of the drawer without gloves on. Shouldn't you have the gloves on first even though the container was rinsed out? I always wore gloves before touching anything that had come in contact after washing. This is what my instructor taught me 13 years ago.

    That is fine, you may do so. These containers have been disinfected and you are washing your hands before and after, so it is not a testing standard to have gloves on. Remember that these videos show MINIMUM testing standards and may not reflect all the steps of clinical practice. Just be careful what you touch with gloves on because you may inadvertently transfer contamination to other surfaces. Good luck!

    You will record the time you are documenting. You are never required to raise the bed to waist level for any skill (because some beds in some settings cannot be adjusted) but if you do, you MUST put it down at the end of the skill. Post-mortem care is not performed on the state test as a clinical skill, but you must have a general idea of what is required for this skill because it may be on the written test. Good luck!

    You will wash your hands after performing your closing, then document. This keeps your hands clean when handling the pen to prevent cross-contamination. Once you document, you do not have to wash your hands again (this is NOT a testing point). You may, if you wish, but it is not required and certainly not preferred practice for a healthcare setting. Handwashing is done after the opening (before gathering materials) and after the closing. This is appropriate for the test. Good luck!

    Yes, you should have gloves on if you are going to be touching the catheter. The tubing may have urine or other body fluids on it. If you are just LOOKING at the catheter and not touching it, gloves are not necessary. Hope this helps!

    Blood pressure is no longer a tested skill in Florida. You should learn the proper way to obtain a blood pressure, however, because it is an important component of being a CNA. CNAs are required to manually check the blood pressure if an automated BP device registers an abnormal BP, so knowing the proper way to take a manual BP is very important!     The drainage port should not contact the inside of the container to avoid bacterial contamination that could eventually ascend into the bladder. For the exam, it does not matter if a gloved finger touches the inside of the container. However, The finger never made contact inside the container, so I am unsure what you mean. Good luck.

    Every state will have different requirements, however, I have reviewed New York's specific requirements and cleaning the container twice is not required for the exam. You can clean twice if you wish (it is not counted against you) but it is unnecessary. This container is only used to measure and transport this single patient's urine and the patient never comes into contact with the container. Thanks for the comment!

    So after you drained a bag. "While getting on a dirty floor, contaminating your scrubs" you turned the sink on with the same glove??? Hmm hope someone don't get pee on their hands from the dirty sink fixtures! If you are going to train do it the right way.

    No. You must score a 70% on the exam to pass. Missing a few questions will not fail you. Missing a lot of questions in a single category may fail you, however, so it is good to have an overall understanding of the theory related to: 1)role of the nurse aide 2)promotion of safety 3)promotion of function and health of residents 4)basic nursing care provided by nurse aides 5)providing specialized care for residents with changes. These are the topics you will be tested on. Good luck!

    They told us that under no condition can our uniforms touch the floor (we need a barrier for the knee) because of Clostridium difficile in hospitals. Seems like our procedure was much more elaborate.

    Closing the lid on the toilet is NOT tested on the CNA exam. You may close the lid if you wish, but it is not required for the exam. Thank you and good luck.

    Hello from New York State. I just wanted to stop by and thank you for your instructional videos. I have taken the Nurse-aide Licensure exam about 4 years ago and I completed 3 skills for them the way you completed them. Superb instructional videos. I believe the way you perform these skills is excellent and you are excellent at key infection control. Thank you so much for all your help. When I Perform Patient care I do exactly as you do! I was even promoted because the nursing supervisors said I am truly excellent at my job. I have one question since your are extremely intelligent at this.. How many times should a Urinary catheter drainage bag be emptied per shift? I always empty the bag when it hits half way . I just want to make sure this is correct. Thank you for all...

Congratulations on your promotion! Regarding the catheter drainage bag: you will follow the instructions in the care plan (if specified). If there are no instructions, general nursing principles dictate that the drainage bag should be emptied once per shift (usually at the end of the shift) AND as needed prior to the end of the shift if the bag is approximately 3/4 of the way full (this would be for patients on diuretics). If you empty the bag during the shift due to excessive urine production, you must document the amount on the I&O form and still empty the bag again at the end of the shift (so the new shift always starts with a clean slate) and document that amount as well. Emptying at the 1/2 mark is fine, but remember to empty at the end of shift too! I hope this helps and wish you well in your career!!! :-)

    This skill may be done with a manikin in the bed or a real person. It does not matter which one is used for this skill, as there is very little involvement with the "patient" other than during the opening or closing. Hope this helps. Good luck on the test!

    Quick question, when doing this skill on the day of exam do you clean the tip of the urinary bag with an alcohol wipe from where you empty it from?

    You should be talking to the PATIENT, explaining step-by-step what you are doing to gain their cooperation and lessen their fear and anxiety. Think of what you would expect if you were the patient lying in bed, alone and scared, not knowing what was wrong with you or what the helathcare worker was doing to you. By doing so, you will satisfy the requirements of the test and make the evaluator aware of all steps you are completing. But the focus is on the PATIENT, not the evaluator. Thanks!

    Is it ok to raise the bed to make it easier to drain the bag? If so, do you have to lower it before you go measure the urine?

    Why would you need to alcohol swab the drain on the reservoir? It's not in direct contact w/the client (i.e. swabbing saline locked IV sites), and you don't get cultures or perform a urinalysis on urine in the foley since it's not sterile. Unless perhaps the bacteria could migrate into the reservoir, up the tubing, & into the body (I think I just answered my own question).

    after recording the measurements don't you have to wash your hands again?? Always before you leave the patients room, that's what my teacher said to us she stressed the importance that we would be washing our hands 3 times for this specific skill.

      Please refer to the new videos for updated skills performance. You want to be very careful not to introduce bacteria into the drainage system, where it could multiply and progress upward toward the bladder. Since the pathogens are enveloped inside the catheter tubing, they bypass all defense mechanisms and emerge from the catheter directly into the bladder. It is an overwhelming invasion with trillions of bacteria invading all at once - that can be dangerous to the patient. Keeping the port from touching ANYTHING will reduce the amount of bacteria that are introduced into the catheter bag and reduce the likelihood of a UTI.     You will physically wash your hands before and after every skill until the evaluator tells you that you can simulate handwashing. You are being graded on handwashing at the beginning of the first skill and must continue to wash your hands until told otherwise. Some states do not allow simulation at all and candidates must wash their hands before and after each skill because you cannot simulate in real life and if you don't wash hands during the test, then you probably won't in real life either!

    I don't know if this is legal for the exam but can I give you a little tip . If you think this right I was thinking once you have a chux, you can take the bag and take the first pair of gloves and then re-glove.

      There is no reason to use 2 sets of gloves for this skill. it is not required for the exam and serves no useful purpose. One set of gloves, as demonstrated in the video, is the correct way to perform this skill according to current state standards. Thank you for your comment!

    When catheterization, idk if it s for the test, but if there is a helper to hand stuff does that person have to wear only one or both gloves?

      First, catheterization is not a testable skill for the state CNA exam. Catheter CARE is a testable skill, which involves cleaning the catheter and the peri area. For the CNA exam, every skill will be performed alone, with no helpers. However, in a clinical setting, if the person assisting with this procedure will be touching personal skin or body fluids or any soiled items, gloves must be worn. Hope this helps!

      +Yuri Barros Yes, gloves must be worn anytime you come into contact with personal skin, body fluids or any soiled items. If both hands are being used, both hands need gloves.

    after pouring the content of the container inside the toilet, you need to grab a clean dry paper towel to on the faucet instead of using the soiled gloves

      It is fine to use a paper towel to turn faucet on, but not required since the faucet is considered unclean (as are your gloves). You may use a paper towel if you wish-but it is not a graded point. Thank you for your comment.

    She put her finger inside the measuring container. It might not be a new one therefore, is not clean. Don't put your finger in there even if you have gloves. At least in Oregon this is what we are supposed to do.

    Ewww, do NOT kneel on a filthy disgusting floor! And later your knees touch a patient's bed? Don't pass that disgust everywhere you go! Either squat down or put ANY barrier(folded sheet, towel, chuck, etc.) between the floor and you. I know 4yourCNA defends this by saying your uniform is considered contaminated when you start your shift, BUT there are different levels of contamination and the floor is worse than most. For instance, you might put a patients pillows on a chair in the room, but you wouldn't put them on the floor! Never put anything on the floor that you can't sterilize or throw out(or incinerate!)!

    You don't even know the patient cuz it's the first time meeting the patient so how the hell you didn't ask the patient to see her name tag these shit will let you fail your test😏

      Many patients well not have ID BRACELETS (home care, assisted living, long term care) so this is not on the state exam. Identifying the patient by name is the appropriate action for the state exam, which is demonstrated in the video.
