    Yes! One of my favourite movies of all time! The twist blew my mind the first time I saw it! Wish I could go back and see it again for the first time

    There's an interesting theory out there that the Tyler alter ego twist was really just a kind of red herring to a much bigger twist that few ever put together. What must always be remembered is that Jack/Tyler is what we call an unreliable narrator meaning we cannot trust anything that he says or shows us as an audience bc the story is being told from the perspective of someone who has no credibility. There's a good video of this theory on YT but to summarize it goes like this.
    Insomnia was never really what he was suffering from but rather he was diagnosed with testicular cancer. This is why there is such an emphasis in various parts of the film related to cancer (notice Tyler and Marla are almost never without a cigarette?) and cutting off balls. Jack, or the real Tyler as that is his real name, is terrified of losing his masculinity (and his balls) and therefore creates Bob, then Marla, and of course Tyler. Bob is the alter ego who is a more pathetic, more exaggerated version of himself basically ready to accept his fate and simply accept the misery of his new life, Marla is the emasculated version of himself trying to show that his balls aren't what makes him a man and his life isn't over if he loses them, and Tyler is the uber masculine version who believes extreme measures must be taken to fight this reality and retain his masculinity. These alter egos are waging war inside Tyler's mind throughout the entire film on how to best deal with his situation. There are tons of clues to this theory but to list just a couple that suggest they are not real: Bob wears and dies in the exact same shirt we were shown earlier in the film when talking about the large man who died in the burned car with fat melted to the seat. Tyler is shown as a member of the hotel staff on the television set before Jack/Tyler creates what he will actually look like when he meets him on the plane. And Marla has the same medication Tyler asks for sitting on her nightstand, walks through traffic like a ghost, steals from everyone and anyone, and even shows up at a testicular cancer support group yet the only person who ever even acknowledges her is Tyler and/or other figments of Tyler's imagination. Ultimately, the Marla alter ego wins out as Tyler's mind ends up killing off the other two and finally choosing how to accept his diagnosis and live the rest of his life. This is why the final scene has Marla and Tyler looking nearly identical to each other, with Tyler seemingly at peace despite having shot himself and not wearing pants a popular metaphorical sign of masculinity, as they watch everything get set back to zero and start again anew.

      Didn't know it was a theory, always found it weird that the only backstories we are provided with are the narrators, Bob, Marla, and Tyler. Haven't really thought about him having testicular cancer but that would definetly explain the absurdity of those cut balling scenes and also could tie into the doctors scene, insomnia caused by the idea of having testicular cancer and him being there in the first place because of the cancer and not because of the insomnia, he just asks about it. And your description about each of them actually lines up with what i was also thinking.
      First we are presented with his character, a succesful assurance guy? with lots of money having issues sleeping for a period of 6 months where he is troubled by the cancer news and don't really want to accept it, causing him insomnia.
      After 6 months he starts accepting the inevitable and goes the first time to one of the support groups for TC, always found it weird for the doctor to suggest it out of the blue. Then comes Bob, from an influencial aclaimed and awarded body builder to that version of Bob influenced by the cancer, mirroring the narrator, who accepts it and sobs and cries over it, having gone through with the procedure, it shows him one of the outcomes.
      After some more time, comes Marla, that version of him that did the procedure and results in all the self loathing from Bob being replaced by self destruction, that version of yourself that you turn into when you just do not care anymore, similar to Tyler (think about when you did something perceived as destructive and you double and triple down since it does not matter to you anymore). The difference being that Marla only resorts to self destruction while Tyler exposes the world to that destructive behavior.
      And Tyler, that version of him that realizes he hasn't really accomplished anything and all the self-loathing/destruction gets turned into anger he puts out on the world, towards a society that abandoned him since '...these things do not interest me; what interests me is tv with 500 channels, viagra, rogaine etc" basically he never thought he could get cancer, he never smoked, was living a decent life from what we've been shown, and all his money couldn't change anything, that's why he also blows his appartment.
      We also have the bathroom conversations "We are a generation raised by woman, i wonder if another women is the answer we need"
      Then we have the 'besides humping, Marla and Tyler were never in the same room"
      Why is he deciding to kill Tyler? Because he chose a women, a mother figure who was always truthful and nurturing in favor of an irresponsible imaginary version of his dad that only created broken families "never met my dad; changes cities every 6 years creating a new family" - Tyler: "like he's creating franchises" same as the narrator said at the end "Tyler was busy creating franchises all over the country".
      The scene at the end after he leaves the hotel is again extremely telling. He first calls Marla as soon as he goes into the hotel room. Then Tyler takes over, does all the calls from the phone, and the narrator wakes up. After waking, he rushes to leave the building but is held by the receptionist asking about the calls. He is asking 'what, what calls, when were this made?!" basically not remembering the first call with Marla he did when he was in control, so that means that call never happened since it makes sense not to remember Tylers calls but not his.
      Another 2 scenes that make me wonder are the Chloe scenes, they were so random, firstly we meet Chloe begging for sex, and the next scene with her is when the narrator ignores her and goes to grab Marla to tell her off when Chloe was approaching him. I don't think it's a coincidence, Chloe and him were pretty much alike, he was ashamed of her going that low with the speech but he feels exactly the same, longing for companionship, and opting for Marla, an imaginary stranger he never spoke to instead of actually Chloe is the moment he gives in. Same as what happened with Tyler when he gave in with the scar scene or the car crashing scene "just let go" after which he is just 'being Tyler more and more". Don't know..
      2:33 aren't those balls? looks weirder than actual lungs
