That’s how they’re designed so multiple spikes don’t penetrate the tyre at the same time causing a massive blowout which could cause the vehicle to flip.
It will still puncture the run flat tyre and your right you can still drive on it but at very low speeds with no sharp turning etc or the tyres just rip themselves to bits. So yes they do work on run flats by puncturing the tyre and damaging it to the point it just disintegrates with heat and heavy load, so you would still be able to continue driving for a bit longer than if your on normal tyres but in the end its the same result.
It will pop the tyre just like ordinary tyres and damage the sidewall in some cases, either way runflats are not designed for high speed or sharp turning etc so even if you could still drive on them after a while the tyres would fall to bits under the heat and pressure.

When you deflate a run flat is it not the idea of the tyre to continue when flat??
