    Dr. Lome- I've been eating WFPB for 4 years and am doing a pretty good job of it with the exception of plant fats (nuts, avocados). Drs Essylstyn and Ornish got their results with very low fat plant based diets but how do we know that fat is the critical factor? These diets have a lot going for them beyond the fat content. Whole plant fats have essential fatty acids and are rich in other nutrients. They also been associated with lower rates of stroke and generally cultures who eat high fat plant diets fare well. I've never gotten a satisfying answer to the question "are plant fats really bad?" Just wondered what your thoughts are? Thanks. -Mike

      Michael Miller Great questions! Animal models to reverse atherosclerosis have also always been very low fat and similarly the only diet in humans that has been shown to do the same are very low fat 8-12%). Research trials comparing plant fats to animal fats always substitute one for another and don’t include an arm with very low-fat or no fat (i.e. 8-12% total fat calories). When animal fats are compared to plant fats of course plant fats seem superior and appear to reduce overall cardiovascular risk. This is what we call a “substitutionary benefit”, but it does not mean that plant fats are healthy, just less unhealthy than animal fats. Certainly if you included more plant fats in your diet from childhood while eating a predominately or exclusively plant-based diet avoiding animal products you should not develop cardiovascular disease especially if your blood LDL cholesterol remains less than 70 as I talked about in my previous video. However someone who is already diagnosed with heart disease looking to reverse it really needs to go way down on the fat according to the data.

      @@DrStevenLome Thanks so much for your thoughtful response. You bring up a good point about getting away with a higher plant fat WFPB diet if one begins in childhood. My dietary changes moving toward a vegan diet happened very slowly over a period of decades and I didn’t go fully WFPB (no oil) until age 51. My LDL is high for a vegan at 102 (total cholesterol 151) so maybe I will reduce the nuts and avocados.
      I still struggle a bit with why nature would provide us with nuts and other high fat foods plant foods rife with nutrients and apparent other health benefits. I’d love to see a human study comparing higher fat WFPB vs low fat WFPB. In the meantime I couldn’t be happier with this lifestyle, 22 lbs gone, back pain gone, skin condition gone, improved lipids, improved mood, reduced vision prescription, etc. I follow in my grandmother’s steps who had a heart attack/stents at age 75, 2nd heart attack/bypass a few years later, and then thrived (event-free) until the age of 93 following Dr. Ornish.       Michael Miller You are welcome! Just go all in on the Esselstyn diet for 3-4 weeks then get a lipid profile to see how low your LDL goes. You can order lipid profiles online without a doctor and to check LDL it does’t even have to be a fasting test! Good luck!

      You seem to be confusing Fats in their natural state and Oils which are highly processed by humans. There is a difference.

      RonGallaghercollection even fats in their natural state from whole plants will not reverse heart disease. Certainly eating fats from plants instead of animals from birth will result in lower blood cholesterol levels and prevent heart disease to some degree. Reversing heart disease however requires all fats to be very low... even fats from whole plant foods.

      @@DrStevenLome You are a very gentle spirit. I appreciate your videos. I went through heart failure and had a stroke (I lost the tip of a toe), and also had a metal heart valve put in. I was born with a bicuspid valve. I was on Warfarin for a few months. I started to get messages from Source which suggested I go vegan. I was already interested in being vegan before I got sick, so I was open to the idea. I also got messages suggesting that I stop taking all medication. I was intensely afraid to stop taking Coumadin, but I did anyway. And I stopped my other pills as well. That was 2015. So far, so good. I take 2 aspirin a day. I rarely think about being afraid of not being on blood thinners…. I believe in healing from Source, love between people, and I think in terms of fields. Being vegan is one factor, and the others are psychic health and the willingness to be honest and fair in life as much as possible. I am 44. If I live a long, healthy life, I would love to tell others my story. It’s dumbfounding and profound. It’s a risk, but so is Warfarin. That stuff is poison. It made me feel sickly all the time, as well as depressed. I am better off without it.

    I’ve already had one of the risky procedures you talk of, full CABG. Been fit and active all my life but I think too much oil red meat saturated fats etc I was on Keto diet 6 months prior to cardiac event! So now 4 weeks into recovery am following a plant based low fat diet for life and loving it. Aiming to keep my cholesterol low and new tubes clear to ensure a full recovery and be fitter than before.

    I have been on a WFPB SOS free, lots of dark leafy greens for my omega 3s and it's working except my LDL. At 65 years, angina LDL 222 during NSTEMI . 2 stents in LAD. On AMLA 4,000mg in place of Lipitor for 2 weeks and LDL is 139. I need to recheck it again. How soon should I recheck this. I have a familial tendency to high LDL. This Amla is exciting what do you think.

      Kaye Pellow AMLA does have some data...but not much and not long term with hard endpoints like MI or death. Certainly does not appear to be harmful. If you are protecting your endothelium and have negative inflammatory markers, most lifestyle medicine experts would say you are fine doing what you are doing at your current LDL, but the lower the better on the LDL. Even though I hate to say it if I was you I would still try a low dose statin as well to try to get LDL under 70 or even lower...

    Hi Dr Lome, I've been WFPBNO for just over a year - cholesterol slowly coming down. You say in the video above that we shouldn't have too much ground linseed. Can you please quantify that? Thank you!

      Zoe Alford Two tablespoons per day to get your omegas is OK according to Esselstyn data, but no other nuts/seeds or high fat plant foods. This is of course assuming heart disease is present.

      How old is a person when they start to acquire hard plaque I know multiple factors will matter in regards to this, also can a person become healthy as in very fit in the gym and endurance if they have calcified arteries or will it be detrimental on the circulation.

    Dr Lome, what are triglycerides? Are they free floating oils or are they in a particle form like ldl and Hdl particles? Curious too what are mono- and di-glycerides which I’ve seen added to processed foods.

    If someone has a Coronary Calcium CT test and it shows "0" calcium deposits, does that mean their risk of having heart disease is 0? Or does that test not show soft plaque and therefore they could easily have early heart disease? Thanks for the succinct yet detailed coverage of these topics!

      @Citizenthirteen Thank you--what you say makes sense. I know folks who are being told by their docs that their "0" coronary calcium score means no heart disease. Seems a bit misleading.

      @@marianblum1064most people who have a score of 0 are on a plant base diet or consume little to no cholesterol. Most red meat eaters and those who have high cholesterol have a score of at least 1 or more

    I went on a vegan diet and my cholesterol was still 185. I avoided all animal food
    and ate a zero cholesterol diet and was still at 185. The only thing that
    dropped it was a drug called Repatha but it is so expensive and insurance
    fights you tooth and nail with respect to being able to get it/. I went
    from 185 to 50 with that drug. I have had a heart attack and have
    three stents. I am not over weight. I WORK OUT AND WEIGH 140 LBS
    AT 5'8. I HAD THE HEART ATTACK AT 60 years of age and am now 66.
    (That damned caps lock!!!!!! UGH!) In my case I was told it is just genetic.
    Some of us just live with high cholesterol no matter what we do other then
    to take drugs. Even statins would not lower my cholesterol. But, I found I was
    allergic to statins (Massive joint and muscle pain within two days).

    Dr. Lome: I have been on Dr Esselstyn's diet for some time now and have achieved the desired lipid profile, but my cardiologist refuses me to stop the statins based on a study that shows that these drugs prevent secondary MI. Is there any truth in this, and at what stage can I stop the medication?

      Hello! Glad to hear you’re doing well on the whole food plant-based diet and your cholesterol numbers have come down. Certainly data shows that heart disease patients have a mortality reduction on statin therapy. The data concluding this did not include people on Esselstyn‘s diet. Thus there truly is no data to answer your question accurately. If your LDL is really low at least less than 70 and inflammatory markers are normal as well, theoretically you should be fine. We would usually at least continue a very low dose of a statin therapy according to guidelines however if somebody insisted and understood the lack of data mentioned above I am OK taking people off the statin. Hope that answers your question. Each situation is different. Find a plant based cardiologist near you and discuss.

    Question: I had a Cat scan to find that my carotid was blocked 70%. I went on a WFPB diet, no cheating. I What is the best follow up test to see if it remained the same or reversed? I'm claustrophobic with an MRI. ( I love your videos, thank you!)

      Companion Animal Outreach - Sarah Manns Thank you for your comment! Your best bet is a carotid ultrasound. Any other form of imaging will either exposure to radiation or require MRI. Get your LDL cholesterol crazy low...stick to WFPB. Honestly the best way to know that your carotid artery is doing good is simply the fact that you feel good without any stroke symptoms! Wish you the best!

    Why is it that people with low cholesterol have been seen in studies to still have heart attacks ? Also why isn't total cholesterol being taken into account here, as cholesterol is an important part of the body.

    Has the brachial artery tourniquet test ever been done using animal fats such as nuts, avocado, soybean? Weren't Dr. Esselstyn's patients really, really sick as in near death with no hope? We all should follow such a strict diet? I understand no animal products, no oil and keeping fat before a certain percentage but for everyone?

      I think it's acceptable to have a few nuts & avocado if you don't have heart disease. Those trying to reverse it cannot have them.

    As a professional, I am very encouraged by your channel. A colleague had sent me a link, where you were defending Dr Fuhrman's high-fat diet. I was disappointed, that you supported Dr Fuhrman's high-fat ( 30 -40 %) recommendations. (There are a multitude of Dr Fuhrman's videos and interviews available to interested researchers, in which he often cites a 30 -40 % fat diet as healthy and desirable).. This is not only incorrect, but actually life-threatening to patients with atherosclerosis. Dr Caldwell Essylstyn and Dr Dean Ornish have published multiple reputable studies in noted journals on the dangerous effects nuts have on patients with atherosclerosis heart disease. But reading your comments here, and watching your videos, I find that you DO NOT, in fact support Dr Fuhrman's demonstratively false assertions. As you yourself posted here; "but it does not mean that plant fats are healthy, just less unhealthy than animal fats." Kudos Dr Lome. I applaud your courage and honesty in setting the record straight. As someone who is well-versed in the literature, it is very unfortunate that Dr Fuhrman, and alas, Dr Greger are both seriously misinformed about the treatment of atherosclerosis. These men have done outstanding work in bringing a healthier lifestyle to thousands of Americans. Unfortunately, both of these men have recently been lured, it would appear, by nut-industry-funded fake-science which disregards the contraindication of oils and nuts to patients suffering with atherosclerosis. This is no small issue, as you are obviously aware. It is unfortunate to see both men's reputation suffer such a self-inflicted mortal wound. As an aside, There is a gentleman who is posting videos renouncing the false-science that Greger and Fuhrman continue to promote.       John Burns Thank you for your comment and kind words. I have never recommended a high fat diet. Low fat whole food vegan is what I preach. From a heart disease perspective it makes sense to limit fat (especially saturated fat) to the degree that gets your LDL cholesterol as low as possible. Hope you enjoy the videos!

      HDvids101 Thanknyou for your thoughts! The scientific evidence shows that even naturally grown meat has cholesterol and saturated fat (actually the same amount as factory farmed) which will raise LDL cholesterol and contribute to heart disease. Only a 100% plant based diet has been shown to reverse heart disease as the video states.
