    The very best exercise after TKR is to get a rocking chair and whenever you sit down sit and gently rock. You will find that the motion will keep the joint lubricated and will easily increase your ability to bend it. My replacement is now 20 years old and functions quite normally. Longevity is the goal.

    I've had total knee replacement on both knees over 10 years ago, it's the best thing I ever did,wish I'd done it sooner.I found cycling was the best exercise for total recovery.

    I think this exercise is exactly what I need for more flex action I can't do the squats comfortably but believe this might be my best bet, I'm only at 100 and its been eight month, doctor just told me to do squats and that would help me get there. But I think this might be what I need to help me get to the squat. Anyway I'm gonna try it. Can only help to try it.

    Sorry, two questions. I didn't have TKR, but have extremely stiff joints from prolonged inflammation (gout) . How can i tell difference between scaring and simply a shortened muscle (quad)? Secondly, is there a version of this exercise which a person can do by themselves. I have done the sit on edge of chair and push stiff leg back exercise. I can do that easily but doesn't seem to help with improving flexion.

    I am a b/l Tkr patient since 25 May 2015..Max hospital Mohali gLife is l living hell, I feel implant have become loose and there is .CRP ,bone scan,angio etc are WNL. Primary implant Smith &Nephew used.Tell me who can advise me on revision.When are you available.Regards

    i tore my meniscus, i've been putting a pillow between my butt and heels and sitting back on my heals. the pillow allows me not to flex too aggressively

      How far out was your MUA from your original surgery? Did you ever find out the best exercise to keep the knee bending normally? Was the MUA painful afterwards?

      It’s best to ask your Ortho Surgeon or your PT . Not everything on TH-cam is the best advice for every patient .

    Had TKR some two months ago. The patient in this video seems not to feel pain when the doctor moves her legs even against resistance . Anyone does that to me and I jump to the ceiling. It still hurts for me to move my legs the way this patient is being made to move hers. I am making gradual progress in range of motion but very very incrementally. One physician assistant told me that I should be moving my legs normally after two weeks. Not so. It took me about 6 weeks to be able to get a 80 degree bend from nearly 0 degrees after the surgery.
    I feel that though some people recover very quickly and are moving normally after a week or sooner, I and many other people have a much longer recovery process. It is hard for me to get others to understand it.

      Me too, I’m 7 & 1/2 weeks post op and couldn’t bear this. I’ve managed to get a 95 degree flexion by this point n feel really disappointed. I’m pushing myself but the results are very slow. If you believe all the case studies on. the internet ( even the NHS) 120 flex is the norm / goal at 6 weeks. It’s unrealistic for a lot of people. I’ve had misinformation & conflicting advice at every turn from discharge nurses / physios to community physios & my surgeon regarding how to sleep / sit, how hard to go at the physio ( stop at a 5/10 / if it makes you grimace.) Then saw the surgeon at 7 weeks post op who said, “ do it ‘til you can’t bear the pain’ it’s a bit late telling me that at 7 weeks! I wish I’d never had it done, nor listened to all the people saying, “ get it done, you’ll be good as new, pain-free” etc. I feel like I’ve elected to become crippled. I had a limp before and couldn’t walk long distances but worked full time, swam, walked my little dogs & looked after my home and family. The other knee is also knackered but I’ll live with it.

      It's very long must visit your surgeon.....and ask for physiotherapy.

      @@i_am_sandeep10 I guess there is no time frame limit for bending the knee. All kinds of progress is achieved with physical therapy after the necessary surgical intervention.

      I had 3 manipulations done on my knee.Still only bends at 63°.

      @@cyaxares-median That is a very logical and accurate comment. Try asking your doctor if there is a standard time frame for people to get full range of motion or better range of motion by a certain amount of time after the surgery. He or she probably would not be able to answer.

    I was able to do 121 degrees after a knee replacement, knee manipulation, and about 3 months of physical therapy. However now, I feel as if I cannot bend much again. The knee is still swollen after 5 months. Should I do this and not force to bend more? Will I gradually bend as I was doing before?

      hi, my wife had her knee done 6 years ago, it's not a quick fix, but gradual, she did all the necessary exercises relentlessly, she had 110-degree flexion, but through scar tissue build-up has now reduced to less than 90 degrees, the hospital advised her to do lunges as often as possible, but I think this video is really helpful and am looking forward to my wife's knee regaining full flexion

      "@amyr3285" I cannot say I know for sure. I am no expert and what is said in TH-cam cannot be considered official medical advise but I have seen videos and read comments in which people say that doing a lot of intense exercises can make it worse and you need to rest and not always push it. I think the knee being swollen after 5 months is not normal and you need to contact a competent professional to be on the safe side.

      However ever you are able to bend your knee pre op it’s great indicator what you will be able to accomplish post op.
      Many Ortho Docs recommend some physical therapy before the TKA to relax & loosen up your muscles and promote better flexibility ,that’s if you are not in too much pain and are able to tolerate therapy

      "@pamelawilliams2739" I am two months after surgery and made some progress in range of motion but some people lead me to believe I should have made even better progress than I had. I am sorry you are still experiencing difficulties but it helps me to realize that some people take longer than others. I have heard that some people take a year to completely get normal movement and range of motion.

    I am a bit disgusted by so called "experts" and gurus providing exercise advice yet have likely never worked in rehab with a total knee. After working with over 1000 total knees, this doc is not wrong in stating that excessive and aggressive stretching may increase scar formation and stiffness. In fact, very early and aggressive stretching will lead to increased swelling and pain, resulting in more stiffness and likely microtrauma. As for his exercises of straight leg raises and his high torque stretches, its absolute nonsense. The crank-like stretching does not follow arthrokinematic motions and can actually damage the prosthesis. The straight leg raises work the hip flexor more so than the quads, and cause shearing forces in the low back and hip. Also the leg is heavier and weaker post surgery and the leg raises are very difficult for people to perform. In fact an active straight leg raise is often used a special test to test for a hip impingement or lumbar instability. His statement of starting this exercise at 6 weeks in unsubstantiated , as by 6 weeks, most people are finished with rehab and can just start progressing their normal routines. I suggest, focus on managing swelling the first 4-8 weeks post op, so a lot of elevation of the leg and repeated knee movements, eventually cycling. As for gradual strengthening, start with natural movements at 2-4 weeks like squatting, light leg press, total gym squats, supported step ups and eventually step downs. If scar tissue sets in, try sustained quadriceps stretches with a strap lying prone and hold for 3-10 min a few times per day for flexion, and for extension perform "modified hangs" seated in a chair using some weight for similar durations. Modified hangs can be started immediately post op, while prone quad stretches a few weeks after surgery as the incision has to have healed enough to tolerate this. My advice, don't let people crank on it unless late in the rehab phase and they are trying manipulate scar tissue in the knee (6+ weeks post op). And yes, to some of the comments below, knee motion after a total knee or almost any knee surgery can be improved even years after with persistent sustained stretches of 3-10min multiple times per day. Some good late but aggressive late stage stretches (8 + weeks post: prone hangs with weight just below knee (up to 10 lbs) for 10min for knee extension, and for flexion perform quadruped to heel sits on a mattress. Remember that the longer the knee has been stiffer, the more difficult it will be to get back the motion, but possible.

      My RPT ( PHD )said “different strokes for different folks , some patients may not respond the same way .
      If you look him up, this Highly regarded Ortho. Surgeon and co- founder of GRIT Belgian Sports Clinic is probably more of an Expert then 95% of youtube Doctors & Therapist .
      I’m pretty sure that he knows what he is teaching here.
      I would not refer to him as “ so called expert” unless you can “ beat him” at his credentials & experience

      One thing I do agree with (because I understand it really well) is that certain exercises work the hip than the knee and I have been going through physical therapy for the past six weeks or so after my TKR some three weeks before beginning that therapy. I have been put to do exercises that only hurt my hip and strain the hip (and I had double hip replacement the year before). So I often have to tell the therapists to avoid giving me certain exercises that cause hip pain rather than help the knee. Furthermore I have to insist that making it hurt as much a possible does not really help.
      It is a difficult struggle.
