This song is based on a poem Till wrote about himself being ugly. He's not mocking her for being ugly, he's expressing the horror and desperation of feeling unworthy and the importance of seeing others with your heart.

You are not catching the true meaning of the lyrics.
He doesn't understands it because he is looking at the lyrics with a spesific image of rammstein and till in his head. Many reactors just se how badass they are and don't care about the rest.
That's why I said in couple of my other Rammstein reactions that I won't always get the real message behind their songs right away and if I missed something or got the wrong message to correct me in comment section. I'm just giving my first impression in 10-15 minute video and I don't have time to sit back and think deeply more time about the song. I caught now the true meaning from comments and it made a perfect sense to me. Thanks for pointing out.
