I really don’t want surgery. I was told I have EXTREME SEVERE deterioration in both my hips. I hear you that strengthening the glutes are IMPORTANT to create balance. I have very limited mobility moving around. I just turned 60. I really do not want surgery. I am an alternative gal. My pelvis journey has been 4 years now with pain and discomfort walking. It’s affected living a normal life. I am optimistic! I trust the process AND. I am curious. Thank you for this video as it was both helpful and thought provoking
I was diagnosed with bone on bone hip arthritis, on one side, when I was 27 (I have a connective tissue disorder). I was given no advice, just a walking stick and that I should try to put off surgery. Diagnosis in the other hip at 50. Despite some restricted lateral movement in both hips and occasional flare-ups, I’m still managing to live my life normally (and mostly pain-free) at 68, with no surgery. Now, I work to strengthen my glutes which has also helped. Thanks for your advice - I would probably have a greater range of movement if I’d been given it in my 20s.
Thank you, Dr. David! I'm working on my glutes. Thank goodness, they both still fire. My three younger siblings have all had hip replacement. I'm 73, and am going to listen to you!
Wish I'd come across your channel 7 years ago when this arthritis problem noticeably worsened. That "focus on your glutes and not on your front muscles" is so spot-on for me. Any suggestions for doing that in a pool? I feel like I could be a poster girl for this video. Such a relief to learn that it could take a month to get the glutes to activate independently.
This is as big a 🦆 as I have seen on youtube. Who is he. Credentials? Just trying to profit on the back of one of the world's most widespread diseases.. Beware folks. Do't pay him a penny and take his advice with a big pinch of salt. From someone with ten years experience of O.A. .
Thank you for this video. In 2019 I had a full right hip replacement at the age of 47. Last year my left hip started hurting along with both knees. I have been so depressed with the thought of another hip replacement and then two knee replacements. Now I have hope that I may be able to avoid those surgeries with strengthening the right muscles. I wish I lived in Texas I would be a patient.❤
I don't have 90% mobility. BUT I don't want to get surgery. I was diagnosed with HO in 2012. Following your protocol, strengthing gluts, lower abs. Seem to be improving, but I really feel it when temperature changes or precipitation in the Forcast. Thanks for your enthusiasm 😊
You are very welcome John! We are sorry to hear that you are having such bad problems with your hip :-( We are glad to hear that it seems to be getting better though! Keep up the great work and we would love an update in the future! 😀
I have been doing yoga for over 20 years….when my hip starting acting up I thought it was my psoas muscle…so i did more yoga….not a lot of glute work….your videos are giving me hope that I can improve my hip oa. Working on my glutes with your helpful information. Thanks so much…I have even changed the way I walk at your suggestion!!

Hi Laura, Thank you so much for your kind words! We’re so glad to hear that you’re feeling hopeful and excited to start implementing the protocol. Taking that first step is such an important part of the journey. You’ve got this! 🙌
I wish this information was available 12 years ago when I started having hip/back pain. I’ve lived with chronic pain and monthly visits to pain clinics and still unable to sit for more than 30 minutes to an hour at a time. Last month I went back to the ortho hoping it was time for a THR and possibly an end to this constant nagging back hip and leg pain. I was told that a THR is not in my near future and my spacing is still good. BTW I’ve also had full imaging, lumbar, pelvis and hip and the only issue they have found was FIA and a torn labrum. I really had given up on PT been there and done that over a dozen different therapists. I ended up worse after treatment every time. I have no training but I can tell you that 15-20 minutes on a bike every visit to warm you up is NOT the way to treat a torn labrum,FAI and arthritis from personal experience. I’m going to throw everything I can at this one last time. Following your channels PT protocol and looking at regenerative options PRP and peptides. 15 years dozens of doctors and $10k or more worth of injections and imaging and the medical systems best has offered was Norco 🙁
You are on to something doc. Sitting for office work for 20-30 years gets everything out of alignment. Stand wrong ,hold head wrong from looking at a computer, walk wrong. Tight glutes certainly helps alleviate posture frim just sitting and having no glutes even just walking on flat cement. Thanks for paying attention to the balance but we just have to live outdoors more.
Yeah, I heard that all from 2 ortho docs and I was in so much pain, so -> surgery. Pain continued months after and then I was introduced to Dr David who talked about glutes. Got his program, worked it, pain subsided more and more, 95% less, bc I was diagnosed with L3/4 bulging disc. It may never have been necessary to have that hip replacement. All I know now, is whatever Dr. David says is the truth for me and. Keep doing all the exercises, daily! Thanks Dr. D. ❤
Hey Elvire! We are so sorry to hear that you were in so much pain. We are so excited to hear that you have noticed a 95% improvement! Keep up the great work and we really appreciate you sharing your testimonial with us!
@epmanualphysicaltherapy And I was doing so good, and started to slack a bit. YIKES - horrible back/thigh pain since 8 days, can hardly walk. So taking it easy and back full force attention in the back program exercises plus infra red wrap and bed. Probably takes a week or 2, so now I know that apart from doing the regular ones I cannot go without those specific back/glute ones. Thanks Dr. D it IS muscle imbalance. Period! 😃
Great info. Your advice helped so much with my knee. No current hip problems bur watched just to avoid. Glutes for both knee and hip double duty from my exercises.
Hi Patricia, Thank you so much for your kind words! We’re so glad to hear that the advice has helped your knee. Being proactive about your hip health is a great approach, and you're absolutely right-strong glutes play a key role in supporting both the knees and hips! Wishing you continued strength and pain-free movement!
Everything he said i agree with. I kept pointing out my inceases in pain and got dumb answers if any but in the end they were flexion and tight rectus. I did therapy which included rectus stretch and some glutes and got good releif but not that balanced strength enough. Got posterior hip replacement resurfacing but my strong flexers, adductors, incised glutes caused me to badly injure tissues in front of joint. 2 years post surgery ,4.5 years not being able to do my favourite activities and still trouble. Terribly weak glutes and strong flexors , poor walking. He said it all.I have not heard anybody explain this so SPOT ON! I regret the surgery. My other hip has worse arthritis and has little problems.
Thank You for this information. For the past couple years my pelvis tends to rotate causing the hip to pop. Very painful when it happens. I can't walk when it does. When it goes away I would try to strengthen the muscles, but through your teaching I've realized why those exercises haven't worked. I didn't understand why or what excercies to do since the ones I know were not working and actually causing the pain. I was perplexed. I have been thinking it was the lack of abdominal strength, so again you confirmed a lot of ideas for me. Thanks again.
Hey Rachel! Hip dysplasia can be complicated and is something that drastically differs between people. This makes it very difficult to generalize things that would be helpful for this condition.
Good day Sir...thank you..thank you ..i found you..opertion always doctor advice for me about my hip's i ignore the advice to do operation ..i follow your explanation step by step days come i feel good now no more so much paint sometimes i take vitamin d every other day and magnezium..drink only black coffee avoid sugar.. and don't carry so much heavy things..againt thank you sir..
I learned from some of the Postural Restoration TH-cam videos that the trigger for full glute activation during a walking cycle on either side is the move from the outside of the foot rolling over onto the ball of the foot behind the big toe, where you push off from. I’ve practiced this with a walker/rollater and it makes a big difference. I’ve also seen a presentation from a corporate safety officer that discussed the fact that people quite literally “forget how to walk” as they get older and bad postural habits accumulate.
Hi jSheapullen, It’s never too late to start making progress, and we’re so glad you found this information helpful! Thank you for your kind words, and keep going-you’re on the right path!
Yup flatter than captain asking me to walk the plank. Working on it. Even after 15 months of physio. Just now can fire the glutes again building capacity up. Thanks for that know how...
QUESTION: I just switched from a cane to a walker to off-load weight more on the OA hip during this time to quiet the inflammation. Do I lightly hop/step on the ball of the foot or lightly place my whole foot down on the OA side? Yup, I left the surgeon's office and cried in the parking lot. Then I got MAD and have been looking around since then to get the help that makes more sense to what my body is feeling. Your conversations and information having given me new hope, thank you!
@juliaryan1410 - sounds like my exact story when I was told I needed hip replacement and Dr wanted to give cortisone shot to relieve pain. I left office and cried in my car also. Been diagnosed with Left hip OA, trying to do what I can to avoid surgery. Hope you feel better and find some relief with these videos 🙏🏼.
Can you discuss bone cysts? I'm told I have a cyst in the bone above the hip joint. Will balancing the muscles address that? Or is there another treatment for it, if indeed, it's a problem?
Thank you so much! I'm very sensitive to x-ray can't receive them or any type of radiation. And I have metal in my body somehow so no MRIs. That's okay. It started with a herniated disc. I'm just worried that I hear my femur hitting on my pelvic bone. I have pelvic interior tilt and one leg is longer than the other one. Any ideas for best help? I'm strengthening my glutes decompression feels great.
Do you know any Drs in the Houston area that have your same beliefs on hip arthritis? I wish I lived in your area because I would loved to avoid hip replacement. I definitely feel like imbalance in my muscles.
Hi Saj, We’d love to help! If you’re interested in a virtual consultation, you can find more details here: www.epmanualphysicaltherapy.com/virtual-visit/. Wishing you the best on your journey to relief and recovery!
I am back again!!!! After helping my knee arthritis with your advice ( glutes ect), I am now slammed with great pain in my lower back. Can barely walk, bend etc. Suspect herniated disc/ sciatica. Do these exercises help this kind of problem. 🇬🇧. Age almost 80.
So bad I couldn't sleep for 4 days as it started years ago gradually from acute to chronic pain and as a fully insured upper middle class 62 yr old guy, I still didn't go to the Doctor. So, after 3 Chiropractor visits and after 3 weeks of chronic pain and stretches, things are getting better. I must admit, it was so bad a friend gave me a Lortab and the pain went, "poof". but as an educated person, I want no part or map to opioid addiction so I'm sticking with the stretching and chiropractor. I'm researching PRP or stem cell if it goes back to pre pain of a month ago.
You helped me last month w a sciatic problem. The glute and low ab exercises worked immediately! Very good video....back it up w needed exercises! Thankyou so very much
Hi personally am a person with hip joint restriction, cant bring my knee to my chest. I suffer this from ankylosing spondylitis, will your member program help? Doctor say i most probably need hip replacement
Hey Jason, sorry to hear about your AS and hip arthritis :-( The Hip Arthritis Recovery Program can definitely benefit you even with the spine condition. The program is designed to be safe for other conditions, plus you'll be able to get on a live call and ask questions. Even if you do absolutely need surgery, going through this program will be beneficial so that your have the best result after a hip replacement.
Dr David , yeah, did I ever freak out when I got my hospital diagnosis, sever arthritis in my left hip with bone on bone opposition, subchondral cirrhosis and a flattening of the femoral head, an EDS problem. There is advanced degenerative disease of the spine L5 and S1. PLEASE PLEASE can I repair this problem with hip exercises, I don’t want surgery as all my connective tissues is a problem as I suffer from EDS also. Before I knew this information I was exercising every day for an hour or more, and my hip got worse and worse, now I can’t walk far, or carry anything heavy and the crunching and snapping is off the charts, painful and noisy. But before I had my slip and fall my hip hardly hurt, even though I’m sure it was bad then , but was held together by ligaments , now they are all stretched out and my left hip is now unstable. PLEASE 🙏 ANY RECOMMENDATIONS. 3:01
I have hip arthritis and orthopedists are recommending total hip replacement. I’d like to try your recommendations first, though. My hips have become unaligned; one hip is lower than the other, so I limp. Does your program address this issue?
Hi Adey, We’re sorry to hear you’ve been struggling with hip arthritis and the challenges it brings, like hip misalignment and limping. Many people don’t realize how difficult it can be to navigate these issues until they experience them themselves. It’s great to hear that you’re interested in exploring non-surgical options first-you’re taking a proactive and empowering step! We recommend starting with our Hip Arthritis Help Playlist on TH-cam, which offers free exercises and insights: th-cam.com/play/PLJVIPiFvv1mbICbP6qlXUZwMyAJlKD1dt.html. These exercises can help improve alignment, mobility, and strength. If you’re looking for a more structured approach, our Hip Arthritis Recovery Program dives deeper into addressing muscle imbalances and movement patterns that may contribute to hip misalignment. You can find more information here: epmpt.mykajabi.com/hip-arthritis-recovery-program. Stay consistent, start slow, and listen to your body. Many have seen great progress with these recommendations, and we’re hopeful they can help you too! 💪 Keep at it-you’re doing something great for your hips!
Thank you so much for sharing your gift. I am having a hip problem and began your methods to strengthen targeted muscles. I would love to go back to skiing 🎿 but gentle skiing on the bunny slope. Leg strength and balance is imperative. What do you recommend to prepare? How long? 🤔🤔🙏🙏🙏 Thank you!!❤
Hey Lorna! Using the elliptical can be a method for targeting the glutes. It is important to concentrate on those specific muscles while you are exercising and determine whether or not the glutes are actively performing the majority of the motion.
Hey Lorna, Here is a link to a video that has some great glute exercises. The one that feels the best to you is likely the best exercise for you to do. Top 6 Standing Glute Exercises To Help Knee Pain th-cam.com/video/jD7IRBfJ-mM/w-d-xo.html
Dear David thanks a lot for your vedios , should we use one cane in the opposite side of the arthritic joint or 2 crutches to keep the arthritic hip off the ground completely ? Pls answer me ASAP
No body mentioned butt to me these years with hip OA. Mother recently who has memory issue said my butt flat as flat as pancake lol. Said i am working on it...
We're glad you're aware of it now Frank! You're right no one in healthcare talks about butt muscles and their importance with hip arthritis!! Get those glutes going!!!
Hi Carole! Here are a couple of glute exercise playlists. We hope this helps! “Top 5 Glute Muscle Exercises For Knee Pain” th-cam.com/video/NXdCjT2QtJA/w-d-xo.html “Top 6 Standing Glute Exercises To Help Knee Pain” th-cam.com/video/jD7IRBfJ-mM/w-d-xo.html
i literally just got back from the doctor after having xrays done on my hip, he tells me i have OA as both my hips (extra portion of bone around the head of my femur). meanwhile late last year i was having physio on a hip injury sustained at jiujitsu, physio tells me i have a week right glute as a result of the hip issue so strengthen that booty cheek. fast forward from christmas to new years week and (again at jiujitsu) my right quad gets strained and then i have two weeks (the last two weeks) of MASSIVE pain in my right leg hip (which is why i went to the doc for xrays) this video seems more inline with what is going on with my leg then what the doctor said because i dont understand how i can have no pain or issues in my hips and then two weeks later its my OA causing these issues
That is a great way to look at it Kenith! Pain from OA should get progressively worse! Sudden onset of massive pain does not add up to simply an osteoarthritis problem. We have a lot of great content for this if you would like to learn more. Hi Christian, We’re sorry to hear that you’ve been struggling with such severe hip pain. Many people don’t know what it’s like to face bone-on-bone arthritis and the prospect of hip replacements unless they’ve experienced it themselves. Thank you for sharing your story with us-it’s not easy, but you’re taking an important step by exploring your options. You might find our Hip Arthritis Help Playlist helpful: Hip Arthritis Help Playlist. It includes exercises and tips that could help provide some relief and improve strength while preparing for surgery. If you’re looking for a more structured approach, our Hip Arthritis Recovery Program may also be worth exploring: Hip Arthritis Recovery Program. Stay encouraged-whether you’re preparing for surgery or working to improve your hips now, every step forward counts. You’ve got this!
I am not a candidate---ever---for hip (bilateral) replacements. I am 100% dependent on a walker for EVERY step. My question is, will strengthening the glutes and the abs, "help" the pain I have to live with for the rest of my life? Or...is it just always going to be this way, so...why bother? I've avoided a wheelchair---except for transport---for 3 years. I'm not getting worse, nor better. My hips do grind and pop, but what hurts the most is the top of my thigh. So....? Thank you.
Hey Verne! We are so sorry to hear about your hip pains :-( Pain in the hip can be due to many different issues. Sometimes it is multiple issues converging to create a lot of pain! For cases that are really difficult we like to advise virtual consultations to discuss anything and everything that may be able to help you start feeling better. The nice part about this service is it can be conducted remotely so you do not have to be at our clinic in person. Hi I have a hip fracture,,what do I need to get cured...pls advice as every doctors advice different opinion...some say to tale rest 24 hours and some say continue with ur normal routine...I am suffering since 6 months kindly advice...can v contact each other for better solution...pls do advice...
Your words described ne, today. Juet increased meds, time to restart physio. To use glutes, stop using leg muscles. Age 69 soon. Big imbalance but can isolate glutes. Yoga experience mught help. Ill look for 28 day program.
What annoys me is that none of the therapists that I've seen have explained this to me. If you've studied the body wouldn't it be logical that the glutes is the main thing to focus on. 🤦♀️ it's narrow mindedness. The book says this so that's what it is. Think outside the box for gods sake
There's a lot of don't do this and don't do that. What about some positivity? Like DO this and DO that. Or do you first have to PAY YOU to hear this? Next

I’ve also seen a presentation from a corporate safety officer that discussed the fact that people quite literally “forget how to walk” as they get older and bad postural habits accumulate.
I learned too late about the glutes...but so glad I learned. Thank you.
Hi jSheapullen,
It’s never too late to start making progress, and we’re so glad you found this information helpful! Thank you for your kind words, and keep going-you’re on the right path!
Are pelvic floor physical therapy exercises good for the hip?
If the cartilage is worn down so you are bone on bone, is that recoverable? Also torn and frayed labrum and some muscle tears.
Excellent information!
Great Elfrida! So glad to hear that you like our information!
Yup flatter than captain asking me to walk the plank. Working on it. Even after 15 months of physio. Just now can fire the glutes again building capacity up. Thanks for that know how...
Built up thigh and dominant walk. Changing that waking up my glutes. Median and mini have a bit of strength bit glute needs strengthen.
How about activation of both glute at same time while walking initially
QUESTION: I just switched from a cane to a walker to off-load weight more on the OA hip during this time to quiet the inflammation. Do I lightly hop/step on the ball of the foot or lightly place my whole foot down on the OA side? Yup, I left the surgeon's office and cried in the parking lot. Then I got MAD and have been looking around since then to get the help that makes more sense to what my body is feeling. Your conversations and information having given me new hope, thank you!
I use two canes , like mountain hikers and have a walker for breaks from walking with the canes. It helps.
He does have an online program. All the best.
Any chance of getting a assessment done?
Hi Saj,
We’d love to help!
Big problem is the hip pain. I always heard is necassary to walk, also with pain. I will change this.
What glute exercises would you suggest? How to do them?
You helped me last month w a sciatic problem. The glute and low ab exercises worked immediately! Very good video....back it up w needed exercises! Thankyou so very much
Hi personally am a person with hip joint restriction, cant bring my knee to my chest. I suffer this from ankylosing spondylitis, will your member program help? Doctor say i most probably need hip replacement
Hey Jason, sorry to hear about your AS and hip arthritis :-(
The Hip Arthritis Recovery Program can definitely benefit you even with the spine condition.
I’d like to try your recommendations
first, though. My hips have become unaligned; one hip is lower than the other, so I limp. Does your program address this issue?
Hi Adey,
We’re sorry to hear you’ve been struggling with hip arthritis and the challenges it brings, like hip misalignment and limping.
How long? 🤔🤔🙏🙏🙏 Thank you!!❤
Is using the elliptical a good exercise for strengthening glutes
Hey Lorna! Using the elliptical can be a method for targeting the glutes. It is important to concentrate on those specific muscles while you are exercising and determine whether or not the glutes are actively performing the majority of the motion.
@@epmanualphysicaltherapy thank you so much what is the best gluten exercise

What are the best and worst exercises in the gym for strengthening beyond initial strengthening and activation via glute squeezes and bridges?
Do you have a video on firing up your glutes? Walking properly?
Dear David thanks a lot for your vedios , should we use one cane in the opposite side of the arthritic joint or 2 crutches to keep the arthritic hip off the ground completely ? Pls answer me ASAP
No body mentioned butt to me these years with hip OA. Mother recently who has memory issue said my butt flat as flat as pancake lol. Said i am working on it...
We're glad you're aware of it now Frank! You're right no one in healthcare talks about butt muscles and their importance with hip arthritis!! Get those glutes going!!!
Please can we have some glute exercises.
Hi Carole!

I keep working mu GLUTES , so am looking for my Hips to align them selfs. Will they do this .
i literally just got back from the doctor after having xrays done on my hip, he tells me i have OA as both my hips (extra portion of bone around the head of my femur). meanwhile late last year i was having physio on a hip injury sustained at jiujitsu, physio tells me i have a week right glute as a result of the hip issue so strengthen that booty cheek. fast forward from christmas to new years week and (again at jiujitsu) my right quad gets strained and then i have two weeks (the last two weeks) of MASSIVE pain in my right leg hip (which is why i went to the doc for xrays)
this video seems more inline with what is going on with my leg then what the doctor said because i dont understand how i can have no pain or issues in my hips and then two weeks later its my OA causing these issues
That is a great way to look at it Kenith! Pain from OA should get progressively worse! Sudden onset of massive pain does not add up to simply an osteoarthritis problem. We have a lot of great content for this if you would like to learn more. Here is a link to our playlist for hi p arthritis:

My doctor told me my hips are finished. It's bone on bone. I'm scheduled for hip replacement on both hips later this year
Hi Christian,
We’re sorry to hear that you’ve been struggling with such severe hip pain. Many people don’t know what it’s like to face bone-on-bone arthritis and the prospect of hip replacements unless they’ve experienced it themselves. I got a problem with low back the pain is so strong and goes to my for leg I cant sleep . At nights
Me too, not every night, but yes, it's horrible 😟
What if you have trendelenburg gait?
What about if your diagnosis is AVN?
can avascular necrosis (AVN) of the hip be healed with the same routine??
What if my angle at knee ( knee to chest) flexion is 85 degrees, do I still have a chance at avoiding the hip replacement??
The dr. I went to Saif I have the hips of an 80 year old, and I was 55.
Thx 2025
😊
Here is a link to this service if you are interested in learning more about it:

What annoys me is that none of the therapists that I've seen have explained this to me. If you've studied the body wouldn't it be logical that the glutes is the main thing to focus on. 🤦♀️ it's narrow mindedness. The book says this so that's what it is. Think outside the box for gods sake
Only for me to find out bilateral dysplasia angles 20+
Its my job 10....to 15 miles a day
I cant drink the tables even hi blood tables
Why would anyone pay a physio when everything of worth can be found online?

@@epmanualphysicaltherapy yes. I’m hoping you can with your program. Your philosophy matches what I believe.
