In Sonic code gray, I think that they did not implement a death animation. Since no animation exists, the game crashes. Hope this helps.

I wish it wasn't late at night when I first watched this video. Then I could've screamed with uncontrolled laughter.
The reading sonic code grey would freeze and put text on screen is because the console has a built in debugger to detect when a game isn’t functioning correctly and will proceed to lock up the cpu.
The reason why it would get stuck on the note it was playing was because when the cpu locked up, it still sent signals to the synthesizers but it would just be the same note since the cpu locked up is basically the same as when a program stops responding in windows but the whole pc isn’t responding.

This game that you said about Ketchup and Phantom Sonic is called "An Ordinary Sonic Hack" featuring villain, called "Sonic.exe" also called by Creepypasta, when Tom got disk with word written with marker that says "SONIC.EXE" and note from Kyle says to NOT play this game, but he loved Sonic games, so he loaded it up and he can't turn it off.
