    EMT-CC here. To do CPR effectively it is recommended to have a firm, flat surface under the patient. If they "dig a hole" under the patients back not only are chest compressions compromised in efficiency but there could also be spinal damage.

      And from the story it sounded like OP was being told to dig a hole under the neck so that the head dips backwards, not only spinal damage but breaking the neck

      Yep I am CPR certified and I have never been told about putting anything under a patient's back when doing CPR.

      @@etcetera1995 true, which is screwed up - I'm a nurse because I want to help people get better, but there's definitely people who are in the profession to go on a power trip. Usually, they end up moving up the management ladder either because they want even more power and/or to get them away from patient care because they usually suck at it!

      @@TheKillerqueen40 It's infuriating- the job SHOULD attract the best in people, because they require compassion and intelligence and care for others. And of course they *do* attract decent people, I'm not so cynical as to think that *every* teacher hates children and every doctor is just out to pad their bank account. But the worse case examples stand out so much more than the ones who do their jobs for the right reason.

    Hey, I’m CPR certified and what that lady said I’d possibly SOME OF THE WORST ADVICE FOR CPR EVER.

      I mean, not to mention, with CPR, you need to start ASAP and not stop until ambulance/EMT take over.. if you waste time to dig a hole, that's vital moments that could result in the person not having oxygen, potentially causing brain damage or death.
      And even if you have someone else dig the hole, once you start, you cant stop or move the person.
      So that's just dumb advice from "Maggie"
      And I only have basic CPR and First Aid certificates

      Just a thought, but what if she gave that advice knowing it was bad so that OP could potentially screw up big time and get fired. If that's true, then she was willing to risk an innocent person's life just to get rid of someone she didn't like.

      I know very rudimentary CPR (due to elderly people visiting my workplace and some elderly home care in the past), and the digging would not only waste time for life-saving but also not align their spine and position lungs/heart correctly to be able to save a life. If I may quote Dr Mike here on TH-cam: "Chest compressions. Chest compressions. Chest compressions"...

    I can guarantee you Maggie wouldn’t have lasted more than a semester had she gotten into that nursing course. I know people who have been in nursing programs and it can get pretty cut throat and you can get booted for the smallest infractions. Her stupid “you’re doing it wrong, this is how you do it” bs and superiority complex would no doubt have gotten her kicked out almost immediately

      I agree. I got through the program and graduated in December. She would have made so many enemies in the program with her attitude. She also did not have the right work ethic to study for those exams. She would have failed horribly

    The CPR part is the worst part of this for me. I think that she genuinely believes that instead of pushing whatever is inside of you out so you can breathe again, pushing you inside a hole in the ground will fix the problem

    You’re right about the cpr. A hole is an awful idea both because of less pressure and because sand is a great material to do things like cpr on because it’s very moldable yet stiff.

    Hey Rslash, you're absolutely correct about the CPR thing. Along with not being as effective, having a hole under them would also take extremely valuable time away from actually saving the victim, when in most cases you only have a couple of minutes to actually react due to the fact that the brain can only survive without oxygen for a few minutes before tissue starts to die.

    As a former Self-Aid and Buddy Care Instructor in the Air Force, you do NOT dig a hole under the person you are performing CPR to. You can do more harm, than good.

There were a few more telltale signs that no matter where you are that are noticeable, like OP thinking that you could just do a course to transfer from being a pool lifeguard to a beach lifegurd and as a newb at the beach would be offered the 2IC role. Also if you are on patrol at the beach you don't just drop everything and train others, hell you would be shot for not you know, watching. But yeah, I only gave the first example because it is easily searchable.

    I’m CPR certified for my job-I work with kids, but not on a beach. I’m a figure skating coach and I primarily coach groups of about 10 kids or adults, depending on the class. What they told us in training, is “if someone starts going into cardiac arrest, or something else happens and they need CPR, take off your jacket, lay it underneath them and use the hood as a pillow, then start CPR immediately. It ALWAYS has to be on a flat, even, stable surface”. On the beach, digging a hole in the ground when your performing emergency CPR is the exact opposite of what you’re supposed to do and be prepared for (according to the instructor who gave us the class and certification).

    ProRevenge tip: If you have a toxic employee that you have to work with, document _everything_ that s/he does to you and to other co-workers. After enough evidence is gathered, present said evidence to the Manager, HR, or other relevant office that could deal with the problem.

    I subscribed because you present these items so well. Then you did two things: gave a wannabe TH-cam pro some kind and wise advice. You said you have to make 100 bad videos before you make one good one, etc. Then you read a CB post where the beggar went back and forth with a therapist who could nor fit her kid into his schedule. The way you alternated voices gave such a perfect picture of her I wished the button said "Love" instead of "like." I decided to binge your Playlist and like each one. (Munch on that, algorithm!) The more time I spent enjoying your gentle but spot on humor tne more I knew that I had to offer more support. After all, anyone who isn't a Choosing Beggar knows that money is how we honor people's time and talents. So you have a new patron. Keep sifting stories and I will be listening and enjoying.       Honestly, having Maggie suffer in the harsh reality of life is the best revenge of all. Because she saw her own little world just fall apart due to her entitlement and stupidity. Now that she doesn't have that, she's learning the hard way that life isn't so forgiving when you have your head so far up your ass. I mean, she lost her scholarship and got KICKED OUT of the college course in her second year. Better yet, if she doesn't straighten up soon, it's just gonna get much harder and harder.

    Just passed my Standard First aid course in May, and omg. Digging the hole would not only compromise the stability of the patient you're working on, (uneven surfaces + compressions = high chance of spinal injury), but to take the time to dig the hole could in some cases kill the patient. Time is not your friend when the person literally cannot breathe, or their heart is not pumping. When you are doing CPR, you are in a sense that person's heart and lungs. If you dig a hole under them, their chances of surviving plummet. I cannot express just how relieved I am that that woman did not make it into nursing.

      oh, the digging a hole was miscommunication, she said "if you do CPR in the sand you will end up digging a hole", for starters that's not how regular sand works, you will not end up buried on the sand by someone doing pressure on your chest... she was literally making up lies about how Op was doing something wrong when he wasn't.

    I think what Maggie was saying was that "if you do CPR on the beach without a back-board, the chest compressions would cause the body to dig a hole". It's different than your interpretation of what she said, but just as ridiculous.

      If that's the case then, depending on the type of sand, that's actually solid advice. I've been around CPR being done on loose desert sand, and we got a hard board, which the medic requested. It was the type of sand that only packs together after you sink into it, like snow. Not what you want when you need to perform CPR and every second counts. So, while the medic was getting the patient ready to receive CPR, we broke the legs off a table. No idea what happened to the guy, but he was breathing when he got onto the medevac.

    Lifeguard here for almost 7 years. Have been told that the surface more or less doesn't care (You only need a stable one), for CPR. So as long as the ground is flat, and nothing can give more damage to the person, the better.
    The only time you need to think about the surface is when using the defibrillator, on a dry space, since you guessed it) it gives a shock with electricity.
    So as long as you can have the person on a stable surface, with more or less nothing under their back, you are fine to do CPR.

    I had a similar ‘rookie’ interaction when training as a dental assistant. One of the actual dental assistants at my work prac kept calling ‘freshie’, saying that most of the freshies don’t last because of the smells from peoples dirty mouths, halitosis and stuff.
    I let her go on and on and at the end of my first day zi told her that bad smells won’t be an issue since I was born without a sense of smell.
    I also passed my pracs with flying colours.

    I too was a lifeguard instructor/examiner and a First Aid Instructor/Examiner. Never have I heard of digging a hole underneath a victim. You are correct; for the most effective CPR, the victim should be on a hard flat surface, otherwise the chest compressions are practically useless. A quick response time and proper technique is vital in emergency situations. Digging a hole would be basically digging their grave.

    As someone that was a lifeguard for 9 years, I'm baffled at this lifeguard for her belief that you "need to dig a hole". Not only is it idiotic for the reason that you need a flat surface under the person, but it's also completely stupid for someone to have the idea of digging a hole. If you were to dig a hole, you're wasting time and risking that person dying.

      This is my second time hearing the story, and it's been killing me trying to figure it out. The part that makes me think it is not Cape Breton, is that I've never seen life guards there. Of course I'm from the mainland, so I've only been to that part of the province a few times and definitely haven't visited every beach. So it is possible that there could be lifeguards there? I was thinking it is more likely to be NB. The topography there is similar to NS.

Sounds like everyone else on that beach wanted rid of her, and in the end they gave OP a reason to do so. Also hilarious to see that right at the end that everyone else in Maggie's family hated her and her mother. But I guess we all know where Maggie got her narcissim from now...

    Chest compressions are to make the heart beat, not to force water out of the lungs. But yes, a hole under them would make compressions significantly less effective, or ineffective altogether.

    To agree with all the other people that have commented on CPR practices, a firm sturdy dry surface area is the most ideal for CPR. Firm and dry is key . Your chest compressions may be impeded if the surface is not hard enough. Also if you dig a hole in the sand underneath the patient, there may be more moisture which can also affect the usage of the AED that is to be used in combination with CPR if available

    i was a lifeguard in my youth (albeit almost 40 years ago). I worked mostly at the local man made city pools, but occasionally I'd be asked to work on the beach at a nearby lake. I had to assist with two CPR events while at the beach. The first time I pulled the woman out of the water while another guard brought a BACKBOARD to help solidify the ground and make it firmer. The second time the board was missing (another story for another day) and we just went to work. Digging a hole not only wastes time but would hinder the CPR efforts and possibly do other bodily harm. If Maggie would have been caught doing that by our supervisors (or had enough staff document and report it), she would have been immediately terminated and her certifications brought up for review.

    R/Slash, many people here have commented how to do cpr correctly, so thanks everyone! I do personally suggest however that you take a cpr course for infants and keep a chart near where your daughter eats, so you feel confident in saving her life if it comes to it. We love her like our own, so keep baby/slash safe!

    Finally, the great and powerful r/slash can use my knowledge!
    I’m a doctor, I have to know CPR. I’ve been through several CPR courses, though not one specifically for a lifeguard and I’m not an anaesthesiologist.
    1) digging a hole is a waste of precious time. One needs to start right away with the CPR if it’s needed (need implies checking the pulse and breathing). Only two things you need to do before is call for help (CPR is very tiresome, it’s always good for several people to do it and the most experienced one to coordinate and/or apply intermediate/advanced life support alongside basic life support) *and* check if it’s safe for you to apply it (you cannot save anyone if you’re about to hurt and/or incapacitate yourself; you’re not being a hero, but an idiot).
    2) unnecessary turning and carrying the patient without proper equipment could hurt them (further). Especially if they have broken bones.
    3) you assume right; you want to perform CPR on a hard surface. Sand isn’t exactly hard, but it will support the patient well enough. Digging a hole behind their body will likely put strain on their spine and ribs, raising the risk of actually hurting your patient while you try to apply heart massage, especially if they’re old (old people’s cartilage and bones are more brittle than those of young people, especially children, and even a proper CPR can break their ribs).

    I'm pretty late, but for a certain course I needed to get a CPR certification and one of the first things you learn is to make sure they're on a flat surface. She literally knows nothing unless she intentionally was trying to get OP to get fired via bad advice.

    The excuse for what she said about CPR was that while doing it on sand you'll end up digging a hole under the patient by preforming the compressions. The quote 5:27 "If you don't have anything under them you'll dig a hole in the sand!" Basically she was saying that doing it on the sand like that was not a stable surface. Sand does not compress in that way, and the back of a person will make it a stable surface. So no hole will be dug by the patients back while doing the compressions.

    Its been a few years since I learned CPR, but one of the key parts is making sure the person receiving is stable/on a firm and flat surface. I wasn't a lifeguard or anything of the sort, but I'd have to imagine sand being annoying for such purposes, even more so if you dug a hole under their back. Not to mention any injuries that might come from it. Lady was 100% BSing.

      I mean others have said here that Sand isn't that bad considering it might seem soft and brittle but it can hold well. Making a hole however is what really messes with what Maggie's "pointers" were

      Yeah, over here in Finland generally everyone that is not self-employed has gone through the "First Aid Course EA1" at least one time in their life, whether it's in any of the schools or even at workplace
      ( this necessarily doesn't mean everyone has a first-aid-card in the first place. Though some want a up-to-date "EA / FA1"-card on each of their faculty/workforce-member ).
      When I was in boy-scouts, we rehearsed all the first-aid at least once every 6-months.
      And in all of the schools, there was at least a day dedicated solely on first-aid which also included CPR.
      Over the years the "routines" have gotten far more simple
      ( especially the "recovery-position / semi-prone / NATO--coma-position" in which you adjust the patient to "stay-up" on their side ).
      But we've been also given various other new tools to train with most important being the wall-mounted emergency / automated external defibrillator (AED).

    *rSlash, your request has been answered: I am a CPR Instructor Trainer (as well as an AFA Instructor Trainer): I train the instructors who train the people doing the CPR, so yes, I am qualified to answer your questions.*
    *CPR requires a firm surface under the patient in order to work. While sand is not perfect, it's close, because of it's high density and it's peculiar physical characteristics, i.e., it will conform to the shape of the patient as you proceed. In **_theory,_** I can **_maybe_** make an argument for Maggie's position on "digging a hole". If you take a moment to wipe away the dry sand to get to the wet sand, it would be minutely "stiffer", and therefore a slightly (although likely immeasurably) "better" surface, although, really, I doubt it. Realistically, there **_should_** be a backboard available to slide under your patient, and then CPR should proceed apace after being positioned upon the board.*
    *TLDR: While Maggie's position on "digging a hole" **_might_** be defensible **_in theory,_** in practice it would not be a practice that I would certify. Nevertheless, it's a an interesting question in an abstract kind of way, and I'll try to do a little research on it the next time I have an hour and nothing to do. "Maggie" gets an "F Minus: for being a ridiculous piece of work who is more interested in showing off than she is in potentially saving anyone who may need it. Further, her "retraining" of others could actually result in harm, thus violating the First Principle of Medicine ("First, Do No Harm"). Were she my lifeguard to recertify, I would fail her and have her start over from scratch - correctly.*

    ER-RN of 30 years. CPR- we teach that the first thing to do is get your patient to a flat solid surface. Hospital beds are designed with a “CPR” Handle, which immediately drops the bed flat and raises a solid board under the mattress so that chest compressions are as effective as possible. Dig a hole under the patient? Why? Its a waste of time and energy. It would make more sense to lay the patient on a board, like a surf board or a back board. Someone didn’t understand the priciple of CPR or didn’t care that incorrect and useless steps are being advised.

    When I took my first CPR course, the instructor taught us to perform chest compressions at the base of the person’s sternum, completely contrary to what our handbook (and common sense) said. When I took this to a higher-up, they confirmed that this was, indeed, an extremely bad idea, and could crush their diaphragm or cause the person’s lower ribs to fracture and dig inwards, potentially puncturing the lungs and suffocating them, not to mention having next to no effect on pumping blood through the heart. The instructor disappeared the next day, and we just continued with lifeguard training as if nothing had happened, but as I worked as a junior lifeguard the next summer I was in a state of constant terror that I had been taught something else wrong and would screw up someone else’s life with a critical mistake.
    Once I got through that summer, (luckily the worst thing I had to deal with was a little girl who wandered off the end of a dock and bobbed around in her life jacket until we found her), I never had the courage to go back to lifeguarding again 😬

    18:04 just a little note on this, CPR isn’t about the lungs, it’s about manually pumping the heart. So yes, the digging a hole underneath is the WORST thing to do because you don’t have a surface to push against and thus can’t compress the heart to get blood pumping

    So Rslash, I think you misunderstood the story a little.
    Maggie wasn’t saying he needed to dig a hole before performing CPR-she was saying that if he didn’t have something under the patient, he would be pushing the sand underneath and effectively digging a hole while performing the CPR, and likely that it wouldn’t be as effective as a result or something. Basically: She was saying that he needed to put them on a solid surface and that sand doesn’t count.
    I am NOT CPR certified (took a course as a kid but it’s been years). But I do remember something about solid surface so like don’t lay them on a couch or bed to do the CPR-the ground is better.

    I'm certified in CPR. One of the first things they teach you is to have a firm, flat surface under the person in need. Sand is already not the ideal environment, but digging a hole makes it worse as you could ruin the person's spine

    The thing is that you read it incorrectly, Rslash, according to the story she didn't say that you need to dig a hole before doing CPR, but that if you do CPR on sand without putting something underneath the person, you just dig a hole underneath them 5:24 (apparently she thinks that while doing chest compressions, the send is pushed aside and a hole is formed beneath the patient, so chest compressions are not effective enough), that's why he says that it "doesn't make sense" because he has "basic knowledge of physics that pushing on sand isn't the same thing as digging", which isn't a sensible response if your reading is correct.

    Hi Firefighter here. To perform CPR you should preferably have an even and sturdy floor. Sand does work as it is sturdy when compressed under the weight of a body. Digging a hole is nonsense. It not only takes precious time away from continuing blood flow to the brain but also impair the effectiveness of the heart massage given by the helper.
    Don't be afraid to give CPR the person is already on deaths door. And a few broken ribs are better than a not beating heart. The attempt alone is better than doing nothing.

    I've been in Boy Scouts, First responder training, AND Pool life guard training. I have never heard of digging a hole for a patient in any of the courses. The issue here was that Maggie had it out for OP, and (Likely) felt he would outshine her, which resulted in all her negative traits being pulled into the light by OP.

    I was a lifeguard for 2.5 years. Listening to this story physically made me cringe. I was the head lifeguard and helped trained new lifeguards coming in. Her level of "better than you" would have never flown at my work. I regularly wrote up guards that acted like that and I went out of my way to be nice to my guards. I always had amazing reviews from customers and other guards. That's how you're supposed to do it!

    I've listened to this episode sooooo many times and only now do I finally understand what Maggie and OP were saying about CPR. The recommendation is to press on the sand, to compact it, so that it's solid and firm, so that you can perform CPS effectively and safely. But somehow, Maggie though this meant to dig a hole in the sand.

    Living in a beach town we (my family and a lot of locals) call non-locals “blow ins”. Pre-COVID we had a ton of blow ins. With lock down we still have them with just a few less (easy to tell, they drive VERY slow pass the beach)

    Hey, I'm a certified to perform CPR and it has very little to do with the lungs and everything to do with the heart. The point of CPR is to keep the patient's blood pumping by forcefully beating their heart. You have to compress the chest down 5-6 cm (~2.5 cm) and digging a hole under the chest would make compressions completely ineffective.

    I'm redcross cpr certified and lifeguard instructor certified. What I can say for certain is everything Maggie said was some grade A bullshit. The sand isn't an ideal place to do cpr, but digging a hole behind the patients back is infinitely worse. Plus it wastes valuable time you could've been saving this person's life.

    CPR certified train attendant here, you are wasting time digging a hole, an seconds are valuable in such situations. Also you risk spinal fracturing on top of the ribs you'll break doing it RIGHT.

    Yes, that was insanely incompetent of her. Digging a hole under the patient would not only take up crucial time, it would either be to small for their torso to fit in but would potentially take away from some of the pressure on their back while you pressed on their chest or if it was a big enough hole for their torso to fit in, then the patient isn't laying flat on their back... there is no scenario where this isn't a HUGE NO NO!

    I was CPR certified once upon a time, and I have no idea what this "dig a hole" nonsense is about. They told me that chest compressions are by far the most important thing to be doing. Any time digging a hole is time not spent doing chest compressions.
