      I've had two years of suffering with a bad mesh implant. Im not blaming the mesh. I blame the surgeon who felt his expertise was such that he didn't need to carry out a pre op exam. I blame the surgeon who didn't feel the need to check the size and location of the hernia during the surgery. I blame the surgeon for selecting a mesh that didn't provide anywhere near enough coverage on one side of my hernia and way too much coverage on the other side. I blame the surgeon's colleague who carried out a diagnostic laparoscopy and concluded that the mesh was correctly placed by virtue of it being placed in accordance with the manufacturer instructions, not correctly covering the hernia because his laparoscopy could not locate the original hernia. I also blame him for reporting that the coverage was good because it was a huge mesh. I blame him for failing to engage with or answer the questions that were asked and not having enough about him to admit that he couldn't answer the question. I'm in the UK and the NHS makes it almost impossible to genuinely dispute a clinical finding. The NHS complaints policy in practice really is that another practitioner reads what the 1st practitioner recorded and repeats it. They then state that it complies with the procedures even when you've produced copies of those procedures that are the complete opposite of what was done. I don't fundamentally blame the mesh, there are many meshes out there to suit the application. The real problem is that the majority of hernia surgeons are not hernia specialists and just do hernias on the side, the UK has no surgeons who only do hernias. They don't have the time to dedicate to studying the different types of mesh's and their different applications. The best way to repair a hernia is to put the time and effort into getting it right first time. Where problems arise it's usually because corners were cut.

      @ I understand your frustrations, but if you had been watching this channel before you had your operation I think you would have been for more clued-up and could have got a lot more questions to ask your surgeon beforehand and might have had a better outcome. Yes, the NHS has a lot of problems. I have learnt a lot from this channel, and this kind of honesty and detail is what we need to get better outcomes from our NHS. This doctor is one who does put the time in if you follow this channel, she is a hernia specialist. Thanks for sharing your experience, though this confirms a lot of my worries concerning the NHS .

      @ChristianParkesArt I definitely wish I'd done my research 1st. The really scary thing about the NHS is how they give priority to making things look right on paper rather than getting things right in practice. They give priority to cost effective repairs over patient outcomes. In my case there's been 200 medical appointments following my first surgery just to save 45 mins on a proper clinical examination. They put things off for as long as possible and end up costing themselves way more than it would have cost to get things right first time. In my case they are currently using the risk of damage to the arteries as a reason to block the mesh removal. They conveniently overlook that I'm spending 16 hours a day in bed. I was an active 37 year old at the time of the occurrence. I was doing quite a physical line of work and I was also training for a canoe race.

    I am mid 60s. I have bilateral inguinal. The right side is quite large bulge but I do not really get symptoms. The bulge does go in on lying down at night but still feels a bit 'fatty'. I have been offered surgery twice but not gone fwd. I do not have any other medical conditions other than arthritic knee/Bakers cyst. Still in 2 minds

    I just wanted to thank you for all of your videos. 7 months ago I had a bilingual inguinal repair with mesh, an umbilical repair with sutures and a surprise hidden 6 cm incisional repair as well. I have a lot of other issues in the same area- IBS, IC, hip labral tear and Hypermobile EDS so I've done a lot of research on hernias and what to consider with all of the other above issues and your videos have been so helpful. Now I'm back watching because of a recurrence on one of the inguinal hernias that I need to have repaired. Your approach is very informed, practical and often very positive. Thank you!

    Stay away from mesh once it scares down through the membrane layer the doctors lie and tell you nothing is wrong while these evil things are ripping your guts out !! This part of the Healthcare industry must be investigated! All the doctors lie to protect the implant makers at the expense of the patient!! I'm asking every senator and congressman including president Trump to investigate this rico style cover-up of the of the implant industry!! This is not Healthcare it's murder for hire by the doctors!!!! STAY AWAY FROM THE MESH !!!!!

    Mesh should be banned from all hernia surgery!! When it goes bad in scars down through the membrane layer and starts attaching to your organs and bowel,the doctors wouldn't help and lied to me repeatedly while refusing show scans and deal with the scared down and bleeding implants!!! This was a death sentence until I took control of my Healthcare!!! The doctors always coverup for the implant makers!!! Our lawmakers need to investigate this atrocity!!!

    Hi Dr. I had laparoscopic left inguinal surgery repair done four weeks ago with mesh. The entire post surgery pain was immense, but improved a lot with pain killers about a week post op. The left testicle remains a cause of my concern over four weeks post op the pain is still lingering though less in intensity to 1/25/25. On my two weeks post op follow up visit, I told the surgeon about the pain and he told me that it can happen post op and it will get better. He asked me if I've had sex. I said no because it's painful and I had no erection since surgery. He suggested that I wear support and I did since then. The pain subsided somewhat, but still happening and my left testicles hurts a lot if pressed on. I also have numbing pain in my thigh, pins and needle pain in the groin back of my knee and calf. Ibuprofen helps somewhat, but never eradicate the pain which returns fully after a few hours. After calling and texting the surgeon to complain about the problems, he ordered a ultrasound of the scrotum, which I had done yesterday. The reading showed that I have varicocele in left testicle. Any suggestions on what next? Based on the ultrasound, the surgeon sounds happy that was the only thing I had going on post op. He hadn't seen me since the ultrasound , the report was emailed to him. I should mention that I also had umbilical hernia repair with mesh done at the same time. Other than some lingering aches in the area, the umbilical area seem to be recovering well thus far. I wish I had done more research before having this surgery, I would have found your channel then and asked more questions. I went into the procedure with trust of the surgeon because I worked with him for some years in the medical field.

    A sigmoid scope 3 years ago revealed that I have a 16 yo mesh, broken that has perforated my sigmoid colon. Part of it was partially blocking my colon but was dislodged by an different, earlier scope. The latest scope, 2 years ago, revealed that the mesh was balled up, fully perforating my colon and the opening was inflamed. I am having trouble finding a confident surgeon to remove it. Do you do online Or zoom consults?

    Could low BMI be a risk factor for recurrence? My surgeon says my recurrence could be because i am so thin... Ihave just suffered a recurrence of an inguinal hernia 7 months after repair. At the time I had 4 hernias repaired, bilateral inguinal with mesh and an incisional hernia from previous c section. I also had a non mesh umbilical repair. One piece of mesh was used to cover BOTH right inguinal and C-section incisional (6 cm) That is the side where the inguinal hernia came back. I did see a study that said high BMI and low BMI can both contribute to recurrence, but i never hear anyone mention low BMI as a factor in recurrence. Have you ever heard of this? Does this make sense? I dont smoke, don't drink alcohol, eat really healthy, exercise, hardly ever even cough and no constipation.

    I had a hernia mesh repair in 2009. I now have lower back pain, joint pain, and itchy back. I heard that the microplastics build up around the kidneys. Is there a way to detox from the mesh daily with a certain binder or does it have to be removed?

    Hello I had a lower groin lnguinal hernia open surgery stitches no mesh 10 weeks now but I'm a little bit worried for example if I passed wind I can feel a little bit of pressure there sometimes and I've done a bit of cleaning around the house today just a little bit of very low pain I've been doing my exercises and my physio just said I can start doing light upper body exercises I'm just wondering if that feeling when I passed wind is that normal I didn't have any pain before my operation and there is no lump anymore thank you

    I had laparotomy due to neuroendocrine pancreatic tumor, and in the aftermath, becaus my connective tissue is brittle, and there were weak points in my bellybutton due to a laparoscopic surgery two months prior to the pancreas surgery, I had a hernia in my bellybutton which grew in size faster than anything. So 3 months after the pancreas surgery, I hada hernia repair mesh implanted. The whole scar region had multiple hernias like a row of pearls side by side. They implanted the mesh in diagonal, because it was too small. I still seem to be bothered by the corners of said mesh and I had fluid buildup after the surgery, but was told, it will sort itself out with time. It still hurts and prevents full sideways movement in twisting my torso like in some yoga poses. How do you decide, who is a candidate for mesh removal, and is the risk of new hernias higher after the mesh removal?

    I had a golf ball sized inguinal repaired at Shouldice 7 weeks ago and had very little pain before or after surgury. The care at hospital was fantastic. I've been walking regularly since surgury and do modest stretching. I am now getting some dull pain in my testicle and intermittently above where the surgury was done in the same abdominal muscle. I'm not too concerned about the muscle, as i'm hoping that will sort itself out. The repair seems really nice and tight and scar tissue ridge is now starting to soften. The testicle pain is dull and intermittent so again I'm monitorng it but hoping it goes away. My bigger concern is the neuropathy I have in the entire area on the leg side of the 4-inch incision, right down to the fermoral crease. Totally numb and at first the neuropathy was very uncomfortable, just like with shingles, which I had 25 years ago (I'm 61). It hurt to even wear underwear for about 3 weeks. That severe discomfort has dissapated but the continued total numbness in a roughly 5 inch by 3 inch area is worrying me, plus the dull pains. I read from Shouldice that this condition is not totally unusual and can last for several months as nerves re-grow, but other men I know who had the surgury had none of this. I was not informed by the surgeon that there was nerve damage or thtat they had to do nerve shifting or repair. When should I start to be concerned that my damage will be permanent? I fear I am going to be one of these 1-3% who have chronic nerve problems. I welcome any feedback.

Totally numb and at first the neuropathy was very uncomfortable, just like with shingles, which I had 25 years ago (I'm 61). It hurt to even wear underwear for about 3 weeks. That severe discomfort has dissapated but the continues total numbness in a roughly 5 inch by 3 inch area is worrying me, plus the dull pains. I read from Shouldice that this condition is not totally unusual and can last for several months as nerves re-grow, but other men I know who had the surgury had none of this. I was not informed by the surgeon that there was nerve damage or thtat they had to do nerve shifting or repair. When should I start to be concerned that my damage will be permanent? I fear I am going to be one of these 1-3% who have chronic nerve problems. I welcome any feedback.

    Please Dr.Towfigh, our son is 40 years old, after a varicocelectomy 3 years ago, he has had constant groin pain. He went to countless pain management doctors, had dozens of shots to try to locate which nerves are the problem to no effect. The pain seems to be spreading to the back and he is screaming for a solution. We were thinking denervation would help but now we're scared of side effects. Please, where do we go from here?

      Depends on the reason. If it’s a reversible reason, yes. But I can’t think of what that could be except for major heart or organ damage improvement

    What is the risk of reqerance increas e with out the Cremester muscles being cut. Would be good to know the risk reward ratio of cuting these muscles.

      I believe the risk of recurrence was reduced by about 1% or in that range. I personally don’t think it’s worth it as patients do get some testicular pain, pulling, discomfort from this. So I don’t cut the cremasteric muscle.

    I had a hernia repair (left side inguinal, open, no mesh) about 25 years ago. Zero issues with the open proceedure. Now I've popped a gasket on the right side (inguinal again) and am finding it difficult to find a surgeon that will fix it without mesh. Mesh implant is a deal breaker for me. Boise, Idaho

      Difficult to find such a surgeon because it would be the incorrect choice of surgery. Redoing a tissue based repair with another tissue based repair will increase risk of recurrence and chronic pain and thus is not considered standard.

    Happy new year to you! I would like to know your feelings on a open repair on a Inguinal hernia? I am 64 soon to be 65 years old in good health with a BMI of 19.4. I am considering all options, but I went to see someone who said due to previous abdominal surgeries due to endometriosis, etc.. He thought it would be better to go with an open repair with no mesh.

      Based on being female and thin (BMI 19), I would trend toward a non-mesh repair if your lifestyle, tissues, and health are amenable to a tissue repair.

    I learned a lot on Dr. Towfigh's channel before my own robotic double inguinal hernia repair with two polyester patches four years ago and was able to actually have conversations with the surgeon both prior to and after the surgery! Since then, I am on the carnivore diet and NEVER get gas and bloating anymore and avoid eating plants of any kind. I eat butter, beef, bacon, eggs and occasionally chicken. Now, I wonder if gastric pressure from bloating due to eating plants and having constipation from it is what caused my hernias in the first place. Channels for carnivore are Old Guy Carnivore, Kent Carnivore, My Zero Carb Live with Kelly Hogan, Dr. Anthony Chaffee, Dr. Ken D. Berry.

    Dr. Towfigh, Within many articles I have read that after an inguinal hernia repair fails and a surgery on the recurrent hernia is completed, it is much more likely to fail once again. Would a return to intense strength sports be possible for an individual who receives a surgery for a recurrent inguinal hernia? (Powerlifting Training specifically) Would this individual have to eliminate resistance training and/or intense exercise for life? I fear that after the first surgery fails, an individual is simply fragile for life. Thank you for your genuine advice and wealth of knowledge.

      You are right that once you have a hernia, that disrupts the core and it’s never like normal again even after repair. That said, exercise alone, including powerlifting, should not be the cause of hernias. There may be a genetics issue underlying any hernias. I usually encourage the exercises, including lifting. Know that in all my years, I have treated only 1 wrestler (WWF) and no power lifters. So, that usually means they are not the ones getting hernias.

    You never get straight answers from medics. Millions of patients are wrongly diagnosed every year and some die as a result. You also need to remember that medics and drug companies are in bed together for profit, as are manufacturers of surgical mesh and other devices/implants etc.

    Hi Dr Shirin Towfigh, I am just 4 weeks out of mesh removal surgery. Still some persistent testicular ache, and some pain on ejaculation. Could it be because surgery was unduly delayed? Workers compensation refused to pay, and this has been dragged out a long time, not just the ten months since I have filed the claim, but years actually since I have found doctors in Australia who can recognise and treat the condition. I believe that delay itself has caused significant further injury.

      Delay in surgery usually does not affect outcome for mesh removal surgery. The surgery has complications as any revisions surgery may have. You will need more work to get toward a pain free situation. I believe I discussed this in detail on 01/07/2025 episode

Spermatic cord entrapment has progressed gradually, and as I understand there is continual foreign body response and this creates increasing contraction over time, so the symptoms would become worse and worse from delay over time, as I understand it. Complete medical denial has been ongoing for many years for me. I knew things were wrong immediately after surgery, 24 years ago.
