Hormones, HRT, and Sleep Quality: How you can sleep better! | Felice Gersh, MD
Menopause is a sleep crusher for many women. In this talk, I explain the relationship between estradiol, progesterone, and sleep, and why HRT can help women in menopause sleep.
And I'll also discuss research that I've been looking at that calls into question the current protocols surrounding oral nighttime progesterone.
If you struggle with sleep, you'll want to listen to this talk.
Join me to explore the complex relationships between menopause, hormones, HRT, and sleep.

Wow, this is a very informative video and explains why I've been having frequent trips to the restroom at night! My testosterone levels are elevated & I was wondering if ther may be a connection. This is we must continue to do our research & not totally rely on doctors. Thank you Dr. Gersh!
So interesting. My lining thickened on estrodial and I was bleeding so my doctor told me to go off. Why isn’t there any standard yet!!!!???
My doctor refused to give me and even scared me. He also said he doesn’t recommend blood test since it’s unpredictable daily and can’t be relied on. I am so tempted to take the 5mg also. I am currently on estradiol patch at 0.0375 dose.
Is it ok to be on estradiol patch at 0.0375 and dhea at 5mg? I have symptoms of a low testosterone and I am constantly waking up early too.
Can you offer virtual appointments? I contacted your office, but no response.
DrGersh, I’d really love to talk to you,not for personal issues, but to benefit all women concerned… I believe I have something to contribute 🥰
I’ve been on 100 mg of progesterone and was just given 25 mg of estrodial to start. Will I be a test monkey too?
Love your videos!!! Thank you ❤
Felice needs to be on a podcast with MENOPAUSAL and POSTMENOPAUSAL females, and not with this one here. 😏
Absolutely antibiotics are the wors!! Dr Burke is talking about a new science proving /we don't have to finish the whole thing+ it's totally killing our good bacteria 🦠 it does not come back if you don't work on it.
I have a great solution for that! I took oral micronized bioidentical progesterone (100 mg) for two months and felt pretty bad. Then I switched to compounded TROCHES of the same thing, that I place between my gum and cheek and slowly absorbs directly through the mouth tissues over about 30 minutes. I have no side effects what-so-ever with the troche’s mode of delivery, and my blood levels of progesterone are much higher than they were with the oral capsules. So much so that we decreased to 50 mg instead of 100 mg with the troches. Best of all, no side effects.
So sorry, what's the torch and which brand. I am in Canada and was taking prometrium 100, for 6 weeks, did not help for sleep, just lost a bunch of hair
This sounds interesting. Willaston my compounding pharmacy about it. Thanks
@@cilitagaspar2132 It's not a brand, it is made by a local compounding pharmacy using bio-identical progesterone. Any compounding pharmacy can do that, I think. Prescription needed.
How should I be taking it? I am on the progesterone pill.
Thank you for continously learning to help more women. I wish I could tolerate progesterone and HRT. I had to stop taking it.
I would love to see the research you are using for this gut microbe theory. I would also suggest that you require anyone on a high carb (vegan) diet to wear a CGM. It is estimated that 90% of Americans are on the verge of or in a state of metabolic disfunction. (Diabetes) Carbohydrates are NON ESSENTIAL. This is basic human biology, not my opinion. Humans need fats and protien and water to thrive and survive. Animal protein is the only source of protein that contains the correct ratio of amino acids to make cells, and provide building blocks for human tissue. The gut may need enzymes, the gall bladder my need help to secret bile, and the stomach may need HCL to break down food when the gut biome is compromised, But eating only carbohydrate does not fix these problems. The vegan diet DOES however, spike blood sugar at every meal, promotes nutrient deficiencies of B vitamins and the hormone also called vitamin D, which ARE found only in animal protein. These plant foods actually FEED the sugar and yeast that thrive in the gut. Every carbohydrate breaks down into sugar. Bad gut microbes feed on sugar. I cannot see how it could be possible to stop sugar dependent "bad" microbes in the gut by feeding them more sugar and ignoring animal based proteins which provide healing and cell building amino acids. Not possible.
In perimenopause was on Prometrium 200 mg days 16-25. It made he so dizzy then I would sleep for several hours and wake up around 3 AM and couldn't sleep. Quit taking it and then I developed new onset anxiety. Do you do phone consults for out of state?
You kill more animals to sustain your vegan diet than we meat eaters do…. Go away
Can confirm, bio progesterone saved my babies (low during pregnancy) but when I took it orally I would sleep like 11, 12 hours combined per day. When I switched to vaginal it was a huge relief and I felt normal again.
What would be a good alternative? What about an IUD that contains progesterone? Why can't they make a progesterone patch like estradiol?
Mine is a troche I dissolve in between my cheek and gum so it’s absorbed via the skin. Works well.
Looking forward to the next episode 🩷 TY so much Dr Gersh!!!
I am on estradiol patch and 100mg OMP daily. Since OMP does a first pass through the liver and then converts to Allopregnanalone, does it still work to protect the endometrium effectively? I get no "sedative" qualities or benefits from the OMP, it makes me bloat and have digestive issues, actually. Would a transdermal "progestin" be more beneficial to protect the endometrium?
So it’s not protecting your Uterus properly if you’re peri menopause?
@myssilightful Or if you're in menopause with a uterus ?
Very interesting with respect to Estradiol helping with sleep. I currently split my estrogel application between night and morning because I was told it could keep you awake. Maybe I should move my full dose to night?
I wonder that as well. I take mine in morning...but split dose would be worth a try
Thank you. Looking forward to part 2 of this discussion 🙂
Yes look forward to hearing the solution. I cut my progesterone to 50mg, cyclical. Continuous 100mg felt like an overdose with body aches and weight gain.
How do I do it?
Quietly I look forward to the 14 days of amazing drug induced feeling sleep. But if it’s not doing anything effectively except making me sleep I sure want to hear the alternative.
Do you only take it 14 days of the month? Do you do it at the beginning or the end? I take it every day. That’s what’s my naturopath has me doing.
@-living4jesus4ever- I feel much better since started taking it only in the last week of my 25 day cycle
@@-living4jesus4ever- I only take it 14 days the way Dr. Gersh recommends for physiologic results. Most people do not want to "bleed" again, and so doctors prescribe the continuous progesterone. I do have to warn you, you will begin bleeding again, but it's not bad.
I use NOW progesterone skin cream (at night) & sleep better thru the night with more bladder control.
ABSOLUTELY AGREE. I just started BHRT: estradiol Patch w/ oral progesterone 125 mg 6x/week, and I feel drugged the next day.
Are you doing progesterone oral, topical or ?
Go cyclical, that way you only take progesterone in last 10-13 days of cycle.
Hi Doc! very informative howver can you tell us what do you recommend as you said you like to help women? please elaborate & finish your thoughts on this matter as I'm taking oral "Bhrt" progesterone pill 💊 at night which helps me go to sleep and that was my biggest issue for menopause. I didn't have hot flash
Wow she is amazing love to hear. She has so much information and easy to understand.😊
Can you just use the progesterone pill vaginally? It will dissolve and be absorbed?
I've tried oral & vaginally - simply putting compounded cream on my forearms ten days of the month is enough for the physiological effect of light period when combined with .025 Estrodile patch constantly.
Please could you discuss alternative methods of administration!
I absolutely agree, which is why I refuse to take pill progesterone. Devil in the details.
How do you take it then?
I’ve been taking progesterone 400 mg capsules at night. Your right. It turns into pregdelone (misspelled). I’m going to use one capsule vaginally to see if that helps. Im already on pellets (estrogen & testosterone) and still cannot sleep. I’ve been taking 300 mg of gabapentin to help me sleep at night. Im on a 50 mg estradiol pellet. I will ask him to increase it. Subscribed!!
Oh WoW ! Great new information. Thank you, thank you thank you Dr. Gersh. I was not doing well on progesterone. I can't wait for more information on this. I'm looking for a different way to take it but because of what it does to me, moody, bitchy, weepy. I am off of HRT for now.
You can also take it via a skin cream. That is what I do.
@@ags1370 Yes, thank you !
You can take it vaginally or rectally. There is a French study that showed it absorbs well this way.
I just saw your video about how oral progesterone gets turned into pregnenolone and not much progesterone is left. So how is there enough left to protect the uterus and other organs? Using it as a suppository gets much better absorption and bypasses the liver so stays as progesterone. No depression or sleepy side effects either. Why don’t you recommend that method?
Is there more to this video? You don’t go into what method is better. Or maybe i missed it?
She never gives the solution
@ yes. It’s pretty annoying.
Can you also address the needs for women in clinical menopause and the dosing they need as uterus, ovaries are removed? 😮
What if you had a radical hysterectomy? Do you need progesterone? I’ve been told, no.
i’ve heard that progest-e sublingually is better absorbed as progesterone (without MCT oil). many products on the market do have MCT in them, so it would be great to verify which products work best.
Yes! I’m in a hormone group on fb and they highly recommend vaginal or rectal progesterone. Absorbs so much better.
What about the one I have that is in liquid form with Vit E from Forefront health that you apply to your cheeks and gums?
Does progesterone give anyone else a heartburn feeling when they take it?
I found taking progesterone orally did help me sleep but also made me feel sluggish and low in mood. When I switched to using it vaginally, I felt a lot better, even though my sleep is not as good now.
Try taking a little gaba and or L-theanine for the sleep part
I don’t think you ever tell us what optimal lab values are for estradiol. Could you please discuss that?
Is 0.025 estradiol and 100mg oral micronized progesterone considered supplemental in a peri woman? Does the dose of progesterone increase to 200mg if a cyclic method is used with daily estradiol patch?
What about women who are in their 70s and 80s? My poor mom can't stay asleep for more than 4 hrs. What can they do if they have not been on HRT? Can they start it?
Is Qlaira with natural estradiol and dienogest (progestin) ok to take
This is gold
You know the concept of the “carrot or the stick”, right? So, I got curious to see What IF I kind of neglected taking care of my liver and decided not to go through the effort and discipline of a vegan liver detox. What could possibly happen? I heard that the liver can regenerate, at least in some cases. What does the liver have to go through to recover? th-cam.com/video/i9b5jHwEkHw/w-d-xo.htmlsi=FP2Ib4V5FPexId5p Here is a presentation by a scientist discussing her research trying to understand this process. I listened to it 3 times because it’s technical (for me). The bottom line is the carrot 🥕 is the better choice to me - much better than the stick/ Surgeons scalpel. Good luck. 💜
