    My science brain LOVES her detailed explanations since it adds to what my hormone doctor has educated me these past 9 years. My inquisitive nature paired with BHRT has enabled me to feel wonderful during my different phases…Perimenopause, Menopause, and Post Menopause.

    Thank you for sharing such important information. This video got me thinking. Dr. Lisa Mosconi showed how a woman's brain changes during perimenopause and menopause. Many areas of the brain shrink. This is due to estrogen deficiency. We get sleep deprivation and other neurological disorders. Even if we start HRT during menopause and eliminate many unpleasant symptoms, sleep disorders remain because our brain is already damaged. I would be happy if there was a study that showed whether hormone supplementation in the early perimenopause can protect our brain and prevent sleep disorders.

      Early perimenapause the progesterone hormone is much lower and it messes with alot of things , especially brain fog and so on I have and still am researching this , testrone gets low too like estrogen and up and down smh

    Dr Gersh, I had the privilege of listening to you at A4M last year! I always appreciate your wisdom, but it makes me angry & sad that media is now infiltrated with doctors ONLY disseminating information that has been approved by the North American Menopause Society, who is completely opposed to complimentary medicine, including compounded BHRT & botanicals. One doctor who spoke at A4M this year calls them the OBGYN Mafia 😂. We now know that hormones can be very low and fluctuate greatly for 10 years BEFORE menopause, but most doctors don’t treat peri- menopause and you’re lucky if you can find one to treat menopause. Keep up the great work being a voice for women’s health ❤

    My periods became closer together with lighter than usual bleeding about a year ago. I started sequential HRT a couple of days back after suffering for 3 years where my GP kept bouncing me off the cardiologist, neurologist, hepatologist, psychiatrist etc. I then decided to see a menopause specialist.
    Dr. Gersh, your videos are a lifeline for 40+ old women. My obgyn wanted to start me on OCP but I had been your TH-cam student so being in a metabolic syndrome with fatty liver, I convinced her to do hormonal supplementation with 0.05 E2 patch and 200mg of microionized progesterone for second half. This is my second day of the patch. Just feeling some cramping. Thanks for being a guiding light for all of your viewers. I am a GP who quit her job due to cognitive and musculoskeletal systems of perimenopause 2 years back. My favorite pastime is to watch your videos. God bless you! Very very few are as intelligent as you are. I have a question - can you bleed outside of the withdrawal bleeding at the end of 14 day progesterone? You saved me. I was going crazy.

      Hi there, thanks for sharing. I am NP who was in similar shoes....I decided to just do daily estriol cream PV...there was no info on that but I was able to stop the crazy bleeds and crazy fluctuations. I wanted to let you know that there is a smaller dose estrodial patch available. It's intended for osteoporosis but I think its going to be my next step...its 0.014 dose I believe....and as for progesterone I can't take it orally and side effects to cream but it works vaginally...I know it does because I measured my lining on it and with top only...I just wanted you to know that you are not alone and my symptoms have been extremely severe enough for me to stop clinical practice. May be we should open online biz helping women lol. Best wishes.

      That's Ethanol Estradiol and a Progestin. Its essentially a hormone disruptor. Women shouldn't take that pill. She isn't on about the pill. She promotes Estradiol and Progesterone. And cycling. You also need to be on Magnesium Glycinate at night. Some say Magnesium L-Theonate helps with moods.

    I'm only 35 but have been experiencing worsening menstrual migraines, night sweats every night, a significant loss of libido and even some sensation "down there". I'm otherwise healthy, exercise regularly (but nothing extreme), and eat healthy. This is very frustrating as I always had a healthy sex drive my whole adult life, but there has been a definite decline in the past 5 years with it now being so bad it seems that I am not interested at all except for the day I'm ovulating. I also wake up every morning drenched in sweat, which is so annoying if I want to take a shower in the afternoon or evening to save time the next morning because I end up just feeling gross! I have also noticed my cycles are getting shorter. I have had a longer cycle all of my life (closer to 35 days, and pretty regular), and it is now down to 28-31 days. The shorter cycle doesn't bother me too much, but it is just a change I noticed alongside these other, more annoying changes. Wish my gyno had a better solution than birth control! I haven't been on bc for most of my adult life because I don't handle it well, but I really resonate with the idea of supplementing with low doses to just make up for whatever deficiency is going on.

      I’m 39 now but I definitely noticed a decline in my hormonal levels impacting my mood, sex drive, ability to metabolize alcohol, sleep, anxiety, joints, ability to heal from sickness and injury. Just CRAZY.

      You definitely have premature ovarian insufficiency. Unless its hereditary from your mother. You need supplement.

      Please look up GERD and Harvard research on the subject 2024. You may need to reconsider whether it is worth developing a serious disease….and the risk is substantial.

    Is 0.025 estradiol and 100mg oral micronized progesterone considered supplemental in a peri woman? Does the dose of progesterone increase to 200mg if a cyclic method is used with daily estradiol patch?

    i went on low dose lo loestrin at 43 for heavy cycles and critical anemia... i don't regret it-saved my life... now i'm on lower HRT and having more symptoms. i have to find a dr who will give me better hormones like less synthetic.. my testosterone came right back fast off the pill -- free t is 4.8 can't believe it at 50! I'm exhausted thought i was low t.

      My journey sounds very similar to yours, except the testosterone -- mine tanked and stayed there. I am taking the highest dose patch (.1mg) but it's not enough. Finding a provider(who takes insurance) that will prescribe more has been a challenge.

    Hi Dr. Gersh, I’ve been following you ever since your youtube video pops up through algorithm. Amazing your knowledge and appreciated for wholesome education. Just one thing I wanted to ask you is how about synthetic hormones tablets ? Can see your directions for bioidentical hormones therapy for best benefit. But here in Hong Kong, it seems not so common. I am in perimenopause with symptoms then Gyn dr. prescribed birth control pills and after 3months I asked to switch to hormones replacement pills because my expectations is beyond just hot falshes or sweating as I have RA on my knee and everywhere joint/muscle pain. What I want to learn is what is your perspective on synthetic hormones replacement vs bioidentical hormones. Because I can’t find any videos you are talking about it. Can you help to make videos on synthetic hormones replacement please ? Much appreciated !

    What if your hormones still cycle but not bleed ( as the E is just not high enough)? What if those cycles are just too short to supplement them into ( to frequent) bleeding?

    I just started progesterone 100 milligrams and an estrogen patch, the lowest, and I am in perimmenopause. I have noticed that by day 10 I am extremely gassy and bloated, so I will stop the progesterone, and then restart again in 2 weeks. Is there anything that one can take for the gas and bloating and or water retention? Please 🙏 🙏

    Why not oral micronized progesterone for heavy bleeding instead of the progestin birth control? What is the advantage of the birth control over taking progesterone?
    Is heavy bleeding a contraindication for estradiol supplementation?

      Only speaking from personal experience, I’m 47 and after suffering with irregular, very heavy periods (unable to leave the house heavy) for the last couple of years, I then bled for a whole month! I decided enough was enough. My GP prescribed HRT with transdermal estrogen gel every day and oral micronised progesterone taken 2 wks on 2wks off.
      I started off very low, (1/2 a pump) with the estrogen gel, to see how my period was affected.
      I’m now 3mths in and I’ve managed to get up to 1 1/2 pumps of gel per day. I feel like it has transformed my life. So far my periods are a lot less heavy and don’t last as long. I can now leave the house on day 2 & 3 of my period which I haven’t done for years!
      Not to mention the improvement in my mental health and energy. Wish I’d tried it a couple of years ago.

      Sharing from personal experience I've taken Progestin (Norethindrone Acetate) and it stopped ovulation and kept the uterine lining from building up thick so that I didn't bleed heavily. I've also tried Micronized Progesterone but it didn't stop ovulation and I bled super heavily like I always do.

      @@elizabethgood3383I had worried that adding estrogen would make bleeding worse, but I read somewhere that a low dose can prevent the pituitary gland signaling to the ovaries to make high surges of estrogen that are common in perimenopause. I don’t know if this is based on any scientific evidence. It may have been coincidence, but I noticed my periods have been better regulated since I started using 1 pump of estrogen gel daily, compared to when I just took progesterone alone for a month. Only downside is they have been more painful on hrt.

    HRT has significant risk of causing GERD - both estrogen & progesterone individually! This should be talked about….

      @@mellove389 maybe try the two hormones individually for a month or so to see if one is more a problem than the other then go from there? I might not use hrt for this reason despite the other benefits. If you’re interested, you can look up the Harvard study done this year on it for more info.

    Though I have a healthy heart and no history of them, I've started having frequent palpitations. Before they began, my dr switched me from estradiol cream to sublingual, and I think that's the culprit. 100 mg of progesterone has changed my life, and I dont want to stop that or the estradiol. Has anyone had this issue? If so, what was the answer for you?

      Don’t take estrogen orally .. you are correct , you can take it as a cream and won’t have the palpitations

    Dr Gersh, for women having heavy bleeding or more bleed days, can progesterone, even 100mg, not help reduce the heaviness? It seems to be doing that for me. Or is that Mother Nature on her own? Btw the bloating from the progesterone is a bummer. Estradiol, 2 sprays of Evamist a day, is a functioning miracle for me. I can focus on work, less intense food cravings, less irritability, feel more like myself. Sleep still is a random struggle some nights. Also, I thought the FDA concluded that drospirenone-containing birth control pills have a higher risk for blood clots ?😮

      I've started with progesterone 100 in capsules in days 15-25 of the cycle. And in a month added estradiol (Estrogel) 1 pump. Period after that was not so heavy as they were this year. I am glad ))

      I use oral micronised progesterone 2x 100mg for 2 wks a month along with a low dose of estradiol gel and I’ve found it has helped a huge amount with lessening my issues with very heavy and frequent bleeding.

    Hi Doc
    I am not able to take estrogen supplements as i had erathemya nendosa / Blood clots w the ortho 777 pill.
    So at 53 i have some estrogen markers but very low progresteron, my gyno doc suggested progesterone supplements?
    Can u advise if that is safe to take for my sensitivity? I am still suffering some issues with brain fog pain and fatigue . No bleeding for 7 months

      You may want to ask your doctor about the transdermal estrogen patches. The blood clot risk is associated with taking Estradiol orally since it has to pass through the liver, but estradiol patches do not have blood clot risk.
