Acne: from teen years to menopause and beyond | The Dr Louise Newson Podcast
This week on the podcast with Dr Louise, Consultant Dermatologist Dr Sajjad Rajpar returns to talk about acne and its effect on women during the perimenopause and menopause, including on their self-confidence and mental health.
Dr Saj explains the causes of acne and the different treatments available, including an encouraging new treatment that could make a big difference to acne sufferers in the long term.
Finally, Dr Saj shares his three tips for anyone who might have acne:
Keep skincare simple and targeted. Pick one of three active ingredients - retinoids, benzoyl peroxide or azelaic acid - and give it three to six months to work. Only supplement it with a very simple supporting skincare regime.
Assess the impact of the acne - if it is leaving persistent blemishes, pigmentation or scars, then escalate the treatment. If the first line treatment hasn't worked, give it three months and see a medical professional to try to get it under control because prevention of scars is so much easier than trying to reverse and cure them.
Be mindful about all the different risk factors and triggers, and do not underestimate the impact of diet and stress. Where we can treat acne holistically, we should.

    I had horrible aching feet, varying joint pain (very active/very fit) & the hot flashes during night, etc but the strangest was constant dry, peeling chapped lips. Every day the skin would flake & I would pick off skin & repeat. To the point I saw my dermatologist when I was peri menopausal & she never thought about hormones. Once I officially went through (suffered greatly for long enough) menopause & started HRT all of these things reversed. I had tons of other issues that were very life changing which HRT completely turned around. HRT is LIFE-GIVING

    I do have adnomiyosis so bioidentical P. helped me or the fact that i didn´t had ovulation, because i not only stopped my continuasly bleeding, it also made my menstrual bleedign very light. But it musst have been 3 monthss ago when i stared ovulating again, i didnt thought so because i still had smear bleeding which in my eyes showed the absence of P. or in other words no ovulation) but as i proceeded with taking bioidentical p. (cyclical) i got the worsst pmdd Symptoms, i felt depressd, outburst of anger, hopelessness and then su*cidal thoughts. It was a cocktail that switched within a few minutes from one extreme to the other. So this month i didn´t take any P. went to the doctor and actually she saw that i´ve ovulated this month, i was shortly bafor my period. Now that i´ve had my period i know that i didn´t had the pmdd suicidal symptom cocktail. So it wass probaply a too much of progesterone from the intake and my own unknow ovulation which made me felt this way the months before. Unfortunately my doctor isnt very understandig she just told me i ha to take the gestagen pill and was very frustrated as i said i had to think about taking a pill. PLEASE don´t let this Question go unnoticed. Should i really take a chemical gestagen Pill, when i didn´t had ovulation for months,just got it back and now supress my ovulation? I´m scared after taking the pill someday i can´t get pregnant anymore, i´m already 31. Or is it realy neccessary for my mild Adenomiyosis ?

    I have found no one with my unusual menopause symptom…over a year since starting estrogen replacement I suddenly developed a constant burning feeling in the head, face, neck, ears…and it’s not a hot flash. It’s awful, and nothing helps it. No rash, no outward signs, just an internal feeling of burning up. Also developed..after being on estrogen long time…bad dreams, horrible anxiety, paranoid, tinnitus. I can’t tolerate oral progesterone, but I suspect not taking it might be a culprit. No idea what to do.

    I'm on hrt oestragel and utragestran vaginally , I'm still awaiting for coil removal and replacement, I've also got pms which gives me a progesterone intolerance, I've also have underactive active thyroid, I had my blood s done 3 was ago, testosterone normal and estrogen normal ! My thyroid was low and had to increase my thyroxine. I suffer from severe sleep anxiety and parastiia, itchy crawly skin and pins needles crawley skin like insects crawley abd ckammy skin and cold flushesand sleep apnea! I've had problems with my eyes in and out eye clinic s from tearing and dry eyes, bleeding gums dry mouth, uti. Rang Dr as my symptoms are becoming more debilitating! They were only offering antidepressants! I told them that not my issue ! I said antidepressants fisrnt angry with me! They kept pushing them onto me, I refused and didn't get anywhere with the dr😢❤

    I have had acne for many years… ( Have always had a very healthy diet, most veg/fruit home grown)I was told by my dermatologist it was likely to be hormonal. I said well now that I’m menopausal should it go away… No he said, it’s likely to get worse !! I have tried all topical treatments, have had two causes of Roaccutane … and then a maintenance cause of Roaccutane… It’s a really tough drug to be on… I was then placed on Lymecycline prophylactically for over 12 years !!! What on earth did this do to my gut biome ??? Then, I can across Intermittent Fasting ……….. WOW, I have NEVER looked back… my skin completely cleared up… Who knew, I still eat a healthy, plant based diet … I just restrict the time I eat… So I eat between 2pm - 8pm … I have been off all my acne medication for 3 years now… I fully appreciate One Size DOES NOT fit ALL !!! Yes, I do get an occasional spot… It’s small, heals very quickly and doesn’t bother me at all … Just started HRT, at 66 years old, keeping everything crossed it doesn't trigger my acne 🤞 Worth remembering, correlation does not = causation and IF is not for everyone...

    I’m 10 years post menopause and finally on HRT after listening to your podcasts so thank you. One thing I have noticed is that some of the symptoms you mentioned such as palpitations, dizziness, tinnitus, noise fright seem to be more prevalent since I started HRT. Is this due to the change in hormones and should it start to improve with time?

This was fully investigated including several MRIs of the brain to rule out multiple sclerosis. On reflection I wonder if sudden drop in oestrogen levels could cause inflammation in optic nerve as an unusual side effect of menopause?

      Vaginal oestrogen or intrarosa (prasterone = DHEA which converts to oestrogen and testosterone) and topical testosterone I’m guessing. Assuming they’re on an aromatase inhibitor. Maybe progesterone too? I think Louise has done specific videos re cancer patients, try searching for those.

    Yes I struggle with the body odor change in my armpits. Still looking for a deodorant to help. I used to be able to not wear any and be fine, but not now😮. Sore tongue…yes. Ringing in my ear drives me nuts. Even saw and audiologist and ENT about it. Anxiety sucks and I want it to go away. Never had it before menopause and it’s awful. I do struggle with the lightheaded a lot. Seen a lot of specialist to rule stuff out. Ugh menopause. Still trying to find the right patch dose for me.

      I had that and more and was contemplating quiting nursing due to terrible brain fog, anxiety, depression, fatique and more. But after 10 months of not being believed by the GP I got my breakthrough. I spoke to male Doc at the practice after finding out Dr Newson on social media. I was armed with info. He knew exactly what I was going through, got me to see the menopause specialist nurse and now I am on high dose Estrogen, progesterone and Testosterone. My life has changed drastically. I have been on it for over a year and I thank God daily. Get your Estrogen doses right and the rest and they will help. All the best.

      They have done this for me. Started me on 0.025mg patch for 5 weeks, then 0.0375mg for 6 weeks and now on 0.05mg for 6 weeks and meet with doctor again this week. We started me slow because my estradiol had been <5 for over three years and we needed to ease my body back into it. Hope this helps

    So thankful to hear you going into such detail about this topic. I started having palpitations a couple of years ago and I just learned it’s likely when my estrogen dips. My cardiology report, nothing was found wrong with with my heart, and so no treatment . None of the doctors normalized it as part of perimenopause. Not even the office that gave me progesterone and testosterone.

    Very excellent presentation. I am a physician in the USA and prescribe hormones frequently for women perimenopause and post menopause and amazed that many doctors including gynecologists don’t do this. For the post menopausal woman hormone replacement is critically important.

    Why does the beast cancer rate on HRT go up after age 51? I also wonder, if women’s testosterone levels naturally start going down in our 30’s, could it be there needs to be a balance with testosterone added into treatment correctly?     There should be more views and comments for this episode. As a woman with severe asthma I am experiencing a raised eyebrow about my wanting bhrt during perimenopause. Even from online prescribers here in the US. I don’t understand it. I am actually concerned how hormone loss and hormone treatment will impact my asthma stability and disability moving forward. For women with some type of disability or illness and wanting divorce at this time of life, the future is very uncertain. One helpful thing: LDN has helped me with inflammation substantially and I hope midlife specialists will consider LDN for midlife issues.

    What I have found is that the liver and gallbladder play a role when I used to get headaches/migraines. So I now regularly take Dr Sandra Cabot Livatone Plus, Ox Bile and Super digestive enzymes, magnesium powder and haven't had them since.

    OK great But I thought that women in UK should now be entitled to get free HRT treatment. That was after the Davina McCall campaign and I'm pretty sure in Hampshire I can request it....I asked my Dr she said let's see what happens first, you are still only 46, but that shouldn't be the excuse!

    I have had scoliosis since my teens, no surgery. Being concerned about osteoporosis have got osteopeana, as my vertibrae are compromised, I asked GP for hrt. Was discouraged, first put on biphosphonates by GP. When I visited my spinal expert they told me not to take biphosphonates because although a dexa will show improved density, bone surgeons have found that on the operating table when attaching metal screws, with those on biphosphonates the bones were crumbling!!! So have had a battle to stay on hrt, and am concerned that I might not be on the correct dose, plus I am not on testosterone?

    PROGESTERONE INTOLERANCE causes some of us these same horrific symptoms. Puts us in a VERY dark place. Please make a video to help us that cant tolerate any progesterone. I can’t even tolerate it as compounded progesterone cream for the skin 😢. HELP! We need our estradiol!!!!

    Dr, Newson 🙏Please discuss more in depth women that suffer severe PRGESTERONE INTOLERANCE 😭😭😭. We LOVE estradiol, but being forced fro take the micronized progesterone the last half of our cycle in HRT is horrific. Please! Please! Please help us. You may save lives to be honest. Progesterone has tortured us our entire lives and we may not have even know it. Taking it in HRT puts us in a very dark place. And some women that have to take it daily may not even know what’s happening.

    Idea for future video Hrt in older women. Eg 75year old ... to reduce future risk. Heard if cvd not poss. Is that the oral or it also applies to topical oestrogen? Are micronised progesterone and oestrogel safe at any age? Plus vaginal pessary. Thanks

    No disrespect, i find my dr havent got a clue about perimenopause and menopause,they always put u on antidepressants! Im on the waiting list for 2 yrs as got ongoing symptoms, unscheduled bleeding , foramication, paresthesia! Sleep anxiety, which is affecting my quality of life. Im on utragestran vaginally as ive got progestone intolerance as I've always suffered from pms and underactive thyroid and fibromayligia and ostapinia. Im on oestragel 2 morning 2 evening, im awaiting for my misplaced mireena coil removal and replacement, now im worried as ive got all my symptoms back. I did have bloods done testosterone normal estragen normal only thyroid i had to increase medication, since increasing them my menopausal symptoms got worse. ❤❤
