DNA-Based Testing for Chronic UTI: Dr. Nick Parkinson
Can DNA sequencing detect organisms contained within a biofilm or intracellular bacterial community?
In part one of our interview with Dr. Nick Parkinson from Digital Microbiology, we answer the following questions:
● Is it always recommended that a vaginal swab test be conducted alongside a urine test, for female patients?
● Can PCR or NGS tests confirm the presence of biofilms, and is it important to know whether a biofilm is present?
● Are there particular organisms that you see most frequently in cases of recurrent or persistent UTI?
● Given organisms are always present in the urinary tract, how do we know if the organisms identified are causing symptoms?
● Is it possible for known pathogens to live in the bladder microbiome and not cause symptoms?
● Is it possible for ‘good bacteria’ to cause symptoms, and have you seen evidence of this in your research?
● Is there any benefit to testing in between UTI episodes, when symptoms are not present?
● Have you done any testing on probiotics themselves, to see whether they contain what is listed on the label?

About Nick Parkinson, PhD:
Nick helped drive and develop the Digital Microbiology programme from its inception in 2018 as a direct response to his experience of NHS management of recurrent UTIs suffered by one of his family. He joined SBL in 2008 from Oxford University where he was a senior post-doc researching Spinal Muscular Atrophy. Previously he held post-doc positions researching dementia genetics at the Institute of Neurology, London, and deafness genetics at both the MRC Mammalian Genetic Unit, Harwell and the University of Washington, Seattle, USA. Nick gained his PhD in molecular genetics in 1998 at the University College and Royal Free Medical School, London and BSc degree in Genetics & Zoology at Leeds University. He’s lukewarm about football, loves golf and is a committed McLaren F1 fan.

I would like to discuss our lab's methodology that is a hybrid PCR plus Culture test, which is used to confirm viability of organisms, detect additional pathogens not targeted for on PCR and provide patient specific antibiotic sensitivity results.     Do symptoms always mean an infection that needs antibiotics? 2 cultures of proteus positive. What about when there has been the one dose of Fosfomycin and symptoms lightened but never went away then 1 moth and a half later a 2 x’s a day for 5 days of Cefuroxime taken and again symptoms lightened but didn’t clear up and ultimately returned to very uncomfortable level?

    Doctor I’ve been having symptoms for 6 weeks now I’m on my 4th antibiotic. Last week my urologist performed a PCR report that showed a moderate level of e-coli, low levels of ureaplasma and another bacteria. I’m in so much pain. Any recommendations?

    I have had 3 to 4 UTIs (all e-coli)every year for the past 3 years. I have tried all over-the-counter aids. Still get them. I am now resistant to Nitrofurantoin after 3 weeks on it and still have the infection. Just put me on Cipro and I'm a bit nervous. Complete hysterectomy over 20 years ago and take Estradiol and also use Estrogen cream. My husband died 3 years ago so there is no sex. I'm at my wit's end as I am the only caretaker for my 91-year-old mother and cannot keep getting ill. I worry that I'll become resistant to more antibiotics or have to deal with side effects as I've been prescribed so much. Any recommendations would be greatly appreciated.

    Greetings A male Ive got am uti 2016 from my partner(staph epidermidis-without simptoms on the blader just on prostate).On the start been on antibiotics.Gentamicin becme ressistant.Then ciprofloxacin.The lab results were negative after that and the urologyst told me i should not take antibiotics anymore or nobody could help me.Since then ive been on Green's.But the simptoms got worse in a way they irritating my prostate.Since that 8 years i just ejakulated normaly once.... when i was using a germany product cystinol akut(all the other times my sperm is like blocked to go outside and its really nerve wrecking),But the bacteria also got ressistant to that straight away,also to piptamin,nitrofurantion,even origano and some herb teas.Tried almost everything,and every tip and advice is welcome. Ps.Now ive been adviced to ureaplasma and mycoplasma exams

    Powdered D-mannose (2g or more per day), Serrapeptase (loosen biofilms). Also consider Ayurvedic treatments such as Utracon. Got rid of my chronic 3 yr UTI gently == no antibiotics. The urology field is making mountains out of molehills, unfortunately.

      I've tried D-mannose, Vitamin C, and probiotics and nothing works. I've not heard of serrapeptase and Utracon is not in the US (that I can find.). I need something to help. I have 3 or 4 a year and am getting antibiotic-resistant.

    My urine has smelled super foul for almost a year like Ive never experienced. Sometimes test positive for ecoli, sometimes not. Been on several rounds of different antibiotics. Saw 2 infectious disease drs incl Yale, also urogyns, emergency dept, imaging, bloodwork etc and still dont know whats wrong. Inflammatory bladder or IC is what they say. But I had IC dx years ago and the smell was not like this. Also a lot of urethra pain and painful bladder pressure. Please if you have any info I dont have lmk thx!

    I have had a chronic uti for 18 years and I'm fed of clueless urologists and being fobbed of I don't know why chronic utis don't seem to be taken seriously when they should be this condition has killed my life and I don't know if I will be pain free ever again it's horrible being in deep agonising pain everyday for 18 years it's horrific very hard in the uk to get treatment and be taken seriously in the uk

      She’s not behind in anything. Because of her thorough testing my wife was able to discover serious root issues that our other doctors had completely missed. Dr. Mandy is a God send and anyone who doesn’t think so just hasn’t had the privilege of working with her yet. 👏👏

      She has her own vision and methodology and everything is science-based and makes sense. And it works for many of her patients, as you can see. I have seen urologists Professors in embedded utis and they didnt help much in my case.

    This woman is clueless and does not understand medical conditions. Doctor of what exactly? Not a medical doctor. Mitochondrial dysfunction is the cause of basically everything? Please direct us to the peer reviewed published journal articles on this. Another sad and appalling case of Dunning Krueger in someone suffering from unfounded over confidence.

      On the contrary, many of us who have both IBS and chronic/embedded UTI noticed a huge improvement in our gut health whilst on antibiotics. It's really disingenuous to state that antibiotics destroy gut health. Even though wellness blogger types like to throw that language around, the evidence (both scientific and anecdotal) does not support that claim. Also, Erin was talking about the anti-inflammatory effects of antibiotics, not gut health. The anti-inflammatory effects of antibiotics are greatly exaggerated. Many patients who have been on both antibiotics and very strong anti-inflammatory medications such as prednisone have not seen any improvement whatsoever on prednisone, but have seen improvement on antibiotics, suggesting that inflammation is not the issue so much as infection is.

      I found your interview very interesting! I’ve had IC , chronic UTI , OAB, for over 10 years, currently under Harley Street in Uk. I have always thought that there was an immune system ‘flaw’ and genetic component to my disease especially as my father had same problems. I’m sure you have a huge waiting list if you on have only 30 ish patients per year. How long does it take for your patients to get significantly better/cured even?

      Wow your like me I'm under the lutts clinic london. Im trying to antibiotic bladder installations have got them yet though I have had chronic uti for 18 years wish I could find more people with this condition to find out how they have coped with there condition. As I think this illness is a nightmare being in excruciating pain all day everyday in the worst possible place it's life distroying

      @@orlahelendaly7905 biofilms are normal in the body and without them, you would not be alive. Your entire body consists of them. The issue is the types of bacteria that reside in them and how they invade and hijack your immune system. The immune system generally does "shed" the layer of the bladder periodically (check Dr. Malone's work), but if the contents in the urine are still hospitable for them survive, then they can recolonize. I believe an integrative approach is important.

      What you’re saying could be very dangerous for people whose bacteria has become embedded inside the bladder wall aka “intracellular parasitisation of urothelium cells.” Read “Cystitis Unmasked”by James Malone-Lee.

    My problem is that I have an indwelling catheter and urologists rarely want to treat my infections saying that I’m likely colonized with them even though I’ve had symptoms of infection, I repeatedly have several bacterial infections over 100,000 the last time they gave me a dose of gentamicin but it’s two weeks later and it’s all coming back again

      @@LiveUTIFree That would be great. At present my infectious disease physician has me on third week of Cefepime 1 gm for a pseudomonas infection that has repeatedly shown up. The catheter type I believe has created a lesion or wound inside my bladder because of the hard silicone tip in the medline catheter, when switched to all silicone Dover brand by Cardinal I believe that was lined with silver inside and outside, it seems so far to improve, but then again have been on iv antibiotics for almost three weeks. My autoimmune diseases make having repeated infections not only bothersome but dangerous to my health and threaten my life and have caused me to end up in a ventilator after my myasthenia gravies worsened and my diagram became too weak, so having physicians not treat infections become life threatening as much as any overuse of an antibiotic will. No good solution,
