Clozapine: Benefits, Reducing Side Effect Risks, and Alternatives
This is a recorded lecture. It was given at the monthly Psychosis Education Event in January, 2023. The monthly Psychosis Education Event consists of a free lecture on some mental health related topic, followed by an open-forum question/answer session with a psychiatrist and clinical psychologist. It is free of charge and open to the public. If you would like to join, send me a note via askapsychiatrist.info
Clozapine is the most effective medication for schizophrenia. It is the only medication with FDA approval for reducing suicide risk. About 30% of people with schizophrenia do not get significant benefit from standard antipsychotic medication -- yet in that subset of individuals, clozapine is successful in about 50% of cases (the success rate of other antipsychotic medications is about 7% to 14%).
This lecture discusses:
1. Why do some people with schizophrenia-like symptoms not respond to standard, first-line antipsychotic medications?
2. What are the unique advantages of clozapine?
3. What are the most common or worrisome clozapine side effects, and how can their risks be minimized?
4. Are there any alternatives to clozapine?

Calcium channel blockers for anxiety?
มุมมอง 2.6K2 ปีที่แล้ว
Many people with anxiety disorders don't respond well to standard treatments. The pace of drug discovery is slow when it comes to new treatments for anxiety. They can also be useful to prevent migraine headaches. A handful of data suggest...

Statment. I see and hear things which I know are my mind playing tricks on me. Myself? As much as I'd like to blame someone or something for these occasions, my logic runs out as I don't believe that I'm anywhere near important enough to warrant anyone or anything undermining me in such a manner.
I had a brief psychotic episode this year. I was dabbing, but I was also getting absolutely no sleep for a few weeks because of chronic back pain and I was on hydrocodone and flexiril along with the dabs. The hospital at first said it was cannabis psychosis but then after hearing how I wasn’t sleeping and was taking painkillers and highly stressed I think they kind of changed their mind and blamed the weed less. I’ve smoked weed every day for 15 years and never had a problem until now. I love weed. How can I know whether it will cause problems in the future if I smoke again? I truly believe that lack of sleep and stress and painkillers and also stopping alcohol at the same time had more to do with it than the weed did.
Indeed the hot showers are a big time out from puking and feeling sick
The vibes in the world are far too dark to take this plant lightly. That also goes for any 'medicinal or mind altering substance. We are in a perilous and fragile time, seek God instead.
I've been using cannabis for 30 years. Never had I had this happen until this year. It's only from super high potency products such as 50mg to 200mg edibles and live resin vape cartridges and rosin. Now I have to stop my favorite loving cannabis for life because of no dam research. No truth. Well now I can't buy any cannabis. Don't lose your people, warn us. God dammit😢😢😢😢😢😢😢😢
Propranolol did nothing for my medication-induced akathisia.....
Akathisia is absolute agony constantly....... crawling out of your skin is a good way to describe the absolutely least you could do to stop the feeling.... it doesn't feel like restlessness, it feels like your nerves are constantly being stimulated regardless of what you want, or if you want to relax...... it's like restless leg syndrome, if you felt it over every inch of your body...... I wish I could have found a psychiatrist that would have listened to me years ago, but just as someone else had commented, I found that psychiatrist didn't help, you're on your own.... I am so lucky I finally found a nurse practitioner that actually listens to me. Doesn't mean the problem is solved, doesn't mean everything is peachy keen now, but at least he listens to me and if I say a medication is causing issues, he will respond accordingly..... I wish I could have found someone like him when I was in my twenties or thirties....
Thank you for covering this...... they gave it to me for hives, and just told me to watch out for severe depression. I had to look this up, because my anxiety is out of control, and I'm a very very calm person but I am becoming agitated, anxious, on edge, I don't seem like myself at all..... I am ending this today, I'm so glad I looked it up now instead of continuing to take it for months or years and suffering, or worse.....
Dr Messamore, why are not both gaba and glutamate given to schizophrenia patients in lieu of or in concert with antipsychotics?
Please help me. I’ve been suffering since September 2023. I’ve been doing my research and discovered Akathisia. I think this is what I have. I’m losing my hope. I need help soon, I want to save my life. I don’t want to die, but this constant torture is too much. My body feels like it’s starting to break down from all the stress on my body. I’ve reached out to family members. Most people have turned their backs on me. They don’t understand how horrible this is. I’m afraid I’m going to die if I can’t get the help I desperately need. This condition is not fair. It’s hard to get people to believe you. 😢
I take haloperidol as well as invega paleperidone i cant spell it 25 miligrams not sure of the shot, i was wondering about talking with my doctor about taking me off of the haldol because its too much its making me feel bad or wearing thin or i feel like im going through withdraw with the haldol ive been on haldo for ten years or more and im tired of it but i like my invega and they stopped my haldol shot
Uh, "schizophrenia" is not a brain disease. It is an emotional disorder caused by environment.
Thank you for the information.
I son hears voices everyday, and he has smoked weed since he was 16 and I caught him doing meth, this is not a good combination so please don't do it,he goes nuts for about 3 days not eating And sleeping, had him hospitalized 2 times but he won't leave the pot alone. He is disabled can't do anything so sad.
Has anyone looked to see whether the connection has as it's third leg a common mutation: COMT?? I've had that theory for more than 10 yrs.
I had low-grade nausea for two months; lost weight and was about to go to doctor (no insurance or money) when a CHS story came up in my suggestions on youtube. I wasn't at the point of vomiting yet but was using weed in the morning and a bit here and there all day to fight the nausea. And it got worse. Saw the story, googled the syndrome, and stopped smoking right away. Could not eat and kept strength up with sugar water, a multvitamin, and toast. Now it has been two weeks and I am so much better! I miss weed 'cause I'm old and it helped with appetite and the aches and pains but no more for me. Even the more mild symptoms are awful. I am so lucky I saw that story.
Am sitting in the hospital suffering this condition

Im skitzo. Im 57. I dabbled in high potency weed ond off all my adult life. Probably due to the stress of mental illness i contracted psiorasis and subsequently arthritus some years later. I know full strength weed is bad for me, however one thing has led to another i am now on a medicinal programme for pain relief. I mostly take cbd oil however i have also been prescribed softgels with a small amount of thc in them. Once my reservationss were dispelled i found them not only good for additional pain relief but have found them quite good at treating my depression as well. I self monitor my thoughts and feel generally pretty good about things. There is just a trace of anxiety but its at a level where i can cope well and enjoy the stimulus which helps me better engage with my wife of 20 years and feel better in touch with my kids. Believe me, despite my marriage resembling a worn out old sock, my wife would be the first one to complain in the case of any irrational behaviour, but she doesnt. So as far as im concerned, i dont need to worry unecessarily.
When i start watching tv my agitation stops sometimes! Is the action of concentration on a screen activating something in the brain. Is the pleasure dopamine activated stimulated i wonder? So we need more dopamine or to incite what we already have ? I feel something is coursing through my bloodstream. A very weird uncomfortable feeling and my head too.
No mention of alternative medicine treatments like homeopathy which might help/alleviate.
Well let me tell you i started caplyta last month and its caused me to have a hypomanic episode right now. Doctors say I'm lying when i say antipsychotics make me hypomanic or manic yet here i am. I just ran 3 miles because of the sheer restlessness and anxiety i have right now.
Antipsychotic medications can cause a side effect called akathisia. Akathisia is a sense of restlessness. It can be accompanied by feelings of anxiety or agitation. It may be worth talking with your doctors about feeling so restless that you ran 3 miles. You may want to view my video about akathisia on this channel.
Vitamins, minerals, supplements, and prayer, and I'm alive, while 3 friends on psych meds eventually took their own lives, due to the states the years on psych meds developed their conditions into (the middle friend almost burnt down his whole apartment complex once, even though on a whole cocktail of garbage, including lithium). People, there's no such thing as a wonder pill, that's gonna "cure" you of having to put in self-effort into your own mental well-being. I think the vast majority of these tragedies are composed of such "lab rats", seeking such a miracle pill, with nobody there to inform them such an attitude is just going to ruin their health and lives. I too played along with shrinks for well over a decade, taking all of their prescribed garbage, even though it was only adding difficulties to have to deal with, but on this junk you're just so docile you play along anyway, but eventually stopped, seeing what it was doing to me, and to my friends, literally driving them nuts. They did not have anything close in the beginning, to what they had after years of psychiatric "help".
THC activates Catatonia indeed!!
Question: I experienced Tardive Akathisia twice that sent me to the hospital both times. My primary doctor diagnosed it immediately akathisia being caused by Latuda. I quit taking it immediately over a month ago and I am still feeling really weird at times and inner restlessness with agitation, insomnia and irritability. Is this still part of akathisia or something else?
I don’t have an answer, but I was told to take 800mg Magnesium Glyconate & 5 mg Melatonin. It’s not gone, it helps manage & improves insomnia.
My patient's been on Fluphenazine Decanoate injections, with Haloperidol and Carbamazepine couldn't tolerate upward dose titrations; switched to Risperidone and Carbamazepine only but both manic and psyvhotic symptoms come back with a bang and upward dose titrations are making her sleep and the metabolic side effects of weight gain. Currently trying her on Sodium Valproate 1.2g qday in 2 divided doses, Risperidone 2mg BD and a monthly shot of Depot Fluphenazine 25mg. Still not working, patient still overwhelmed by side effects.. please give me some suggestions. I really appreciate the video as it has given me evidence based info that i can use, but please practical suggestions are welcome. My pt is female 31, single and lives with her mother has no other comorbidities and is a mother of one and teacher by profession. Her diagnosis is Schizoaffective Disorder Bipolar type

I recently learned Synthroid has lactose in it as well as blue and red dyes. Aren’t these ingredients triggers for Hoshimotos? I’m lactose intolerant and have had severe diarrhea, my gorgeous hair started falling out by the handfuls and my blood pressure has been off the charts since taking 88mcg of Synthroid. All these drugs are fighting against each other causing the need for new meds for new diseases as a result. It’s way out of control.
Can Hoshimotos encephalopathy be misdiagnosed as Multiple Sclerosis?
I never knew how to describe what I ean feeling
It’s always meds meds meds, and never test test test to search for cause. If you can find the cause you could cure the disease.
Teatotalers get psychiatric diseases to. Explain that!
Another theoretical framework that has no basis in biological an etiology of the illness. It’s just a list of symptoms with unknown causes.
Demonic possession was a reliable diagnosis for millennia and it also had no basis in biology.

Long time smoker, I went to the hospital due to pain in my mid left side. They said it was gas. Took Magnesium Citrate and Miralax. Didn't really help. Fasted 72hrs from weed, felt great. Fasted 24 hrs from food, felt great. Ate food, still good. Smoked, felt TERRIBLE. Horrible side pain. It's definitely smoking, much to my absolute sadness. I don't want to quit, but I have to because the pain is very bad. I'm going to try and take off 2-4 weeks and see if that helps. I'll keep updated
My grandson’s manic schizophrenia was immediately calmed with CBD. The dose is 1ml. every 4-6 hours. CBD absolutely calms the effect of THC. My own self experimentation also found that the effects of alcohol were mitigated. I also found CBD to be an emotional analgesic.
I was misdiagnosed with having Bipolar. Total nightmare.
Euthanasia. The disease is worse than all cancers it kills the self. And instead of medical treatment and emphatic you get treated like shite by the healthcare system.
Sounds like you're talking about the kind of approach that "Open Dialoque Model" in northern Finland has been developed and used, successfully so. I've had psychosis (now schizophrenia diagnosis) for some years now, and I've been applying at least 10 different treatment modalities, many of them having some effect on my psychosis. There's a video on of my story on this on my channel, and I recommend everyone who has schizophrenia or psyhcosis to watch, and keep your own mind as your own instead of blindly swallowing all that I or anyone else, tells you. The antipsychotic drugs haven't been a good solution for me, and if it was up to me, I'd stop using them, because I do not really need them anymore and they harm my well-being more than the psychiatrist seems to realize or admit.
Can certain blood pressure meds (e.g., calcium channel blockers) cause this?
No they can't. Dr said Beta blockers and calcium blockers can help stop it!
Amazing info. Thank you!
I’ve been a heavy cannabis user for 30+ years and I’ve never had this happen to me. 🤷♂️.
My brother started suffering from delusional disorder a few years ago and we were told that it is difficult to treat but sometimes a person will work themselves out of the delusional, like only they hold the key to unlock themselves. As of mow my brother has talked about his delusions in about three year and his demeanor seems to be back to normal. I don’t bring up the past and never ask him what he was thinking a couple years ago. I thank god that he appears to be over it or at least in control of his thoughts
Amazing
Thank you so much! It is very informative and helpful ❤
It is cruel when they write off a real physical symptom who have been living with for years as “somatiform”
Spinal tap should always be performed in first episode psychosis with a sub acute onset to rule out autoimmune encephalitis, yet this is rarely done. Psychiatrist have no interest in cures for the diseases they treat.
