    Good video. Been fighting for CHC since March 2022 now October still not got it for my mum with PSP. System is designed not to fund people. Lesson learnt is not to have savings over £23250, don’t own your home. Then everything is paid for by the State. If you work hard all your life , own a house, have savings, pay taxes, always work, get sick in old age welcome to the £1200+ per week care bills .
    Worse if a couple living together in old age, and one gets sick , the State forces the other half to be an unpaid carer, and ultimately that carer will suffer health and mental damage. The whole system is unfair and a shambles.

      Sadly it is often the case that only those who shout the loudest get the support, and indeed having a professional support you with the process is definitely good advice for everyone.

    Good video. I am a young person who used to receive personal health budgets for around 3-4 months. I got around 200 quid a week from nhs to have homecare/community support to improve my well-being. It was brilliant. But the local commission team told me it was only temporary and it was stopped. Guess what, I've ended up getting poorly again. I Understand nhs is seriously in financial crisis but ppl like me with multiple and long term health issues are being forgotten sadly. Like you said, it is all down to money. The nhs commission team was honest and said they couldn't fund me long term due to funding issues. But then adult care aren't funded properly either to give people good homecare and community support. And sadly, disability funding like pip will not cover homecare services properly especially if you need 7 day homecare support from an expensive care agency.

      This is not surprising but sad none the less. The NHS does have a funding crisis, but it need not have. It is a political choice, and the result of that choice is that people in need of long term support are being denied it, replaced with a temporary fix, which just leads back to needing the support again. As you say adult care has also had its funding cuts so very little there also. Damned fi you do, damned if you don't. The people doing the work are generally helpful people who are having their hands tied, consequently lots of people leaving the NHS and Care services to work in other jobs.

    My mum has got dementia and type 1 diabetes. She is currently in hospital as she was forgetting to inject herself and went into DKA. They have tried to switch her insulin to twice a day and said when she is discharged the district nurses will come in to administer it . The problem is, her levels haven’t settled and it looks like she will need more than 2 injections… but I’ve been told the district nurses can only visit twice. Do you think she would qualify for CHC to either go in a home, or have live in carers?

      Has your mum seen a liver s to watch her bile flow nhs is a disgrace go private also motility tests damage to intestines slow movement and digesting food

      2 diffrent docs open valve in stomach to much or little acid damaged intestine and liver doc bile flow to little

      The risks of harm are fairly high with dementia+diabetes.It is clear that some kind of supervision is required if the levels aren't stable.
      The level of supervision and/or intervention needed determines eligibility.
      The key words here are primary health need,that are beyond the remit of social services to provide.
      Step 1 after hospital discharge is the completion of the initial checklist. if you score high risk in those domains,then you go to DST assessment, if it's low then they look at supported living/care at home/ self funded residential(with/without nursing)
      Having been through this obstacle course recently,I would back up what stu says,and make sure you REALLY do your homework if you have to go through the DST process.
      Make sure you know the LEGAL definitions of primary health need,and do study the relevant case law(ie coghlan case).You will need it because it is not uncommon for the severity of the ailments/risks to be underplayed.

    Is a CCG obliged to complete a 3 Month Assessment after the initial assessment of a patient transferred from a hospital to a nursing home?

    My mum is 92 and has terminal lung cancer. She currently has carers coming to house but her doctor said she will need more help as time advances. He is treating her as end of life as there is no offer of treatment. She has a suspected blood cancer too. Both myself and my husband struggle with her forgetfulness mental health depression snd incontinence. She has respiratory j problems due to lungs. Is she entitled to chc funding. I have bad health and so does my husband we are retired. Would be grateful for your advise. Thx

      CHC funding decisions are based on what medical treatment is required and whether nursing care is required so it will depend on those requirements rather than her conditions or care needs. Everything you have said so far only reflects as being care needs, but of course that situation may change. Rest assured that if CHC funding is viable it will be brought up with you by either the medical professionals or social services.

      If it is clearly ascertained that there is a realistic prospect of end of life,then your doctor should initiate what's called a fast track.

    I'm a student social worker and what I'd like to ask is, does chc only apply to people that have been discharged from hospital with long term health condition or medical needs? can an autistic person or someone with PMLD qualify without a health condition.? Thank you

      If that condition does not need continuing medical intervention outside of the hospital then they would not qualify, so ordinarily Autism and PMLDs do not qualify.
