    The fact that they design prosthetic legs with the idea that someone now has a foot reversed at their knee is absolutely genius. That way they can have a prosthesis that actually bends at the knee and they have full control over it.
    Just an absolutely mind blowing and clever solution.

      But the ankle doesn't fold anywhere as far as a knee will, so it looks rather limited, or can the ankle bend further with some practice or the prosthesis make up for it

    I got so upset when I saw the ankle being rotated 180 degrees, but I felt such relief when I saw they are getting a prosthesis. Mad respect to the doctor or surgeon who came up with this!

    I had this surgery nearly 20 years ago, I was 11 at the time and I had no idea what they where talking about. But I was amazed how easy it was to learn how to walk again, no trouble at all. Next to no phantom pain aswell. Really glad I was able to do this, instead of just chopping the leg off.

      Wow you my friend are a warrior, i just had a medical scare where i thought i might of had cancer but everything turned out to be okay. i reevaluated my life, i stopped smoking and improved my diet. Surgery is terrifying to me i wont lie, so my hat truly goes off to anyone who takes that hurdle and keeps fighting everyday.

      Dealing with it on an actual body is just as bad. I spent a whole dissection trying to isolate a branch of the brachial plexus (nerve organisation of the upper limb) from a cadaver and seeing it just dangling out there at the end was an experience.

    This is really an amazing surgery, I couldnt believe my eyes the first time I seen that news story about that young girl who danced and could still dance even after they surgery. Out of all the surgeries this one looks to affect live the least. Amazing job whoever designed it and those surgeons who perform it.

      the small bone of the lower leg (the fibula) is pretty much useless so they attach the other bone (the tibia) onto the femur, just like that

      The tibia (larger of the two) is cut and connected to the cut femur. The fibula is left whole and the extending portion is inserted into the thigh (unlike what is shown in this video).

    As a childhood cancer survivor I forget that people don’t know about it. I saw two kids on my ward that had this done. It’s really so innovative because you keep a real knee joint and the prosthetics have to be only below knee so it leaves way more mobility. I thought it looked a little funny the first time I saw it but when I saw the kid walk super well on prosthetics real soon it made me so happy. And after seeing it twice it’s really not that weird anymore ^^

      it's all about making it as easy as possible to use a prosthetic leg. Much better to have half your leg removed, but with a joint that lines up with your other knee, then to have a longer (but still shortened) leg you can't bend.

    As horrifying a thought that is backward foot as my new knee joint is, I'm super glad that this not only works but is very effective, apparently. Still, I will never unsee backward foot as a new knee joint. That will haunt me until the day I die, and hopefully no longer.

      I genuinely felt a little sick to my stomach for a bit and cringed. It's really offsetting seeing something that could've easily been made by a horror movie mad scientist, in a real life scenario. Don't get me wrong, if the person receiving this is ok with it and they get their life saved, good for them. But visually.....yikes.

    I thought this was pretty ridiculous at first, but yeah, I can see how it would be incredibly useful to have your foot, even if backwards, for the purposes of operating prosthesis. Whoever came up with this surgery was thinking outside the box.

      I imagine it'd be hella awkward to cope with the fact that your ankle has been turned into a knee and is now facing backwards, tho.
      I'm sure I'd be able to learn to work with it eventually, but it'd be super weird. Glad I still got both legs

    I was one of the first patients ever to have any kind of nerve-sparing joint extraction. My sense of relief having it all over with was incredible! I felt like giving my surgeon a high-five but was still learning to use the foot he'd attached to my arm.

    My first thought was "this seems ridiculous, why not just amputate up to the mid-thigh?" But now I can see that with a rotated foot that acts as a knee joint, someone with a rotationplasty will have a better range of motion. That's actually pretty dang cool!

      also it's easier to walk on since when they wrap muscle around the bone with the regular amputation procedure it's not ideal to put weight on basically raw bone with some muscle padding.

      @@Myemnhk phantom pain is the brain trying to elaborate feedback that just is not there (the video where someone have his hand hiddend, than a prop came in, and after some specular interaction the brain believes that it's recieving impulse from the prop hand instead of the real one, then the prop get stabbed with a fork and for a few instant the person feels actual pain).
      this procedure leaves a full set of main nerves there, so a lot of information is still the original feedback that prevents the "search" in the first place; it's unclear if it's completelly possible to remove phantom pain, but there's no phantom limb effect, as most of the limb, from a nervous prospective is till there and sending correct feedback to analise

      @@very7962 Litteraly the thigh is dead. You can't do anything to it and you need to amputate it otherwise your whole foot will be infected. What do you do? Cut the whole leg, pacient will have trouble using a prostethic. Doing this gives more control to the pacient while using a prostethic and also doesn't just throw away a perfectly functional body part. Also chemotherapy does't always work. In very bad cancer cases it's ineffective

    This is both horrifying yet fascinating at the same time. I applaud the scary genius of this.
    However, how does this work for adults? This video only talked about it as something for a child, but adults are susceptible to developing tumors in or near the knee joint too. I'd like to see how this works out for adults with the healing process and how the heel is matched up with the other knee joint.

      Only if they got the cancer in their knee and they were still a child. So it's probably not very common.

    i wonder what overall has the biggest immediate psychological impact on a patient, the entire loss of a leg that should be there, or a foot where a foot normally shouldnt be? it does save part of your leg and the legs mobility, but like, how many of us ever need to update our brains inner map of the body like this?

      I've studied my brain's inner map all of my life. Though I'm not a prosthetic user I have learned to be semi- ambidextrous as well as am able to remap my fingers to different keyboard keys without looking.
      I wonder, is it my fingers that have memorized the keys or is it my brain? Maybe both?

      Watching my mom battle cancer I have an understanding of this, someone will do any and everything they can to have a chance of a normal life. They are brave and courageous to be battling for whatever amount of health they can achieve.

    Fascinating. It's a brilliant choice to reuse the ankle as a new knee, but I'm curious about the ankle's ability to roll and swivel affecting it's role as a weight-bearing hinge. Will the prosthetic compensate by locking the ankle in place, or is some extra movement allowed for balance and positioning?

      The prostetic has joints that only allow a backward-bending motion / stretching straight.
      No extra movement is allowed or necessary and the ankle could just not provide the stability of a knee by itself.
      The position of a straightened leg would be like standing on your toes … it is possible but extremly unstable by itself.

    That was terrifying to see at first, but then I realized it's actually an amazing procedure. Normally someone who has a major joint removed like that would have serious mobility issues, but this procedure basically lets them substitute an existing joint in for the missing one. I imagine it's still quite a shock to the patient though, looking down and seeing their new leg, even if the procedure was explained to them beforehand. Sort of an instinctual "Nonono, the foot should not be turned that way!"

      I was horrified at first, and then I realized how amazing the procedure was. Then I saw what happened to the nerve and blood vessels at 0:36 and was horrified again.

    I can see how this leaves the patient with more function in the affected limb than amputation. No painful stump, and a new "knee" joint which is critical for walking and running. Amazing they can preserve function in the foot.
